Book excerpt: The progess in biological sciences and its applications in obstetrics in the last twenty years have formed the base for feto-placental semeiotics. In this way, it has been possible to diagnose placental insufficiency even in patients whose clinical conditions gave all the indications of a perfectly normal feto-placental condition. On the other hand, observations have shown that in certain conditions some deficiencies can im prove until normality is reached again. On the basis of the above one feels justified to talk about the therapy of feto-placental insufficiency. Until now this therapy has been aimed at the obstetrical pathology of the mother. Our aim is to illustrate the various steps which should be taken to execute an efficient therapy of the conditions relating to the pathology of the fetus, realizing full well that fetal pharmacology is only now being taken into consideration. The various steps to be taken, the length of therapy, and all the other correlated factors are still the object of individual ex periences, and we are fortunate to have amongst us researchers in this field who have accepted our invitation and can supply us with their personal experiences which can also be compared. The first international symposium on the therapy of feto-placental insufficiency has been organized in conjunction with the Samil-Pabyrn Foundation whom I would lik to thank, and especially so its president, Mrs. INGE SAXON-MILLS.

In particular accurate preoperative analysis of ovarian pathology can improve the selection of the correct therapeutical approach, to reduce the length of operations, maximize surgical technique, and can ultimately improve a range of operative outcomes. New imaging modalities have advanced to the point of high resolution, three-dimensional analysis of tissue anatomy, composition and perfusion. Ultrasonography (US), Computed tomographic (CT) and magnetic resonance (MR) imaging have recently emerged as outstanding non-invasive techniques for the detection and characterization of ovarian pathology. In particular, US has probably now imposed itself as the “state-of-the-art” technique to explore the ovarian neoplasm, thanks the technical advancement like texture analysis (MGV) and 3D potentialities, although MR with new sequences like the diffusion-weighted imaging and greater magnetic fields (3 Tesla or more) may become leading methods in the future. The purpose of this book is to cover all the imaging techniques, potential for applying such imaging clinically, and to offer present and future applications as applied to ovarian pathology with the most world renowned scientists in these fields. The book is designed according to the pathological classification of the benign and malignant ovarian neoplasm by presenting for each pathology the clinical setting followed by the imaging approach. At the end of each chapter a “focus concept” paragraph will be presented with take-home point for the readers.

It is only by comparing experimental results that it is possible to move forward, albeit slowly, towards a generally agreed consensus based upon objective scientific data. It is for this reason that we are particularly grateful to Ayerst Laboratories whose support and help have enabled us to turn our original proposal for an International Symposium into a reality. We are also grateful to the publishers for the efficiency with which they have organized the publication of the Proceedings. It is our hope and that of all the distinguished participants that all readers of this volume will be able to find something in it which will stimulate further thought and discussion - even though they may not necessarily agree with all the conclusions expressed - for the success of a Symposium turns not only on the subject under examination but also on the quality of debate and discussion it encourages.

Book excerpt: Because of increasing worldwide interest in the subject, a decision was made to coordinate the papers presented at the First European Symposium on Hysteroscopy, held in Antwerp, Belgium, in September 1982, into a text on hysteroscopy, including its indications, techniques, and complications. The Organising Committee called on Dr. Louis G. Keith of Northwestern University, Chicago, illinois USA, to edit all of the papers and to prepare them for publication as a multi-author textbook rather than as a haphazard collection of the proceedings of a meeting. Dr. Keith also supervised the many details of preparing the manuscript for publication, including the task of copy editing. The speakers at the Symposium were all eminent European hysteroscopists; the most up-to-date information was presented, assuring the current nature of the scientific data. By way of introduction (Part I), a historical survey of the development of hysteroscopy is presented. The material is then divided into several parts. Part II concerns the technical aspects of hysteroscopy. Part m includes the different clinical applications of hysteroscopy. Part IV concerns special applications of hysteroscopy, and the Conclusion (Part V) contains the remarks of two distinguished American guests. Dr. Alvin M. Siegler presented a special lecture on Jan Palfijn (Chapter 29), and Dr. Louis G. Keith was asked to discuss the relationship between art and medicine at a banquet at the Rubens Mansion in Antwerp (Chapter 30).
