Book excerpt: Recent advances in techniques and understanding in the fields of genetics, embryology and reproductive biology have opened up new ways to treat a wide range of medical problems. They range from new options for infertility treatment and pre-implantation genetic diagnosis to stem-cell-based therapies for debilitating diseases. Since all these approaches involve the manipulation of human gametes, embryos or embryonic cells, and could also permit more contentious uses, they have stimulated a controversial debate as to what aims are desirable and to what extent experiments on human embryos are morally permissible, if permissible at all. The situation is further complicated by the fact that scientific projects are increasingly realized through international co-operation and that patients are increasingly ready to seek morally contentious medical treatment wherever it is available and thus to bypass national legislation. In view of this situation the Europäische Akademie assembled a temporary interdisciplinary project group in which scientists from universities and non-university research organizations in Europe working on the relevant subjects were brought together and charged with establishing a knowledge base and providing suggestions for long-term solutions that would be acceptable for society. Presented here are the results of this project, ranging from a discussion of the theoretical and practical possibilities in human-embryo experimentation and its alternatives in research on adult stem cells, a comparison of the situations and prospects of regulation of embryo research in Europe, a survey of European public attitudes, and a philosophical analysis of the arguments and argumentative strategies used in the debate.

As such examples suggest, the activities of many governmental groups and professional advisory bodies, although varied, tend to converge upon a number of especially important issues. If one peruses the index of documents discussed in Volume Four, certain topics are more often the focus of legislation and official concern than others: withholding and withdrawing treatment, access to health care, consent to treatment and experimentation, and issues posed by HIV testing and AIDS. Such a common focus should not be exaggerated, for the discussion of topics is wide-ranging. But that commonality, when in evidence, is also not surprising. It suggests that key issues and concerns in bioethics may be widely shared among modern cultures and societies, for all the distinctiveness of a particular nation's or region's response to them. Issues of informed consent, after all, implicate more fundamental matters of respect for persons and the rights of individuals in the contexts of therapy and research. Issues of access to medical care concretize deeper questions about the nature and scope of a society's welfare obligations to its citizens.

The short title of the project was Euroscreen, thereafter known as Euroscreen I, in the light of the fact that a second project on genetic screening was subsequently funded. The project was multinational and multidisciplinary, and had as its objectives to examine the nature and extent of genetic screening programmes in different European countries; to analyse the social policy response to these developments in different countries; and to explore the applicability of normative ethical frameworks to the issues. The project was led by a core group who had oversight of the project and members of which have acted as editors for this volume. Darren Shickle edited the first section; Henk ten Have the second; Ruth Chadwick and Urban Wiesing the third and final part. The volume opens with an overview of genetic screening and the principles available for addressing developments in the field, with special reference to the Wilson and Jungner principles on screening. The first of the three major sections thereafter includes papers on the state of the art in different countries, together with some analysis of social context and policy.
