Book excerpt: The slow response of maternal and child health care problems to control strategies in Nigeria suggests that the health sector requires more holistic study. This study focuses on case-specifics of the affected communities and examines the incidence of maternal and child health care from entire cultural domain of the focused communities. The rural communities of Osun state in Nigeria are the object of the study, where the local conception of maternal and child health was examined. Similarly, the local people's perception of maternal and child health was identified and various attitudes and practices affecting health care provisioning in the rural communities of Osun state were similarly analysed. Relying on ethnographic methods of data collection, the text establishes that certain local attitudes are constraints against equitable access to maternal and child health care facilities. It also establishes that there are some other local practices which promote good health of the rural mothers and their children. Specific findings show that illiteracy and low level of economic activities suffered by mothers were the major causes of maternal and child health care problems in rural communities.

Book excerpt: The evaluation of reproductive, maternal, newborn, and child health (RMNCH) by the Disease Control Priorities, Third Edition (DCP3) focuses on maternal conditions, childhood illness, and malnutrition. Specifically, the chapters address acute illness and undernutrition in children, principally under age 5. It also covers maternal mortality, morbidity, stillbirth, and influences to pregnancy and pre-pregnancy. Volume 3 focuses on developments since the publication of DCP2 and will also include the transition to older childhood, in particular, the overlap and commonality with the child development volume. The DCP3 evaluation of these conditions produced three key findings: 1. There is significant difficulty in measuring the burden of key conditions such as unintended pregnancy, unsafe abortion, nonsexually transmitted infections, infertility, and violence against women. 2. Investments in the continuum of care can have significant returns for improved and equitable access, health, poverty, and health systems. 3. There is a large difference in how RMNCH conditions affect different income groups; investments in RMNCH can lessen the disparity in terms of both health and financial risk.

Book excerpt: This thesis employs advanced econometric methods to understand the determinants of child health outcomes and the inequalities in child health that exist in Nigeria. It then investigates whether the effects on deprivation and poverty can be used as a policy guide to reduce existing health inequalities in child health. It makes use of data from the Nigerian Demographic and Health Survey (NDHS) of 2008. The main methodological contributions of this thesis include: 1) the adjustment of birthweight models for sample selection bias using two robust instruments which is a problem ignored in most studies carried out in developing countries; 2) the computation of a proxy for mother's health, vaccination schedule and multiple deprivation in the African context; 3) the decomposition of segregational differences in birthweight and infant mortality (this is surprisingly highly understudied in Africa); 4) the application of a complex integrated framework in the study of child malnutrition, an approach that has not been carried out in a developing countries context; 5) the analysis of a heterogeneity-adjusted approach to multidimensional child deprivation in Nigeria. This is a novel approach to the study of child poverty. Results show that there exists a significant urban bias in infant mortality mainly due to differences in endowments. The weight of the child at birth impacts not only the current health of the child but has future implications on the child's health. Selection bias appears to overestimate birthweight and the endowment effect. A complex methodological approach uncovers various effects of malnutrition determinants and provides greater policy relevance. The findings support policies to enhance both maternal and child health in order to reduce existing segregational inequalities and dimensions of deprivation.

Book excerpt: There is a growing interest in the research literature in exploring how child nutrition is affected by sociocultural practices, such as polygyny. However, evaluation of the effect of polygyny on child nutrition has been hindered by the complexity of the relationship. This paper investigates the effect of polygyny on anthropometric outcomes while recognizing that unobservable household characteristics may simultaneously influence both the decision to form a polygynous union and the ability of the household to adequately nourish children. Polygyny can affect children’s nutrition through increased family size, early marriage, and the level of household investment in child health. In this paper, we apply an instrumental variable approach based on the occurrence of same sex siblings in a woman’s first two births to generate exogenous variation in polygyny. Using data from the 2008 and 2013 Nigeria Demographic and Health Surveys, we find a detrimental effect of polygyny on child undernutrition, with a greater effect in poorer households and those resident in more urban locations.

Today, however, these drugs no longer work against the deadliest form of malaria that exists throughout the world. Malaria deaths in sub-Saharan Africaâ€"currently just over one million per yearâ€"are rising because of increased resistance to the old, inexpensive drugs. Although effective new drugs called "artemisinins" are available, they are unaffordable for the majority of the affected population, even at a cost of one dollar per course. Saving Lives, Buying Time: Economics of Malaria Drugs in an Age of Resistance examines the history of malaria treatments, provides an overview of the current drug crisis, and offers recommendations on maximizing access to and effectiveness of antimalarial drugs. The book finds that most people in endemic countries will not have access to currently effective combination treatments, which should include an artemisinin, without financing from the global community. Without funding for effective treatment, malaria mortality could double over the next 10 to 20 years and transmission will intensify.
