In Cambodia, diarrheal disease is a prominent cause of childhood mortality. Recent findings suggest that greater than 40% of diarrheal diseases in developing nations are attributed to contaminated food. The majority of consumers in Cambodia purchase food through informal markets that escape food safety standards and controls. This is primarily due to the fact that food safety regulations are often poorly enforced, and infrastructural and educational capacity for food safety is limited, particularly in informal market settings. This is critical, since a majority of consumers in LMIC purchase vegetables that are often consumed raw through informal markets. The consumption of contaminated raw vegetables is heavily linked to foodborne illness, and likely contributes to morbidity and mortality in the country. Currently, little data exists on the biological contamination (i.e. bacteria, viruses, and parasites) of vegetables being sold in informal markets in Cambodia. This research seeks to investigate "What is the prevalence and concentration of bacterial hazards on vegetables sold in Cambodian informal markets, and how do handling practices, from farm- to-market, contribute to food safety outcomes in the vegetable value-chain of Cambodia?". Based upon this, the following objectives were chosen: (1) to investigate the prevalence and concentration of Salmonella enterica, as well as the concentration of generic Escherichia coli (E. coli) and coliforms, on different types of fresh vegetables sold in Cambodian informal markets in different seasons; (2) to define the flow and the behaviors of stakeholders within the Cambodian vegetable value-chain through personal interviews; and (3) to identify practices that can potentially contribute to the cross-contamination of vegetables moving through the value-chain. It was hypothesized that the prevalence and concentration of bacterial hazards would vary by vegetable type and seasons, due to differences in food matrix and growth conditions, as supported by scientific literature. Furthermore, it was hypothesized that due to limited educational, physical and regulatory capacity, the likelihood of poor handling practices by vegetable value-chain actors would be high, and contribute to contamination of vegetables along the value chain. Three types of vegetables (lettuce, tomatoes, cucumbers) were collected from informal markets located in two different provinces (Battambang and Siem Reap) in Cambodia in two different seasons (rainy and dry). Samples were subjected to validated methods for the detection of Salmonella enterica, and the enumeration of Salmonella enterica, generic E. coli, and coliforms. Presumptive positive isolates of Salmonella enterica were confirmed using Real-Time PCR. Further, a survey tool was used to investigate the flow of the Cambodian vegetable value-chain, and personal interviews with value-chain actors was used to characterize food safety practices within this value-chain (i.e. farmer, collector, distributor and vendor). The highest prevalence of Salmonella enterica was found on lettuce collected in the dry season (55.8%), whereas the lowest prevalence was found on lettuce collected in the rainy season (15.4%). In terms of concentration of Salmonella enterica, lettuce had a significantly higher concentration (5.66 log10 CFU/g), as compared to cucumbers (4.20 log10 CFU/g) and tomatoes (3.99 log CFU/g). Further, vegetables in the rainy season (5.27 log10 CFU/g) had significantly higher counts of Salmonella enterica as compared to vegetables in the dry season (3.96 log CFU/g). Moreover, the highest concentration of E. coli and coliforms were observed on lettuce during the rainy season (2.75 and 6.31 log10 CFU/g respectively). Conversely, the lowest concentration of E. coli and coliforms were observed on cucumbers (0.81 log10 CFU/g) and tomatoes (3.89 log10 CFU/g) collected in the dry season. Survey results support that a high percentage of the value-chain actors do not practice cool storage, which increases the likelihood of microbial proliferation as well as decrease the quality of vegetables. Additionally, the wide use of inadequately composted animal-source waste, contaminated irrigation water, and the lack of basic food hygiene practices (e.g. using fabric gloves, selling raw meat in the same area as fresh vegetables, lack of handwashing practices etc.) were factors that may be contributing to the introduction of pathogens through cross-contamination. Findings from this study highlighted that vegetables sold in informal markets in Cambodia are contaminated with biological hazards, with high concentrations being observed across all vegetable types and seasons. Contaminated vegetables can introduce bacterial pathogens into informal markets. Furthermore, contamination may be increased through the lack of basic food hygiene practices of value-chain actors, particularly informal market vendors. Many of these practices promote cross-contamination of bacteria from the environment to the vegetables. For these reasons, interventions such as food safety recommendations in the form of training and education programs for value-chain actors, regulatory coordination, consumer communication programs and infrastructure development are necessary to reduce the likelihood of contamination and negative public health outcomes in the country.

In countries with more developed food safety infrastructures, food safety progress is often focused on emerging concerns. For example, it was long thought that finished rendered products were free of biological contamination. However, recent foodborne disease outbreaks associated with rendered products have highlighted the need to better understand pathogen transmission and survival in rendering environments to better eliminate pathogens in such products. The purpose of the first study was to evaluate the effects of moisture levels (0.0%, 0.5%, 1.0%, and 3.0%), storage temperatures (48°C and 76°C), and fat characteristics on the growth and survival of Salmonella enterica in choice white grease, beef tallow, and chicken fat samples. Samples were inoculated with a high concentration (~108 CFU/mL) or low concentration (~105 CFU/mL) Salmonella cocktail (S. Sentfenberg, S. Newport, S. Thompson and S. Infantis) and stored for up to 5 days at 48°C or 76°C. Remaining populations of Salmonella were evaluated daily with and without enrichment step. Bacterial death rates were calculated using the Weibull model for each temperature and moisture level. No significant effect between moisture and/or inoculum level were observed. Book excerpt: The global coronavirus pandemic is revealing major weaknesses, inequities and system-wide risks in global food systems, giving renewed urgency to foster pathways to greater food system sustainability and resilience. Due to rising unemployment, supply chain disruptions and other responses to the pandemic, such as disruptions to social assistance programs in some countries, predictions suggest a near doubling of food insecurity globally. Nutritional changes are also occurring, as food availability and access changes, leading to substitution of dry, canned or processed foods for healthier, fresh ingredients, for some communities, and the reverse for others. These food security and nutritional changes are likely to be as impactful on human health as the virus itself. As a system-wide shock, the pandemic reveals weaknesses of global supply chains. The media highlighted empty supermarket shelves alongside food dumping in situations where producers locked into disappearing food service outlets were unable to access new markets. Farmers with long-standing reliance on migrant agricultural labor that can no longer travel across international borders under lockdown struggle to access support for the upcoming harvest season. The pandemic highlights well-known inequities for marginalized food systems employees; as essential workers are exposed to greater risks of contracting the virus in food-processing, agricultural and grocery store settings, but have little choice in accepting these conditions in order to keep these low-paying jobs. The pandemic reinforces another well-known food system inequity: marginalized and impoverished minorities often suffer from diet-related diseases (i.e. cardiovascular diseases, diabetes) and/or malnutrition that place them at greater risk of morbidity and mortality from the coronavirus. Lockdowns and border closures are reducing economic opportunities such as day labor and agricultural markets in some regions, such as much of Africa; ensuing risks of food and nutrition insecurity for vast segments of the population threaten to set back development, increase social conflict, and catalyze migration. Finally, the current pandemic shines a spotlight on the systemic risk of infectious diseases to emerge and become globalized through local bushmeat markets and international wildlife trade, and how wildlife hunting and trade is influenced by land use changes, including by industrial agriculture. At the same time, adaptive responses to the coronavirus illustrate how more resilient and sustainable food systems could evolve going forward. To avoid supply chain disruptions, communities are increasing their reliance on local food systems, including an increase in urban gardening and community-supported agriculture programs. Small-scale farmers are innovating to connect with buyers and with each other, including through new online marketing initiatives. Entrepreneurs are identifying foods that would otherwise be wasted and directing them to food banks. Retailers and wholesalers are re-configuring their distribution networks to shift food to where it is needed most. Food pantries, local producers and food businesses are also collaborating with municipal governments to address food security gaps arising from COVID-19 impacts.

The food safety hazards considered in the publication are foodborne pathogens and parasites, harmful algal blooms, pesticides, mycotoxins and heavy metals with emphasis on methylmercury. There is also, a dedicated section on the benefits of forward-looking approaches such as horizon scanning and foresight, which will not only aid in anticipating future challenges in a shifting global food safety landscape, but also help build resilient food systems that can be continually updated as more knowledge is assimilated. By building a more widespread and better understanding of the consequences climate change has on food safety, it is hoped that this document will aid in fostering stronger international cooperation in making our food safer by reducing the global burden of these concerns.

Book excerpt: Updates for many countries have made it possible to estimate hunger in the world with greater accuracy this year. In particular, newly accessible data enabled the revision of the entire series of undernourishment estimates for China back to 2000, resulting in a substantial downward shift of the series of the number of undernourished in the world. Nevertheless, the revision confirms the trend reported in past editions: the number of people affected by hunger globally has been slowly on the rise since 2014. The report also shows that the burden of malnutrition in all its forms continues to be a challenge. There has been some progress for child stunting, low birthweight and exclusive breastfeeding, but at a pace that is still too slow. Childhood overweight is not improving and adult obesity is on the rise in all regions. The report complements the usual assessment of food security and nutrition with projections of what the world may look like in 2030, if trends of the last decade continue. Projections show that the world is not on track to achieve Zero Hunger by 2030 and, despite some progress, most indicators are also not on track to meet global nutrition targets. The food security and nutritional status of the most vulnerable population groups is likely to deteriorate further due to the health and socio economic impacts of the COVID-19 pandemic. The report puts a spotlight on diet quality as a critical link between food security and nutrition. Meeting SDG 2 targets will only be possible if people have enough food to eat and if what they are eating is nutritious and affordable. The report also introduces new analysis of the cost and affordability of healthy diets around the world, by region and in different development contexts. It presents valuations of the health and climate-change costs associated with current food consumption patterns, as well as the potential cost savings if food consumption patterns were to shift towards healthy diets that include sustainability considerations. The report then concludes with a discussion of the policies and strategies to transform food systems to ensure affordable healthy diets, as part of the required efforts to end both hunger and all forms of malnutrition.

Although consensus exists on pathways through which agriculture may influence nutrition-related outcomes, empirical evidence on agriculture’s contribution to nutrition and how it can be enhanced is still weak. This paper reviews recent empirical evidence (since 2014), including findings from impact evaluations of a variety of NSA programs using experimental designs as well as observational studies that document linkages between agriculture, women’s empowerment, and nutrition. It summarizes existing knowledge regarding not only impacts but also pathways, mechanisms, and contextual factors that affect where and how agriculture may improve nutrition outcomes. The paper concludes with reflections on implications for agricultural programs, policies, and investments, and highlights future research priorities.

The food industry is the largest sector of our economy; food touches everything from our health to the environment, climate change, economic inequality, and the federal budget. From the earliest developments of agriculture, a major goal has been to attain sufficient foods that provide the energy and the nutrients needed for a healthy, active life. Over time, food production, processing, marketing, and consumption have evolved and become highly complex. The challenges of improving the food system in the 21st century will require systemic approaches that take full account of social, economic, ecological, and evolutionary factors. Policy or business interventions involving a segment of the food system often have consequences beyond the original issue the intervention was meant to address. A Framework for Assessing Effects of the Food System develops an analytical framework for assessing effects associated with the ways in which food is grown, processed, distributed, marketed, retailed, and consumed in the United States. The framework will allow users to recognize effects across the full food system, consider all domains and dimensions of effects, account for systems dynamics and complexities, and choose appropriate methods for analysis. This report provides example applications of the framework based on complex questions that are currently under debate: consumption of a healthy and safe diet, food security, animal welfare, and preserving the environment and its resources. A Framework for Assessing Effects of the Food System describes the U.S. food system and provides a brief history of its evolution into the current system. This report identifies some of the real and potential implications of the current system in terms of its health, environmental, and socioeconomic effects along with a sense for the complexities of the system, potential metrics, and some of the data needs that are required to assess the effects. The overview of the food system and the framework described in this report will be an essential resource for decision makers, researchers, and others to examine the possible impacts of alternative policies or agricultural or food processing practices.
