Book excerpt: Introduction : La base du crâne centrale sépare le compartiment intracrânien et les espaces extracrâniens rhinopharyngés et sinusiens. Les pathologies de cette région sont variées et peuvent se développer à partir des éléments intracrâniens, extracrâniens ou de la base du crâne elle-même. Elles sont rares, surtout chez l'enfant. La voie cranio-faciale a été la voie d'abord chirurgicale de référence avant le développement de la voie endoscopique endonasale. L'objectif de cette thèse était de déterminer les spécificités diagnostiques et thérapeutiques chirurgicales des diverses pathologies de la base du crâne centrale de l'enfant, pour l'Oto-Rhino-Laryngologiste. Matériels et méthodes : Il s'agit d'une étude rétrospective de dossiers de patients opérés par l'ORL éventuellement en association avec le Neurochirurgien à l'Hôpital Pédiatrique de la Timone, entre 1995 et 2017. Les patients ayant eu une lésion de la base du crâne centrale traitée chirurgicalement ont été inclus. Résultats : Nous avons inclus 51 patients. L'âge moyen était de 10 ans au moment de la chirurgie (de 19 jours de vie à 19 ans). Dix-sept patients avaient une pathologie tumorale. Les signes cliniques variaient en fonction de la localisation tumorale (signes rhinologiques, ophtalmologiques, détresse respiratoire). Les fibromes nasopharyngiens étaient réséqués par voie endoscopique endonasale 24 heures après une artérioembolisation. Les lésions intrasellaires étaient aussi abordées par voie endoscopique endonasale. Les lésions intrinsèques de la base du crâne (fibrome ossifiant) ou ayant un contingent intracrânien important avaient nécessité un abord transcrânien ou un abord combiné. Treize patients avaient une déhiscence de la base du crâne. Celles-ci peuvent se manifester par une rhinorrhée claire chronique, une obstruction nasale (méningocèle/méningoencéphalocèle) ou des épisodes de méningite. Pour les kystes et/ou fistules du dos du nez, une communication avec le compartiment intracrânien est à rechercher avec un scanner et une IRM ; un abord transcrânien est à prévoir si une telle communication est suspectée. Les brèches sont réparées par voie endoscopique endonasale. Un abord combiné transcrânien et endonasal ou endonasal seul peuvent être choisis pour les méningocèles/méningoencéphalocèles. Parmi les 21 patients qui avaient une pathologie infectieuse, 19 avaient une complication intracrânienne de sinusite aiguë (empyème extra ou sous-dural, abcès cérébral, thrombophlébite cérébrale). Un drainage par voie transcrânienne était effectué en cas de collection volumineuse, de déficit neurologique, d'hypertension intracrânienne ou d'échec d'un drainage sinusien ou d'une antibiothérapie seule. Conclusion : Les signes cliniques des pathologies de la base du crâne sont variés mais certains sont spécifiques à une catégorie d'affections. L'abord endoscopique endonasal permet une prise en charge chirurgicale efficace des pathologies de la base du crâne centrale, même chez les nourrissons. Cependant, elle ne remplace pas l'abord transcrânien, notamment pour les extensions tumorales intracrâniennes importantes, pour certaines méningocèles et méningoencéphalocèles, pour les empyèmes ou abcès cérébraux volumineux.

Book excerpt: Depuis le début des années 1990, l'imagerie de l'oreille et de la base du crâne s'est considérablement développée. Des pathologies récentes, mal connues ont été explorées en imagerie, en particulier en IRM. L'objectif de mes travaux de recherche, a été de développer des séquences Haute Résolution (HR) sur l'oreille et la base du crâne permettant une analyse sémiologique fine et une caractérisation de ces différentes pathologies dans l’objectif d’une prise en charge plus adaptée. Le début de mes travaux a été orienté vers le conduit auditif interne. Mon but initial a été de mettre point et de développer une séquence 3 HR T2 sur l'oreille interne et le conduit auditif interne ainsi qu’une séquence spin écho T1 HR en coupes fines. Ces séquences ont permis l’analyse précise de l'extension des schwannomes vestibulaires dans le fond du conduit auditif interne. Cette étude a permis de dégager des critères intéressants en imagerie dans le choix thérapeutique (chirurgie versus radio-chirurgie) et dans le choix de la voie d'abord chirurgicale. Mon travail s’est également orienté vers le développement des séquences de diffusion en base du crâne, le but était de développer une séquence de diffusion HR dans le suivi des cholestéatomes opérés de l'oreille moyenne. Les récidives de cholestéatome sont fréquentes et les oreilles opérées sont parfois difficiles à analyser en TDM en cas de comblement de la cavité opératoire. Des séquences IRM spécifiques sont alors nécessaires pour différencier une récidive de cholestéatome, du tissu fibro-inflammatoire fréquemment présent. Plusieurs auteurs avaient montré l'intérêt des séquences de diffusion du fait de coefficients de diffusion très différents entre le tissu fibro-inflammatoire et le cholestéatome. Cependant ces premières séquences de diffusion EPI basse résolution, étaient peu adaptées au rocher et la base du crâne. Notre travail a été de mettre au point et de développer une séquence de diffusion HR Turbo Spin écho adaptée à l'oreille et à la base du crâne permettant le dépistage des récidives de cholestéatome et de valider cette séquence par comparaison au gadolinium tardif et à la chirurgie. Mon travail sur la base du crâne haute résolution m'a également permis de développer des séquences Spin Echo T1 et 3D Echo de gradient T1 en Haute Résolution avec saturation de la graisse permettant une analyse fine des extensions tumorales périnerveuses à la base du crâne principalement dans les cancers du nasopharynx. Ces travaux ont permis de définir les voies d'extension précises de ces tumeurs afin de ne pas sous-évaluer le stade initial de ces tumeurs dans la classification TNM de l'UICC. Ces travaux associés à ceux d'autres chercheurs ont permis de mettre à jour récemment cette classification UICC des tumeurs du nasopharynx. Une de mes principales voies de recherche a été le dépistage, l'analyse et la description des pathologies de l’oreille interne. Une étude longitudinale menée de 1998 à 2008 incluant toutes les pathologies intra-labyrinthiques découvertes au CHU de Lille, a permis de définir et de caractériser trois grands types de pathologies distinctes: les schwannomes intra-labyrinthiques, les labyrinthites, les hémorragies intra-labyrinthiques. Ces pathologies nécessitent des prises en charge différentes. Nous avons pu également définir des critères pronostics en particulier des critères de gravité nécessitant une prise en charge rapide adaptée. Dans le cadre ce travail de recherche sur les pathologies de l’oreille interne, nous nous sommes également intéressés au Syndrome de déhiscence du canal semi-circulaire supérieur, syndrome clinique récemment décrit par Minor en 1998. Une étude précise des cas découverts entre 2007 et 2009, associant le scanner HR de l’oreille et la fusion des images HR en IRM (fusion entre une séquence d’angio-IRM HR et la séquence 3D HR T2), a permis de mettre en évidence une variante de la forme classique, correspondant, non pas à une déhiscence osseuse isolée du canal semi-circulaire supérieur, mais à une déhiscence du sinus pétreux supérieur venant au contact du labyrinthe membraneux au niveau du canal semi-circulaire supérieur. La prise en charge chirurgicale étant très différente en cas de déhiscence vasculaire. Mon travail sur la base du crâne et l'imagerie haute résolution m'a également conduit à m'intéresser aux outils d'aide à la planification et à la prise en charge chirurgicale des tumeurs de la base du crâne. En effet, ces tumeurs, souvent bénignes, sont d'accès difficile en raison de l'importance des rapports nerveux et vasculaires à ce niveau. Le but est de développer une méthode de reconstruction tridimensionnelle de ces tumeurs dans leur environnement vasculaire et nerveux permettant au chirurgien de modéliser et de choisir sa voie d'abord. Ces reconstructions 3D des tumeurs de la base du crâne et de leur environnement montrent l’intérêt de l’imagerie HR de la base du crâne non seulement pour les radiologues mais également pour les chirurgiens et pour l’enseignement de cette région.
