Book excerpt: Newborn infants delivered in a compromised health state often require transport between secondary and primary care hospitals. The objective of this study was to measure and characterize Whole Body Vibration (WBV) exposures during simulated newborn infant inter-hospital ground transport and determine how vehicle-based vibration is transmitted through the chain of equipment used to support newborn infants and whether there is a need and potential for mitigation of these exposures. These WBV exposures were also compared to two typical daily activities of infants, riding in a car seat and riding in a stroller. A simulated newborn infant was transported over a typical transport route utilizing a standard stretcher system and two potentially mitigating strategies: a modified stretcher system with a stabilizing strut and a new, modified vibration dampening stretcher system. The average-weighted vibrations and the vibration dose values were calculated, and the frequency spectra profiles were examined. Relative to the floor measured vibration (0.36 m/s2), the standard stretcher system amplified the average weighted vibration through the chain of equipment nearly doubling the vibration at the interface where the simulated neonate rested (0.67 m/s2). These measures exceeded those found with the car seat and the indoor stroller. The new stretcher system with the built-in suspension reduced the exposure at the interface to 0.48 m/s2. In contrast, the strut-based stretcher system did not perform as expected, increasing the vibration at the interface (1.2 m/s2). Results were similar for VDV exposures. The frequency spectra present in the standard stretcher system was primarily between 4-15 Hz, raising concern because of the overlap with natural resonance frequency of the human body (4-12 Hz). The new stretcher with the built-in suspension effectively negated the vibrations in the 4-10 Hz range. The transmissibilities indicated that there is potential for further mitigation at points between the aluminum transfer sled and the interface. This research presents a unique mitigating strategy to reduce elevated neonate whole body vibration exposures and addresses potential further mitigation points. It also contributes information to the field for further development of standards and implementation of whole body vibration exposure limits that are applicable across the lifespan.

The high prevalence of low back pain in drivers of commercial motor vehicles is well-documented. A number of interventions to reduce low back pain in commercial motor vehicle operators have focused on reducing exposures to wholebody vibration (WBV). Objective. An intervention to be evaluated for the trucking industry is an air-filled ballistic seat pad designed to reduce exposure to WBV. The effectiveness of the seat pad in semi-trucks has not yet been established. Results from a previous pilot study involving 12-ton and 16-ton vibratory rollers used by Seattle Public Utility drivers suggest that the seat pad is not effective at very low speeds (1-3 mph). Methods. The current study measures, characterizes and compares WBV exposures in nine truck drivers who operated their trucks over the same roads. WBV exposures were compared between their existing air-suspension seat and the air-filled ballistic seat pad which sat on top of their existing seat. This study uses a Wilcoxon signed rank test to compare the seat pad’s effectiveness in reducing WBV exposures relative to their existing air-suspension seat. Results. Overall, the truck drivers' vibration exposures were above daily vibration action values set by the International Standards Organization (ISO) and the air-filled ballistic seat pad did not significantly reduce WBV exposures relative to the WBV exposures experienced with their existing air-suspension seat. Conclusions. In the semi-trucks evaluated in this study, the air-filled ballistic seat pad was not effective in reducing WBV exposures. The seat pad has been shown to be effective in reducing WBV exposures in public transportation buses and cars. The answer for why the air-filled seat pads were not effective in semi-trucks may lie in a future analysis of the power spectral densities, which shows the vibration exposures as a function of frequency. Buses and cars produce more high frequency vibration energy, and the air-filled seat pad has been shown to be effective in these vehicles when travelling at moderate to high speeds. Trucks may produce less high frequency vibration energy, and this may have diminished the air-filled seat pad’s effectiveness in the semi-truck evaluated in this study.

Book excerpt: This report describes a project that made measurements of whole-body vibration & noise were made in several Canadian Forces armoured vehicles to quantify & assess the exposure levels according to national & international standards. Measurements were made in the following vehicles: M113A2, Coyote, LAV III, Bison, and M113A2 ADATS (air defence anti- tank system). In all vehicles except the ADATS, noise & vibration signals were recorded when the vehicles were idling, driven over rough terrain, & driven on the highway. The ADATS was assessed while idling and while driven at 8 & 32 kilometres per hour on a paved track. For each condition in each vehicle, the vibration & noise were recorded at crew positions inside the vehicle for 5-10 minutes. Crew members were also asked to complete a reaction to noise & vibration questionnaire at the end of each measurement session. The noise levels in all of the vehicles are compared to exposure limits of the various standards. Means of reducing the noise exposure are also discussed. The vibration exposure was more difficult to assess because of the conflicting caution zones, action levels, & exposure levels set by the various standards.

Book excerpt: This is the second edition of the WHO handbook on the safe, sustainable and affordable management of health-care waste--commonly known as "the Blue Book". The original Blue Book was a comprehensive publication used widely in health-care centers and government agencies to assist in the adoption of national guidance. It also provided support to committed medical directors and managers to make improvements and presented practical information on waste-management techniques for medical staff and waste workers. It has been more than ten years since the first edition of the Blue Book. During the intervening period, the requirements on generators of health-care wastes have evolved and new methods have become available. Consequently, WHO recognized that it was an appropriate time to update the original text. The purpose of the second edition is to expand and update the practical information in the original Blue Book. The new Blue Book is designed to continue to be a source of impartial health-care information and guidance on safe waste-management practices. The editors' intention has been to keep the best of the original publication and supplement it with the latest relevant information. The audience for the Blue Book has expanded. Initially, the publication was intended for those directly involved in the creation and handling of health-care wastes: medical staff, health-care facility directors, ancillary health workers, infection-control officers and waste workers. This is no longer the situation. A wider range of people and organizations now have an active interest in the safe management of health-care wastes: regulators, policy-makers, development organizations, voluntary groups, environmental bodies, environmental health practitioners, advisers, researchers and students. They should also find the new Blue Book of benefit to their activities. Chapters 2 and 3 explain the various types of waste produced from health-care facilities, their typical characteristics and the hazards these wastes pose to patients, staff and the general environment. Chapters 4 and 5 introduce the guiding regulatory principles for developing local or national approaches to tackling health-care waste management and transposing these into practical plans for regions and individual health-care facilities. Specific methods and technologies are described for waste minimization, segregation and treatment of health-care wastes in Chapters 6, 7 and 8. These chapters introduce the basic features of each technology and the operational and environmental characteristics required to be achieved, followed by information on the potential advantages and disadvantages of each system. To reflect concerns about the difficulties of handling health-care wastewaters, Chapter 9 is an expanded chapter with new guidance on the various sources of wastewater and wastewater treatment options for places not connected to central sewerage systems. Further chapters address issues on economics (Chapter 10), occupational safety (Chapter 11), hygiene and infection control (Chapter 12), and staff training and public awareness (Chapter 13). A wider range of information has been incorporated into this edition of the Blue Book, with the addition of two new chapters on health-care waste management in emergencies (Chapter 14) and an overview of the emerging issues of pandemics, drug-resistant pathogens, climate change and technology advances in medical techniques that will have to be accommodated by health-care waste systems in the future (Chapter 15).
