Book excerpt: Globally, the Vibrio parahaemolyticus and Vibrio vulnificus represent important human pathogens associated with the consumption of seafood. In response to the requests for scientific advice from Codex Committee on Food Hygiene (CCFH), risk assessments for the pathogens V. vulnificus, V. cholerae, V. parahaemolyticus and guidance on methods for the detection of Vibrio spp. with seafood have been conducted and published previously by JEMRA. In order to provide an update on the state-of-the-art advice regarding risk assessment for V. parahaemolyticus and V. vulnificus in seafood, an expert meeting was convened. Several critical developments in the last decade were subsequently noted by the expert working group: 1) The emergence of highly pathogenic strains; 2) In response to climate change, there has been a significant geographical spread regarding when and where these seafood-associated Vibrio infections; 3) Demographic considerations are very important; 4) A range of new approaches for best practice; and 5) A range of new methods, such as those utilising genomics and satellite imagery. This report describes the output of that expert meeting.

Book excerpt: There has been an increase in reported outbreaks and cases of foodborne disease attributed to pathogenic Vibrio species. As a result, there have been several instances where the presence of pathogenic Vibrio spp. in seafood has led to a disruption in international trade. A number of Vibrio spp. are increasingly being recognized as potential human pathogens. The food safety concerns associated with these microorganisms have led to the need for microbiological risk assessment for their control. This report provides the review of risk assessment of existing tools for V. parahaemolyticus and V. vulnificus in oysters and different bivalve molluscan species, the available information on testing methodology and recommend microbiological methods to monitor the levels of pathogenic Vibrio spp. in seafood and/or water. Such tools are envisioned to support countries in their efforts to use risk-based approaches in the selection of control measures appropriate for their seafood species, primary production and post-harvest practices. This volume and others in this Microbiological Risk Assessment Series contain information that is useful to both risk assessors and risk managers, the Codex Alimentarius Commission, governments and regulatory authorities, food producers and processers and other institutions and individuals with an interest in Vibrio spp. and its control.

Book excerpt: The FAO/WHO expert meeting held in December 2014 considered microbial contamination of lipid-based ready-to-use foods (RUFs) and the risk of foodborne infections in the malnourished population of children that consume RUFs. The goals of the expert meeting were to: review the status of the microbiological safety of lipid-based RUFs used to manage moderate acute malnutrition and severe acute malnutrition; conduct a comprehensive risk assessment; provide guidance to producers on the general approach and requirements for manufacturing RUFs that are safe for their intended use; and provide guidance to the agencies that purchase RUFs on how best to judge their microbiological safety. The expert committee described three approaches that purchasers of RUFs might use to establish microbiological criteria to assure the safety of RUFs and to communicate to manufacturers their safety expectations. These approaches are: (i) reference to existing standards established for similar low-moisture foods; (ii) determining an acceptable increase in risk over the pre-existing baseline of illness from other sources of exposure; and (iii) process verification sampling using the moving window technique. The microbiological criteria derived by each of these approaches accomplish different purposes, and which is most appropriate is determined by the conditions of manufacture and use.

Book excerpt: Strains of pathogenic Escherichia coli that are characterized by their ability to produce Shiga toxins are referred to as Shiga toxin-producing E. coli (STEC). STEC are an important cause of foodborne disease and infections have been associated with a wide range of human clinical illnesses ranging from mild non-bloody diarrhoea to bloody diarrhoea (BD) and haemolytic uraemic syndrome (HUS) which often includes kidney failure. A high proportion of patients are hospitalized, some develop end-stage renal disease (ESRD) and some die. The information requested by CCFH is divided into three main areas: the global burden of disease and source attribution; hazard identification and characterization; and monitoring, including the status of the currently available analytical methods. This report provides an overview of the work undertaken in response to the request from the CCFH and provides the conclusions and advice of the Expert Group based on the currently available information.

Book excerpt: Shiga toxin-producing Escherichia coli (STEC) infections are a substantial health issue worldwide. Circa 2010, foodborne STEC caused > 1 million human illnesses, 128 deaths, and ~ 13,000 Disability Adjusted Life Years (DALYs). Targeting interventions appropriately relies on identifying those strains of greatest risk to human health and determining the types of foods that cause STEC infections. There are hundreds of STEC serotypes; however, based on the evidence gathered during the review, the Expert Group concluded that the serotype of the STEC strain should not be considered a virulence criterion. All STEC strains with the same serotype should not be assumed to carry the same virulence genes and to pose the same risk, as many STEC virulence genes are mobile and can be lost or transferred to other bacteria. this report proposes a set of criteria for categorizing the potential risk of severity of illness associated with a STEC in food is recommended based on evidence of virulence gene profiles and associations with clinical severity. The criteria could be applied by risk managers in a risk-based management approach to control STEC in food. While ruminants and, other land animals are considered the main reservoirs for STEC, various largescale outbreaks have been linked to other foods. Thus, the report also addresses source attribution of foodborne STEC infections globally in order to inform the development of international standards by the Codex Alimentarius on the control of STEC, and in particular identify the foods which should be the focus of those standards. Finally it provides a review of monitoring programmes and methodology for STEC which can serve as a reference for countries planning to develop such programmes.
