Demographically the most important continent on the world, Asia's population, currently estimated to be 4.2 billion, is expected to increase to about 5.9 billion by 2050. Rapid declines in fertility, together with rising life expectancy, are altering the age structure of the population so that in 2050, for the first time in history, there will be roughly as many people in Asia over the age of 65 as under the age of 15. It is against this backdrop that the Division of Behavioral and Social Research at the U.S. National Institute on Aging (NIA) asked the National Research Council (NRC), through the Committee on Population, to undertake a project on advancing behavioral and social research on aging in Asia. Aging in Asia: Findings from New and Emerging Data Initiatives is a peer-reviewed collection of papers from China, India, Indonesia, Japan, and Thailand that were presented at two conferences organized in conjunction with the Chinese Academy of Sciences, Indian National Science Academy, Indonesian Academy of Sciences, and Science Council of Japan; the first conference was hosted by the Chinese Academy of Social Sciences in Beijing, and the second conference was hosted by the Indian National Science Academy in New Delhi. The papers in the volume highlight the contributions from new and emerging data initiatives in the region and cover subject areas such as economic growth, labor markets, and consumption; family roles and responsibilities; and labor markets and consumption.

It consists of four parts. The first part reports the 6th, 7th, and 8th surveys of “Chinese Longitudinal Healthy Longevity Survey” (CLHLS), with the largest sample of oldest-old aged 80+ in the world and comparable sample of young-old aged 65–79, trends and characteristics of physical health and mental health of older adults in China based on analyses of the CLHLS datasets. The second part focuses on analyses and discussions of the influencing factors of healthy aging from perspectives of families, socioeconomics and community environments. The third part reports studies on the gender and age differences in main chronic diseases’ prevalence rates, inflammation, anti-oxidation, trace elements, effects of the nutritional dietary structure and the environmental selenium on health among the older adults. The fourth part focuses on the analyses and discussions of policy recommendations to face the serious challenges of the population aging in China and the database of the eight waves of CLHLS from 1998 to 2018, which are available free of charge to scholars, practitioners and general public for scientific research and socioeconomic planning. Book excerpt: China, which is fast on its way to becoming the most powerful economic force in the world, has four unique characteristics that distinguish it from other countries in Asia: (1) The proportion of aging population is growing faster than that of Japan (the country previously recognized as having the fastest rate) and much faster than nations in western Europe. (2) An early arrival of an aging population before modernization has fully taken place, with social policy implications. It is certain that China will face a severely aged population before it has sufficient time and resources to establish an adequate social security and service system for older people. (3) There will be fluctuations in the total dependency ratio. The Chinese government estimates are that the country will reach a higher dependent burden earlier in the twenty-first century than was previously forecast. (4) The government’s fertility policy (single child per family) and its implementation has a strong influence on the aging process. Fewer children are being born, but with more elderly people a conflict arises between the objectives to limit population increase and yet maintain a balanced age structure (Peng and Guo 2001). The intersection of these fourfold factors means that the increased aging population is giving rise to serious concerns among Chinese social policy makers. There is a chronic lack of good resource materials that attempt to make sense of social policy in its relationship to examining the problems and possibilities of human aging grounded in an analysis of economic of social policy in China and impact on rural and urban spaces. Such analysis of China will be covered by conceptual, theoretical, and empirical approaches. The book will also discuss substantive topics of housing, community care, family care, pensions, and mental health. The book brings together a truly world class array of researchers to provide discussions of critical implications of aging social policy and the economic impact in China.

The increase in the elderly population, along with the rising prevalence of major chronic diseases and greater longevity among those afflicted with many of these diseases, will lead to a growing number of functionally disabled elders who may need longterm care (LTC). Elders in China have traditionally relied on their families as the main source of LTC. This situation is changing as fewer family members are available to provide care, due to shrinking family size caused by continuously low fertility, combined with increased female workforce participation. Without a well designed LTC system to address the growing care need, it is increasingly obvious that the current LTC situation in China will not be sustainable in the coming decades. Old-age disability and LTC have not yet been well studied in China. Using logistic regressions and a prevalence ratio projection model, and considering international practices, this dissertation addresses three research questions: 1) What are the key risk factors for old-age disability in China? 2) What are the projected numbers of older adults with disabilities in China in future decades through 2050? 3) How can China develop a feasible and sustainable LTC delivery and financing system to address projected growth in LTC needs of this population over the next four decades?

Book excerpt: This study examines the health services utilization of older adults in China with depressive symptoms and chronic health conditions. In older adults with chronic conditions, depression has shown to be associated with greater severity of medical illness, poorer health status and quality of life, and increased mortality. Studies in western industrialized societies have shown that individuals with depression use a higher number of physical health services. However, there have been no studies identified to date that have looked at this question for China, including for Chinese older adults in particular. The healthcare utilization patterns of Chinese residents with depression may show differential patterns from those of individuals living in western industrialized countries due to differences in culture, available forms of help, existing service systems, and policy contexts, including the rapid expansion of healthcare coverage in China in recent years due to healthcare reform. This study employs logistic regression to analyze the relationships between depression, chronic health conditions, and health services utilization among Chinese older adults. It uses nationally representative survey data from the 2011-2012 China Health and Retirement and Longitudinal Study (CHARLS) (n=17,708). The study also presents a profile of service users based on a cluster analysis of health conditions and health services used. After controlling for sociodemographic and health characteristics, depression was found to be positively associated with outpatient service utilization but not significantly associated with inpatient service utilization for the general sample of older adults in China. Among the sample of older adults with chronic conditions, also having depression was significantly associated with greater outpatient service use. Those with both chronic conditions and depression were 1.53 times more likely to use outpatient services than those who only had chronic conditions with no depression. There was no significant relationship between chronic conditions and depression with inpatient service use. Results of a cluster analysis indicate that depression levels and levels of ADLs and IADLs appear to be positively associated with both outpatient and inpatient service utilization. High utilizers of outpatient services tend to be less uninsured and to have the most Urban Employee and New Cooperative insurance, and the least Urban Resident and Government insurance relative to moderate and low users of outpatient services. High users of outpatient services are more likely to be female and rural residents. High users of inpatient services tend to be more rural and to have lower levels of social support than both moderate and low users. Findings on the healthcare usage patterns of this population have implications for improving health policy and interventions related to the treatment and prevention of depression among this population of vulnerable older adults.

Book excerpt: Social isolation and loneliness are serious yet underappreciated public health risks that affect a significant portion of the older adult population. Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. Social Isolation and Loneliness in Older Adults considers clinical tools and methodologies, better education and training for the health care workforce, and dissemination and implementation that will be important for translating research into practice, especially as the evidence base for effective interventions continues to flourish.

Book excerpt: This open access book offers essential information on values-based practice (VBP): the clinical skills involved, teamwork and person-centered care, links between values and evidence, and the importance of partnerships in shared decision-making. Different cultures have different values; for example, partnership in decision-making looks very different, from the highly individualized perspective of European and North American cultures to the collective and family-oriented perspectives common in South East Asia. In turn, African cultures offer yet another perspective, one that falls between these two extremes (called batho pele). The book will benefit everyone concerned with the practical challenges of delivering mental health services. Accordingly, all contributions are developed on the basis of case vignettes, and cover a range of situations in which values underlie tensions or uncertainties regarding how to proceed in clinical practice. Examples include the patient’s autonomy and best interest, the physician’s commitment to establishing high standards of clinical governance, clinical versus community best interest, institutional versus clinical interests, patients insisting on medically unsound but legal treatments etc. Thus far, VBP publications have mainly dealt with clinical scenarios involving individual values (of clinicians and patients). Our objective with this book is to develop a model of VBP that is culturally much broader in scope. As such, it offers a vital resource for mental health stakeholders in an increasingly inter-connected world. It also offers opportunities for cross-learning in values-based practice between cultures with very different clinical care traditions.
