Book excerpt: Excerpt from Clinical Lectures on Certain Acute Diseases The design of the lectures published in the present volume, is to describe and illustrate by examples the clinical history and treatment of the more important acute diseases. There will (the author believes) he found in the following pages evidence enough to show that the ordinary so-called antiphlogistic treatment is unnecessary (to say the least) for the cure of acute internal inflammations; and that the supposed necessity for such treatment rested upon an untenable hypothesis respecting the nature of inflammation and of fevers, and cannot be regarded as a legitimate induction from accurately observed clinical facts. The conclusions, which the clinical observations detailed in the lectures tend more or less to establish, may be summed up in the following propositions: - 1. That the notion so long prevalent in the schools, that acute disease can be prevented or cured by means which depress and reduce vital and nervous power, is altogether fallacious. 2. That acute disease is not curable by the direct influence of any form of drug or any known remedial agent, excepting when it is capable of acting as an antidote, or of neutralizing a poison, on the presence of which in the system the disease may depend (materies morbi). 8. That disease is cured by natural processes, to promote which, in their full vigor, vital power must be upheld. Remedies, whether in the shape of drugs, which exercise a special physiological influence on the system, or in whatever form, are useful only so far as they may excite, assist, or promote these natural curative processes. Book excerpt: From the Medical Bulletin: A Monthly Journal of Medicine and Surgery, Volume 3 [1881]: This admirable work forms the sixth volume of the "Library of Standard Authors" for the current year. It contains twenty-one chapters by Prof. Charcot, devoted almost exclusively to the consideration of gout and rheumatism, the latter three bearing specially on the value of thermometry in old age. The important subject of gout is handled in a masterly manner by the distinguished pathologist, and it is undoubtedly true that the gouty diathesis underlies not alone the diseases to a great extent of old age in this country, but also those of the middle or mature period of life, and in this respect the value of the book to the general practitioner will be at once apparent. In this connection the concomitant diseases of gout as described are of much interest, and the proper method of study and diagnosis is clearly laid down. Although, as is well understood, the disease is greatly modified by climate and locality, and the volume was written for French territory, a careful perusal of the work will throw much light upon maladies which in many instances are to American practitioners not alone of doubtful origin but difficult to manage. Many obscure cases now ascribed to malarial poisoning can be readily cleared up in the light thrown upon them in this work. Rheumatism, in its varied phases, receives the attention it deserves, the relation between the acute articular form and gout, as also chronic and visceral types, being thoroughly described, in a succinct yet comprehensive manner. The treatment of these disorders is discussed at length, due stress being laid on the value of the older remedies, colchicum, iodine and its combinations, etc., but the now fashionable salicylates are not mentioned. The ten concluding lectures are by Prof. Alfred L. Loomis, of New York, and form an exceedingly interesting and valuable part of the volume. Pneumonia, catarrh of the bronchi, asthma, fatty heart, and the important gastro-intestinal disorders of advanced age are taken up in the author's forcible style, and a vast fund of practical information is condensed in a comparatively few pages. The cerebral senile disorders are also given due prominence, and the troublesome disorders of the bladder and prostate are treated in the concluding chapter, which also refers to ammonææmia, a disorder seen frequently in the old, but about which not much is said in works in general practice. Twenty-eight wood-cuts are introduced in the text, and fifteen lithographic figures close the work, the latter not being up in coloring to those in some recent dermatological treatises, but they are nevertheless equal to the ordinary efforts in this direction. The book is a decided success, and will be of real value to the conscientious physician. The translator, Dr. Leigh H. Hunt, of New York, has done his work faithfully and skillfully.

Book excerpt: Excerpt from Clinical Lectures The Clinical Lectures of Dr. Graves, which were originally published in the weekly periodicals of London, were-first printed in their present form in the American Medical Library. They have excited so much interest in this country, and contain so much practical and interesting matter, that a new edition was thought advisable. The publishers of the present Edition thought that a few notes and some additional lectures would not lessen the value of the work, and requested the American Editor to republish a few of the Clinical Lectures given by him at the Philadelphia Medical Institute, and at the Philadelphia Hospital, as a part of the Clinical Course of the University of Pennsylvania. The circumstances which attended the publication of this edition restricted the number of these Lectures, and increased the disadvantages under which they appear, as compared, and, it is feared, contrasted with the Lectures of Dr. Graves. An apology seems necessary for the style as well as the substance of the Lectures; with a few exceptions they were not written by the author, but were noted as they were delivered, and afterwards corrected, so that they are bona fide Clinical Lectures. The Lectures which were given at the Philadelphia Hospital contain some expressions which would not be readily understood without a knowledge of the mode of conducting the Clinical Course. Book excerpt: Excerpt from Geriatrics: The Diseases of Old Age and Their Treatment; Including Physiological Old Age, Home and Institutional Care, and Medico-Legal Relations The generous reception of the first edition by the medical profession is an encouraging indication that the field of geriatrics is not a barren waste, that with more wide-spread interest and more extended research it may become one of the most fruitful fields in the realm of medicine. Containing features new in thought and elaboration and views differing radically from existing views, it was expected that the work would be severely criticised. Reviewers, generally, have been gracious in their criticisms, pointing out errors and questioning opinions. The basic proposition that "Senility is a physiological entity analogous to childhood and not a pathological state of maturity," has been questioned on the grounds that there is no senile norm and no point at which we can fix the beginning of the senile state. All anatomical and physiological standards are based upon averages, drawn in some cases from widely separated normal limits. In childhood we fix standards year by year. We could, in the same manner, fix yearly norms in old age although the limits would be more widely separated as some age sooner than others. And just as we fix the limit of childhood at puberty, although the period of development continues until the third decade, we can fix the beginning of old age at the menopause and male critical period, although some organs begin to degenerate before this time. The basic proposition is being accepted by those who have given thought to the subject and who realize that in dealing with senile diseases the object must be to restore the organism to the normal senile state and not to the normal state of maturity. It is hardly necessary to justify the new classification of diseases. The ordinary classifications, based partly upon etiology, partly upon functions and physiological relations, partly upon anatomical position, partly upon the supposed nature of pathological states, is unscientific and irrational. The classification in this work is based upon the relation of the diseases to the senile processes. This can be elaborated by making subdivisions of the five groups.
