But this fact that the minimum of mortality by phthisis is in the winter does not at all prove that coldness is the most favorable condition for the disease, nor that the results produced by comparing winter with summer will be the same when cold regions are compared with warm regions. In the first place the cold season is, in warm climates, the best part of the year. The genial months for softness of climate are from September to January, while the sum mer, both by the intensity and duration of the heat, debilitates the energies and thus accelerates the evolution of phthisis. Book excerpt: Excerpt from Chicago Medical Review, Vol. 5: May 15, 1882 The reign OF Fannie - The Review sincerely regrets to find one of its cher ished illusions dissipated. It had long eu tertained the opinion that the New York physicians were endowed with considerable intellectual power. This opinion has been proven baseless by the Medical News. That Journal after arguing with the New York physicians, expostulating with them, and it might be said vituperating them were it not that this is too strong a designa tion for anything emanating from the city of brotherly love, suddenly dons the garb of jesop and with an enterprise at which the Review as a brother member of the medical press feels rejoiced, launches a fable at them. This last destroyed all the Review's belief in the New York medical profession. Fables are usually intended to teach children, or races, in an infantile stage of development, wisdom. \vhile the Medical News argued or expostulated with the New York profession, there was still some evidence to make the Review cherish its illusions, but when the Medical News abandoned argument and expostulation and in despair resorted to fables but one con elusion could be drawn; the News regarded the New York physicians as having only the mental condition of children. The Review bows with humility to the psycho logical knowledge of the News. Tobe sure this has been ridiculed by the best. This substance has lately been formed by synthesis out of nitro-ben zol, anilin, glycerin and sulphuric acid. This synthesis is a long step toward making the cinchona alkaloids themselves, and we may safely predict that within a few years these bases may be prepared in our labora tories in great abundance. Perhaps they can not be made in so great abundance nor so cheap as salicylic acid, yet the price can be made far less than at present. Worthington has reported in the British Medical Journal a case that may be of use to us some day when we are in a perplex ity. The case was this: The patient gave symptoms of strangulated hernia, stercor aceous vomiting, intense dragging pain, referred to the umbilicus, etc. There was great abdominal tenderness and a constant troublesome cough. After the most mi nute examination of the inguinal canal, nothing could be found. Several days passed by and the distress became extreme. Injections of warm water were given and large doses of opium. Abdominal section was decided upon, but first it was thought best to try puncture. An aspirating needle was introduced, and a large quantity of atus drawn off with some uid. Relief was immediate, and recovery progressed from that time on. Some days later a small indirect inguinal hernia showed itself on the right side for the first time. Some months after, a long spindle-shaped mass of faces was passed, which, on examination, was found to con tain three teaspoons. Similar cases are re ported in Gaillard's Medical Journal, vol ume 1, page 577, and in Poulet's Foreign Bodies in Surgery, volume I, page 137. Dr. Kohn's case was not attended by haemate mesis as were the other cases cited, and the patient fully recovered. A milk diet seems to agree best With some patients; others do as well upon beef ex tracts. Many severe cases of sunstroke have been brought: in during the past few days. They are mostly of the cardiac variety (of Reynolds); and the chief indica tion, in addition to the reduction of temperature by cold douches and ice to the head, is to support the heart's action. This is done by free hypodermics of whiskey and ammonia and in the worst cases by the application of the electric current to the pneumo-z gastric. This treatment is usually successful. The following case has excited some interest among the staff of the hospital, illustrating as it does the great efficacy of transfusion of blood in extreme cases.
