Se estima que existe un déficit de casi 4,3 millones de médicos, parteras, enfermeras y trabajadores auxiliares en todo el mundo, que reviste la máxima gravedad en los países más pobres, sobre todo del África subsahariana, que es donde más se necesitan. Las malas condiciones de empleo, las altas tasas de pérdida de efectivos por enfermedad y migración, y unos sistemas de enseñanza que no dan abasto reflejan la gravedad de los problemas que sufren esos países en crisis.La fuerza de trabajo sanitaria mundial asciende a unos 59 millones de personas. Una de cada tres está empleada en las Américas - la mayoría en los Estados Unidos y el Canadá -, donde se concentra más de la mitad de los recursos financieros destinados a la salud en todo el mundo. Sin embargo, sólo cuatro de cada cien trabajadores sanitarios se encuentran en el África subsahariana, que alberga una cuarta parte de la carga mundial de morbilidad, pero menos del 1% de los recursos financieros mundiales.En este informe se indica la manera de lograr un mayor equilibrio. Planificando activamente los detalles de la vida laboral de los trabajadores desde el momento de su incorporación a la formación sanitaria, pasando por la contratación, hasta la jubilación, cobra forma un amplio conjunto de acciones de política que pueden contribuir palpablemente a mejorar el desempeño de los trabajadores sanitarios. Todos los países pueden mejorar la productividad de sus trabajadores de inmediato, pero deben también empezar a prever lo que depara el futuro y adquirir la capacidad institucional necesaria para dirigir, gestionar y regular a los trabajadores sanitarios.El liderazgo nacional requerido para ello puede respaldarse mediante una solidaridad mundial que aporte bienes públicos, como marcos técnicos comunes e investigaciones prioritarias, y propicie acuerdos de colaboración en materia de migración y movilización de los trabajadores internacionales en respuesta a las emergencias sanitarias. A fin de ayudar a los países que atraviesan una crisis profunda, se precisa la coordinación y el compromiso urgentes de los asociados internacionales para invertir directamente en los trabajadores sanitarios, ya mismo y a largo plazo. En el presente informe se establece un plan de acción a diez años en el que los países pueden desarrollar su fuerza de trabajo sanitaria y potenciar sus sistemas de salud con el apoyo de asociados mundiales: colaborando por la salud.Una fuerza de trabajo robusta y enérgica constituye una inversión en salud para el presente y para el futuro.

En ellibro se examinan las politicas de cobertura universal de salud de cada pais alrededor de tres temas comunes: la economia politica y el proceso de forrnulacion de la polltica para adoptar, lograr y mantener la cobertura universal de salud; las politicas de financiacion sanitaria para mejorar la cobertura sanitaria; y los recursos humanos para las politicas de salud para lograr la cobertura universal de salud. Los hallazgos de estos estudios de pais tienen por objeto proporcionar enseiianzas que pueden ser utilizadas por los paises que aspiran a adoptar, tograr y mantener la cobertura universal de salud. Aunque el camino ala cobertura universal de salud es especifico para cada pais, los paises pueden beneficiarse de las experiencias de otros al conocer acerca de diferentes enfoques y evitar los riesgos previsibles.
