Book excerpt: HIV/AIDS care and prevention activities in sub-Saharan Africa are mainly the result of collaborations between health care providers (physicians, nurses, and mental health professionals) and traditional healers (Bodeker, Carter, Buford, & Dvorak-Little, 2006; Sukati, 1996). Traditional herbal and spiritual practitioners (also known as iSangoma and iNyanga) have shown little inclination to adopt antiretroviral therapies to treat their HIV/AIDS patients (Shuster, Sterk, Frew, & del Rio, 2009). The proposed qualitative pilot study will examine the background of this reluctance, based on experiences of health care providers who are interacting with traditional healers in Swaziland. This pilot study will use qualitative methods to evaluate data collected from focus group discussions. The information will be collected from Swazi care givers attending an HIV/AIDS workshop sponsored by the United States President's Emergency Plan for AIDS Relief (PEPFAR). The data obtained from this pilot study proposes to improve understanding, and communication, between collaborating health care providers and traditional healers with the specific aim of propelling the incorporation of antiretroviral therapies in the treatment regime for HIV/AIDS patients in Swaziland.

The high rate of infection, coupled with the fact that many South Africans do not have access to adequate healthcare, has led to a crisis that the government has largely been unable to address. After witnessing this health care crisis firsthand while studying and volunteering in Cape Town, I became interested in alternative methods to address this situation. Fortunately, there exists a largely untapped resource in the fight against HIV and AIDS: traditional healers, also known as sangomas. Many South Africans consult sangomas in conjunction with western healthcare providers or as a primary care provider. Though not able to administer medical treatments in a western sense, sangomas could prove to be invaluable resources for educating the population about HIV and AIDS, especially in regards to prevention and adherence to anti-retroviral therapy regimens. Because they are well respected in their communities and share a culture with the population they are serving, sangomas have a unique opportunity to connect with their patients and make a difference in the fight against HIV/AIDS.This thesis seeks to provide an overview of the complicated role of traditional medicine in modern South Africa, while highlighting programs that have attempted to navigate the divide between traditional and western medicine. Small-scale programs have made great strides toward incorporating traditional healers in HIV prevention and treatment programs in South Africa, but little action has been taking on a national level to increase cooperation. By analyzing the successes and failures of such programs, I hope to contribute to the formation effective versions of these initiatives that can be replicated on a larger scale.

Largely in response to the HIV/AIDS pandemic, and limited both by the number of practitioners and by patients’ access to treatment, biomedical practitioners looked toward the country’s traditional healers as important agents in the development of medical education and treatment. This collaboration has not been easy. The two medical cultures embrace different ideas about the body and the origin of illness, but they do share a history of commercial and ideological competition and different relations to state power.Healing Traditions: African Medicine, Cultural Exchange, and Competition in South Africa, 1820–1948provides a long-overdue historical perspective to these interactions and an understanding that is vital for the development of medical strategies to effectively deal with South Africa’s healthcare challenges. Between 1820 and 1948 traditional healers in Natal, South Africa, transformed themselves from politically powerful men and women who challenged colonial rule and law into successful entrepreneurs who competed for turf and patients with white biomedical doctors and pharmacists. To understand what is “traditional” about traditional medicine, Flint argues that we must consider the cultural actors not commonly associated with African therapeutics: white biomedical practitioners, Indian healers, and the implementing of white rule. Carefully crafted, well written, and powerfully argued, Flint’s analysis of the ways that indigenous medical knowledge and therapeutic practices were forged, contested, and transformed over two centuries is highly illuminating, as is her demonstration that many “traditional” practices changed over time. Her discussion of African and Indian medical encounters opens up a whole new way of thinking about the social basis of health and healing in South Africa. This important book will be core reading for classes and future scholarship on health and healing in South Africa.

Book excerpt: Most African countries have ratified the African Charter on Human and People's Rights, which states the rights to health, education, and promoting, safeguarding, and reinforcing mutual respect and tolerance. In Sub-Saharan Africa alone, some 17.2 million have died of HIV/AIDS. The focus here is, therefore, on how traditional healers and indigenous knowledge can empower communities to deal with the social consequences of the pandemic, and, at the same time, be able to ensure that government resources are actually being used to their benefit. This Notes suggests a mini epidemiological study be undertaken by community member and for the community. The process of community-managed research could be facilitated by traditional healers, local NGOs to balance any attempt by local elites to dominate the process.
