Book excerpt: Au cours de ces dernières années, l'endodontie est l'une des disciplines qui a connu le plus d'avancées technologiques aussi bien en termes d'instrumentation que de biomatériaux. Cependant, la complexité de l'infection endodontique modulée par les différentes réponses de l'hôte fait que des traitements endodontiques de qualité peuvent être associés à une absence ou un retard de régression des lésions péri-apicales sous-jacentes. L'objectif de cette thèse est d'interroger la littérature sur les paramètres qui ont été montrés comme cruciaux pour moduler la cicatrisation des lésions péri-apicales d'origine endodontique et qui sont donc à cibler pour les thérapeutiques innovantes futures. Dans un premier temps, ce travail mettra en valeur les caractéristiques de ces lésions péri-apicales en termes d'histologie, de mécanismes physiopathologiques, de manifestations cliniques et de diagnostic. Ce travail proposera ensuite une revue de la littérature pour établir les paramètres pertinents pour moduler la cicatrisation des lésions endodontiques, ainsi que les différentes stratégies innovantes qui pourraient permettre de normaliser les sites inflammatoires et donc optimiser le cicatrisation péri-apicale.

L'objectif de cette revue est de comparer le processus de cicatrisation après apexification et traitement endodontique régénératif sur dents permanentes immatures nécrosées, et de déterminer laquelle de ces deux thérapeutiques garantit au mieux la pérennité des dents sur l'arcade dentaire. Pour cela, nous avons identifié et inclus des articles traitant d'apexification, et de traitement endodontique régénératif, sur dents permanentes immatures. Les résultats n'ont pas permis de mettre en évidence de différences significatives entre les deux traitements, notamment en raison de la faible proportion d'études comparatives dans la littérature existante. Tous deux assurent une résolution des signes et des symptômes, ainsi qu'une cicatrisation de la lésion péri-apicale. Le traitement endodontique régénératif a toutefois permis d'observer un épaississement ainsi qu'une croissance radiculairea.
