Book excerpt: Contexte : 3,2 millions de décès seraient attribuables à la sédentarité, chaque année dans le monde. La promotion de l'activité physique (AP) est devenue un objectif de santé publique, et peut être réalisée sous forme de prescription structurée chez des patients à risque cardio-vasculaire. L'impact de ces interventions nécessite cependant l'établissement d'un référentiel pour leurs modes d'évaluation, souvent variés dans la littérature. Méthode : Cette revue opportuniste de la littérature a été réalisée par deux investigateurs indépendants sur les bases de données PubMed, Cochrane et Google Scholar. Les articles traitant de la prescription structurée d'AP chez le patient à risque cardio-vasculaire ont été analysés, à la recherche des différentes méthodes de mesures employées et au moyen d'une grille d'évaluation innovante. Résultats : 28 articles se démarquant par leur rigueur scientifique ont été inclus, dont presque la moitié portaient sur des populations exclusivement féminines. Un article sur deux évaluait également des populations accumulant plusieurs facteurs de risque cardio-vasculaires. Différents types de mesures étaient employés, et influencés significativement par la pratique régulière d'une AP. Anthropométriques, l'indice de masse corporelle, la fréquence cardiaque, la pression artérielle ainsi que la composition corporelle graisseuse étaient des indicateurs fiables, plus que le poids seul. Sérologiques, ressortaient le calcul de l'insulino-résistance ainsi que le dosage des fractions du cholestérol. Ces résultats pouvaient être combinés dans un calcul du risque cardiovasculaire à 10 ans, pertinent. Enfin, la capacité aérobie avec le calcul de la consommation maximale d'oxygène, ainsi que la mesure de la dépense énergétique par questionnaires, représentaient des alternatives efficaces. Accéléromètre et podomètre étaient surtout réservés à l'évaluation de l'observance de la prescription, ou comme outils motivationnels. Conclusion : Le choix de la bonne méthode d'évaluation doit être réfléchi. Reproductible, fiable et acceptable par le patient, elle représente un élément essentiel à l'accompagnement dans les changements de comportement. Standardisée, elle offre une meilleure comparabilité des interventions de promotion à l'AP.

Book excerpt: INTRODUCTION Au cours du XXème siècle, la sédentarité se développe au détriment de la pratique d’une activité physique. Les bénéfices d’une activité physique régulière sont connus, surtout chez les patients à risque cardiovasculaire (2ème cause de mortalité en France). La prescription d’une activité physique appartient aux traitements non-médicamenteux, encore difficilement acceptable par les patients et les médecins. Ce travail évalue la prescription d’une activité physique régulière en soins primaires chez les patients cardiovasculaires et recherche les freins et les difficultés associés. MATERIEL ET METHODES Les 38 médecins participants ont inclus 197 patients présentant un moins un facteur de risque cardiovasculaire, entre janvier et octobre 2010. Les réponses aux différentes questions mettent en évidence les freins et difficultés de la prescription par les médecins et de la réalisation par les patients. L’analyse des données est réalisée sur les logiciels Access® et Excel®. RESULTATS La non-prescription d’activité physique était minoritaire (26%). Les freins sont d’origine mixte : patient et médecin. Les freins des patients sont majoritairement le résultat d’un manque de temps (au niveau professionnel et/ou personnel), d’un défaut de motivation et d’une absence d’infrastructure. Les freins des médecins sont les supposées du patient (inobservance, manque de motivation) ainsi que leurs propres limites à la prescription d’une activité physique. Ces difficultés sont marquées par le manque de temps en consultation, la priorité d’équilibrer les facteurs de risque cardiovasculaires, les échecs antérieurs de cette prescription. CONCLUSION Les freins à la prescription d’une activité physique sont l’association des limites propres au patient et des représentations du médecin

Étudier les facteurs associés à l'évolution de la pratique d'activité physique après prescription. Méthodes : étude quantitative interventionnelle par suivi de cohorte. Des patients hypertendus et / ou diabétiques non insulino-dépendants et / ou hypercholestérolémiques ont été inclus par un échantillon de médecins généralistes et se sont vus remettre une prescription écrite d'activité physique. Leur niveau d'activité physique a été mesuré à l'inclusion, puis à 1 mois et 3 mois de suivi par le questionnaire IPAQ version courte. Le score IPAQ moyen à 1 mois et à 3 mois a été comparé au score IPAQ à l'inclusion. Une analyse bivariée a été réalisée pour déterminer les facteurs associés à l'évolution du score IPAQ après prescription. Résultats : l'activité physique n'augmentait pas significativement à 1 mois (médiane +566 METmin/semaine, IQR 25-75 : -921 - 1640 p = 0,12), ni à 3 mois (+436 MET-min/semaine (IQR 25-75 : -789 - 1960), p = 0,073) après prescription sur la totalité de l'échantillon. Néanmoins, elle augmentait significativement chez les patients initialement évalués comme « sédentaires » (+833 MET-min/semaine, IQR 25-75 : 421 - 1897 p = 0,007) ou « modérément actifs » (+482 METmin/semaine (IQR 25-75 : -314 - 2674, p = 0,005) Conclusion : la prescription d'activité physique augmente le niveau d'activité physique à 3 mois des patients initialement sédentaires et modérément actifs. La création d'un outil simple de repérage des patients très actifs permettrait d'optimiser la prescription d'activité physique en médecine générale.

Ce comportement de santé peut être modifiable avec l'établissement de stratégies de promotion à l'AP comme peuvent l'être les conseils délivrés par les médecins. L'évaluation de leur efficacité est un temps important mais la multiplication d'outils disponibles rend celle-ci plus difficile. Méthode : Une revue de littérature réalisée par deux investigateurs indépendants sur la base de trois banques de données PubMed, Google Scholar et Cochrane Library a été menée de juillet à novembre 2013. L'objectif principal était de répertorier l'ensemble des méthodes d'évaluation des conseils d'AP, chez le patient à risque cardio-vasculaire, grâce à l'élaboration d'une grille de sélection et d'une grille composite adaptée des grilles de Jadad et de Newcastle-Ottawa. Résultats : Au total, 1914 articles ont été sélectionnés mais seulement 32 articles portants sur les conseils simples ont été inclus. Les populations étudiées étaient sédentaires pour la plupart, présentant un ou plusieurs facteurs de risque cardio-vasculaire. Vingt-trois études utilisaient les questionnaires comme outil d'évaluation. Parmi celles-ci, 19 s'en servaient pour évaluer l'activité physique en tant que critère principal. Si l'utilisation des questionnaires était fréquente car d'utilisation simple, les résultats concernant l'AP pouvaient varier d'une étude à l'autre. Les mesures anthropométriques constituaient le second outil le plus utilisé avec en premier lieu la pression artérielle, le poids, le tour de taille et enfin l'indice de masse corporelle. Les mesures biologiques moins fréquemment utilisées dans cette sélection étaient tout de même privilégiées lorsqu'il s'agissait de patients dyslipidémiques (statut lipidique) ou diabétiques (glycémie, hémoglobine glyquée). Les moins utilisés étaient les calculs de dépenses d'énergie, l'accéléromètre ainsi que le calcul du score de Framingham. Enfin, il faut souligner qu'une seule étude choisissait le podomètre dans un but spécifique d'augmenter l'AP, alors que les autres l'utilisaient pour favoriser l'adhésion du patient. Conclusion : Plusieurs outils de mesure ont été identifiés dans cette revue de littérature. Pour leur utilisation en pratique courante, et en médecine générale, une hiérarchisation semble nécessaire en tenant compte de leur pertinence, et de leur faisabilité.

Book excerpt: L’importance de l’activité physique régulière pour notre santé a clairement été démontrée dans différentes études, comme le montre le rapport de l’INSERM de mars 2008[28], et notamment dans le cadre des maladies cardio-vasculaires que ce soit en prévention primaire ou secondaire. Ceci est d’autant plus important que les maladies cardio-vasculaires sont la première cause de mortalité dans les pays développés. Or, dans ces pays, l’évolution des modes de vie fait qu’on observe une diminution de l’activité physique que ce soit dans le milieu professionnel ou dans la vie courante. Ainsi, moins de la moitié des Français pratique une activité physique favorable à la santé[9]. La lutte contre la sédentarité est donc un problème de santé publique et entre dans le cadre de la prescription médicale. Ainsi, dans le cadre du projet APSP (Activité Physique en Soins Primaires) mis en place par le département universitaire d’enseignement et de recherche de médecine générale de Créteil, une étude pilote, menée à l’aide d’un questionnaire-test (visant à explorer les freins et les difficultés à la prescription d’une activité physique régulière en soins primaires), a été testée en situation réelle sur un petit échantillon de patients. Cette étude préliminaire a donc permis d’apporter quelques résultats permettant d’appréhender les obstacles au suivi d’une activité physique régulière mais aussi d’évaluer l’impact immédiat sur la motivation des patients. Elle a permis de créer deux autres questionnaires (questionnaire à 1 mois et questionnaire à 3 mois) afin d’évaluer l’impact de la consultation à court et à moyen terme.

It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges? What future developments and studies are needed in the field?
