Lack of confidence has been described as a source of uncertainty during the role transition from student nurse to practicing nurse. Cognitive apprenticeship (CA) is an instructional strategy that can facilitate ease of transition for newly graduated nurses (NGNs) during the clinical or preceptorship portion of hospital orientation. The purpose of this mixed methods pretest-posttest design was to explore if there would be a difference in confidence and ease of transition to practice, after completing a cognitive apprenticeship based hospital orientation. Newly graduated, newly hired registered nurses as well as experienced nurse preceptors were solicited for participation in this study. The Casey-Fink Graduate Nurse Experience Survey was administered as both the pretest and posttest. Qualitative data were also collected from focus group interviews of both newly graduated nurses and preceptors. The findings indicated that there was a statistically significant difference in confidence after a CA-based orientation. Furthermore, the findings revealed the importance of preceptor training for effective preceptorships. This study adds to the literature regarding transition to practice strategies as well as pedagogical strategies for preceptorships.

Book excerpt: The transition from student nurse to professional nurse is often difficult for the new graduate especially if transitioning into critical or progressive care areas of practice. It is imperative for new graduates to receive a clinical orientation that meets their educational and transitional needs. The orientation process should provide them with a strong foundation in critical or progressive care practices. Educators are challenged to provide educational opportunities while accommodating diverse learning needs and continuing to maintain competency that ensures the highest quality of patient care and improved patient care outcomes. New graduate nurses hired into the critical or progressive care areas were asked to participate in a study evaluating the effect of blended orientation approaches on their level of self-confidence and satisfaction with current orientation methods. Their level of self-confidence and satisfaction were evaluated and measured using a pretest and posttest questionnaire. New graduates entering critical or progressive care areas of practice were evaluated following nursing school (prior to hospital orientation) and following a two-month hospital orientation model. The results did not reveal any significant difference between pre and post orientation. The review of literature suggests that the use of a more blended learning approach to teaching and learning can be beneficial. Blended learning can include online learning, case studies, human simulation, clinical time with a preceptor, service-based classroom lectures, quizzes, games, videos, mock events, review of pertinent evidence-based articles, and demonstration with return demonstration (p. 10), and is a growing area in education requiring more evaluation and research.

It is important for hospital educators and administrators to understand Benner's stages and model their orientation programs appropriately. The Revised Casey-Fink Graduate Nurse Experience Survey was used to query new nurses (less than 2 years of experience) and their preceptors. Results indicate that new nurses feel uncomfortable performing certain skills and communicating with doctors. They report feeling a lack of support after their orientation period. The results support a change in the orientation process and preceptor education.
