It would be ideal to incorporate an effective anti-microbial agent such as chlorhexidine in a dentifrice formulation. A well-known side effect of chlorhexidine is tooth discoloration. Reduced concentrations have been suggested to decrease side effects. The low dosage 0.12% chlorhexidine dentifrice gel, which is available in the Netherlands, was the subject of two research projects as presented in this thesis. It appeared to be as effective as a regular dentifrice in the prevention of plaque accumulation and is therefore a poor alternative because it is does not contain fluoride. This thesis also shows that a 0.2% CHX mouthwash does provide a benefit which can be used in conjunction with a regular fluoride toothpaste. Lasers are used throughout the medical field for different indication and have been suggested as an adjunct to scaling and root planing in patients with periodontitis. Two research projects as presented in this thesis showed that there is no evidence to support the adjunctive use of the water cooled Nd:YAG laser. The effect of sub-gingival debridement as performed with manual and/or power-driven instruments in periodontitis patients as well as during periodontal maintenance care was not improved when followed by the application of this laser. An extensive literature review supported these findings. These results therefore support a firm statement to refute the use of laser technologies in non-surgical periodontal treatment based on the lack of clinical efficacy and the potential extra costs."--Samenvatting auteur.

Abstract: Background: Chlorhexidine (CHX) is the most effective antiseptic mouthwash known and yet, side effects such as tooth discolorations and bitter taste exist. A new product of CHX with an anti-discoloration system (ADS) was supposed to minimize these side effects while maintaining the antiseptic effects. The aim of this clinical trial was to evaluate the effectiveness of the CHX with and without an ADS on stains, plaque accumulation and gingivitis development. Material and Methods This double blinded RCCT included 26 healthy dental students. After a preparatory period (3 weeks) of professional prophylaxis and optimal tooth brushing, the participants were randomly assigned to one of three groups: Group P (Placebo), Group T1 (0.12% CHX without ADS) and Group T2 (0.12% CHX with ADS). During 3 weeks of abolished oral hygiene, the participants were asked to rinse with a mouthwash twice daily. Allocation was concealed until after the baseline examination. The clinical parameters were assessed on Days 0, 7, 14 and 21 and included the Discoloration Index (DI), the Plaque Index (PII) and Gingival Index (GI). Results All 26 participants completed the study with good compliance. At baseline, there were no significant differences for any parameters between any of the groups. Throughout the study, there were no significant differences in mean DI, PII or GI between group T2 and group P. However, significant differences in mean DI between group T2 and group T1 on Day 14 (0.190.15 vs. 0.870.56, p=0.007) and on Day 21 (0.210.17 vs. 1.130.59, p=0.001) were noted. Also, the mean PII between group T2 and group T1 on Day 7 (0.890.40 vs. 0.130.09, pConclusions CHX with ADS appeared to be effective in preventing stain on teeth. However, the efficacy of preventing plaque accumulation and gingivitis development was greatly hampered by the addition of ADS. Use of the chlorhexidine chip (CHX) in conjunction with phase I periodontal therapy has shown some effectiveness in reducing periodontal probing depth, clinical attachment loss and bleeding on probing. Patients with chronic periodontitis having periodontal pocket depth 5 mm with age group 25-60 belonging to both sexes were included in the present study. The test site and the control site were randomly assigned. Along with phase I periodontal therapy, test sites received CHX chip while control site received placebo chip. Plaque index (PI), gingival index (GI), probing pocket depth (PPD), clinical attachment level (CAL) & gingival margin position (GP) were assessed at all selected sites at baseline and subsequently at 1,3, 6 and 9 months following the day of CHX chip & placebo chip placement. Radiographs were taken at baseline, 6 and 9 months. The results of the present study favours the use of chlorhexidine gluconate chip as an adjunct to phase I periodontal therapy in treatment of chronic periodontitis."
