Caring Democracy argues that we need to rethink American democracy, as well as our fundamental values and commitments, from a caring perspective. The idea that production and economic life are the most important political and human concerns ignores the reality that caring, for ourselves and others, should be the highest value that shapes how we view the economy, politics, and institutions such as schools and the family. Care is at the center of our human lives, but Tronto argues it is currently too far removed from the concerns of politics. Caring Democracy traces the reasons for this disconnection and argues for the need to make care, not economics, the central concern of democratic political life. Joan C. Tronto is a Professor in the Political Science Department at the University of Minnesota. She is the author of Moral Boundaries: A Political Argument for an Ethic of Care (Routledge).

Book excerpt: The Republic of Therapy tells the story of the global response to the HIV epidemic from the perspective of community organizers, activists, and people living with HIV in West Africa. Drawing on his experiences as a physician and anthropologist in Burkina Faso and Côte d’Ivoire, Vinh-Kim Nguyen focuses on the period between 1994, when effective antiretroviral treatments for HIV were discovered, and 2000, when the global health community acknowledged a right to treatment, making the drugs more available. During the intervening years, when antiretrovirals were scarce in Africa, triage decisions were made determining who would receive lifesaving treatment. Nguyen explains how those decisions altered social relations in West Africa. In 1994, anxious to “break the silence” and “put a face to the epidemic,” international agencies unwittingly created a market in which stories about being HIV positive could be bartered for access to limited medical resources. Being able to talk about oneself became a matter of life or death. Tracing the cultural and political logic of triage back to colonial classification systems, Nguyen shows how it persists in contemporary attempts to design, fund, and implement mass treatment programs in the developing world. He argues that as an enactment of decisions about who may live, triage constitutes a partial, mobile form of sovereignty: what might be called therapeutic sovereignty.

Analysts all seemed to agree that public welfare programs were a resounding failure. What should better public care look like? Democracy, Justice, and the Welfare State sets up a dialogue between work on the ethic of care and studies of public care in practice. White argues that care as it is currently institutionalized often both assumes and perpetuates dependency and so paternalistic relationships of authority. Better public care requires that such paternalistic practices be challenged. Care appropriate to a democratic context must itself be a democratic practice.

Feminist scholarship on the gendered construction of welfare provisioning and welfare regimes has produced a conceptually strong and empirically grounded analysis of care, reinforcing the necessity of rethinking the distinctions between "the public" and "the private" as well as the links between them. Yet this analysis, premised on post-industrial contexts, does not travel easily to other parts of the world. Many of its core assumptions – about family structures, labor markets, state capacities, and public social provisioning – do not hold for a wider range of countries. Drawing on original research on the care economy in three developing regions (Africa, Asia, Latin America), this volume addresses a major empirical lacuna while facilitating a conversation across the North-South divide.

Book excerpt: Franco Basaglia (1924-1980) was an Italian psychiatrist and activist who proposed the dismantling of psychiatric hospitals and pioneered new ideas about mental health and its treatment. Basaglia was also one of the principal proponents of Italy's Law 180, which effectively closed down large mental hospitals in Italy. His ideas and his disciples have had a decisive influence in the move away from institutional care in many parts of the world, particularly in continental Europe and South America. However, Basaglia is strikingly absent from the literature in Germanic and Anglophone psychiatry. Most of the literature about Basaglia in the last 40 years has been published by his followers and supporters and has often been largely positive, with little exploration of differing responses or possible limitations of his model. Basaglia's International Legacy: From Asylum to Community provides an overview of current thinking and the international influence of Franco Basaglia. This resource draws on the combined knowledge of clinicians, policy makers, historians, and social scientists, including a handful of Basaglia's collaborators. The analysis reveals that multiple forms of global inequalities are now playing out in the most intimate of spaces.
