La réduction du volume alvéolaire conduit à des problèmes thérapeutiques et esthétiques. La conservation du volume osseux après extraction constitue donc un objectif prioritaire à atteindre, et l’utilisation de matériaux de substitution osseuse dans ce cadre paraît a priori tout à fait justifiée. Afin de palier cette résorption osseuse, de nombreux matériaux de reconstruction osseuse ont vu le jour. Il s'agit d'allogreffe (greffe des tissus d’un individu à un autre d’une même espèce), de xénogreffe (greffe des tissus d’un individu à un autre d’une autre espèce) ou de matériau synthétique. La multiplicité de ces matériaux témoigne de l'énorme demande des praticiens mais prouve également que le matériau de substitution osseuse idéal n’existe pas. Cette ouvrage a pour but d'évaluer l'efficacité des matériaux de reconstruction osseuse dans les alvéoles d'extraction. De plus, nous mettrons en évidence, s'il en existe, les différences significatives, histologiques et cliniques, entre les différents matériaux.

La cicatrisation alvéolaire qui en découle conduit inémédiablement à une diminution du volume de la crête osseuse. Des désordres fonctionnels et esthétiques peuvent apparaître suite aux extractions dentaires; et le succès de la pose d’un implant s’avère parfois compromis. Les aspects histologiques de la cicatrisation osseuse après une extraction dentaire, montrent un tissu de granulation majoritairement présent dans les trois premières semaines de cicatrisation. Ce tissu est remplacé par un tissu conjonctif fibreux vers la quatrième semaine. Des flots dos néoformé entouré d’un tissu ostéoïde apparaissent aussi à la quatrième semaine. Leur surface augmentent et évoluent en un os minéralisé et dense qu’à partir de la douzième semaine de cicatrisation. Cliniquement, une résorption osseuse apparaît aussi bien dans le sens vertical que dans le sens horizontal. D’une manière générale, le niveau de l’os reformé n’atteint jamais le niveau des crêtes mésiales et distales des dents adjacentes, on observe une incurvation dans le sens mésio-distal d’une hauteur d’environ 1,2 mm. De nombreux auteurs ont cherché à mettre en place des techniques chirurgicales visant à réduire ce phénomène de résorption alvéolaire. Tous ces auteurs s’accordent sur le fhit qu’il faille favoriser la colonisation des cellules osseuses sur le site extractionel. Le comblement et/ou le recouvrement des alvéoles déshabités permettent de maintenir le volume osseux de sorte que le site autorise une restauration prothétique répondant aux critères esthétiques et fonctionnels optimaux. Mais aucune ne semble donner des résultats prévisibles.
