Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from Journal of the Royal Army Medical Corps, Vol. 30: April, 1918 Heart. In spite of the most Searching investigations during the past three years for some definite finding to explain the pathology of this incapacitating syndrome, we are still without exact knowledge as to its cause. If one desires a clinical description of the condition, it is hard to improve upon that Of Da Costa. On the other hand, if an analysis of the symptoms, and the study of their possible pathological basis is sought after, the Report Of the Medical Research Committee of the work Of the Military Hospital at Hampstead supplies the latest scientific information on the subject. Before'entering upon deductions from the results of my own clinical observations, I will give a description of the incapacity from which the soldier suffers. The picture varies, but the fundamental factor is, the soldier fails to endure a moderate amount of effort without collapse or fainting; and may be unable to resist exhibiting similar symptoms when under stress of even moderate emotion. The usual history is somewhat like the following I am alright if I go slow, but if I have to try to march with the pack after-i have gone, say half a mile, or on the first bill, I begin to feel my heart thumping. Then I get a stabbing pain at the heart, I have a choking sensation in the throat, my eyes get mazy, I feel dizzy, and if I go on I stagger, and then I fall. The man will then say he gets the same syndrome on attempting to double, and sometimes volunteers that he has a similar experience on putting on his gas helmet. The history is the same in substance; but the description Of the man varies as to the principal or prominent feature, i.e., pain, dyspnoea, palpitation, vertigo, weakness, fainting, exhaustion, etc. The soldier also 'often complains of the same symptoms occurring just as he is going to sleep, or his waking with the pain and palpitation and choking sensation. Many other facts connected with this syndrome become evident through the histories Of the patients, and an interesting one is how Often these men have carried on. In spite of their symptoms. It was a common enough story of the man that he managed for months to get in and out of the trenches by taking his own time, and resting on the way up. Another point Of interest in the symptomatology was that, if the man did fall out on a hill, Often a short rest enabled him to follow on and finish the march. I did not usually have the Opportunity to see these men in the fainting condition, but have seen some when they have been lying after falling out, and the feature was, the good colour Of the man, with the hurried respiration, profuse sweating, and tachycardia. Book excerpt: Excerpt from Journal of the Royal Army Medical Corps, Vol. 25: August, 1915 It was hoped by the authorities in Egypt that the preventive measures now being applied to ankylostomiasis would also prove efficacious in dealing with bilharziosis. These comprise free treatment, the introduction of conservancy, and the dissemination of knowledge of the disease amongst the natives. Such measures would appear, however, to be of little value in the control of bilharziosis, for the following reasons. Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from Journal of the Royal Army Medical Corps, Vol. 31: July, 1918 I was led to this inquiry by 'my previous work on the toxic action of Copper compounds of amino-acids and in particular copper-alanine on protozoa For, in the early part of last year, in some preliminary experiments, the Opportunity for which was kindly afforded me by Professor Hewlett in the Bacteriological Laboratory, King's College, a toxic action of copper-alanine (one part in of water) was found on the meningococcus also, on thirty minutes' exposure. In further experiments carried out in association with Lieutenant Colonel Mervyn Gordon ahd Major Hine, at the Central Cerebrospinal Laboratory, Royal Army Medical College, it was shown that the meningo coccus did not survive in concentrations of copper-alanine 1 in in sodium chloride solution, on twenty minutes' exposure, but in less con centration or in broth or serum, no toxic effect was observed in this time limit. Experiments were also made with the copper-alanine in Ringer Locke solution. A somewhat more toxic effect was witnessed, as in this case the meningococcus did not survive in 1 in copper-alanine on twenty minutes' exposure. No toxic effect was observed on five or ten minutes' exposure, and as for purposes of local treatment, intrathecal or naso-pharyngeal, a short time limit is obviously essential, the above results did not indicate any special advantage in the employment of copper alanine as a bactericide in the treatment of cerebrospinal meningitis. Nor could the solutions of 1 in in ringer-locke fluid be regarded as suitable for intrathecal use without fear of injury to the delicate tissues of the central nervous system. Book excerpt: Excerpt from Journal of the Royal Army Medical Corps, Vol. 25: November, 1915 The necessity for absolute rest and warmth to the abdomen, as by means of a light linseed poultice sprinkled with laudanum, need scarcely be insisted upon, but the Committee would again emphasize the value of careful washing of the stools and a search for sloughs. It would also remind medical and sanitary officers that so far as amoebic dysentery is concerned, the post-dysenteric case with formed stools or slight diarrhoea is much more dangerous to the community than the man suffering from acute dysentery. It is in the former that the resistant cysts are found which may infect food, water, dust, and ies, and find their way by one or other of these channels into the human host. Emetine is apt to fail in the eradication of these cysts, but this is by no means always so. The following measures now employed to protect France from the introduction of amoebic dysentery by convalescents from her Eastern Colonies are worthy of note, and might be followed with advantage. Four or five grains of emetine hydrochloride are dissolved in tincture of opium in the strength of one in fifteen. Eight to ten drops of this mixture are added to a cup of strong tea and given nightly, emetine hypodermically being reserved for acute cases. Such a measure would seem advisable in the case of men getting ready to leave hospital and in convalescent and rest camps. General sanitary measures which are of the highest importance.
