Book excerpt: Introduction: Dental expansion is used to correct transverse discrepancies such as constriction of the maxilla or a posterior crossbite. Rapid maxillary expansion (RME) corrects the constriction of the maxilla by opening the skeletal sutures of the maxilla along with tipping and moving teeth. This standard treatment for the correction of transverse maxillary deficiency applies forces to the maxilla through the anchor teeth which can result in unfavorable changes to anchor tooth angulation and the buccal bone adjacent to these teeth. Objectives: The aim of this study was to compare changes in buccal bone width and height and intermolar inclination in adolescent patients pre and post rapid maxillary expansion and investigate the correlation of these factors with skeletal maturation using cervical vertebral maturation stages (CVM) and mid-palatal suture density ratio (MPSD) at the start of treatment. Methods: This study was conducted on cone beam computed tomography (CBCT) images of 25 patients, pre-, and post expansion. CVMS and MPSD were from the pre-treatment CBCT. Buccal bone thickness, buccal bone height and intermolar inclination were calculated pre-, and post expansion. The correlation between changes in alveolar bone variables and skeletal maturity using CVM and MPSD was investigated. Results: No statistically significant relationship was found between MPSD and changes in bone variables. Further, no statistically significant relationship was found between CVM stage and changes in bone variables. There were no statistically significant correlations; therefore we failed to reject the null hypothesis. Conclusion: There is no relationship between changes in alveolar bone variables around molar teeth following RME and pre-treatment CVM and MPSD.

Book excerpt: The purpose of this study was to use cone-beam computed tomography (CBCT) to assess changes in the volume and cross sectional areas of the upper airway in children with maxillary constriction treated by rapid maxillary expansion (RME). The study group consisted of 5 males and 9 females with mean age of 12.93 years with posterior cross bite and constricted maxilla who were treated with hyrax expander as part of their comprehensive orthodontic treatment. Pre and post RME CBCT scans were analyzed with 3D Dolphin 11.0 software to measure the retropalatal (RP) and retroglossal (RG) airway changes including volume and cross sectional areas. The transverse width changes were evaluated from the maxillary inter 1st molar and inter 1st pre molar mid lingual alveolar plate points. Pre and post RME scans were compared with paired t test and Pearson correlation test was done on data reaching significance. Only the cross sectional airway measured at posterior nasal spine (PNS) to Basion (Ba) level showed a statistically significant increase (P=0.0004). The minimal cross sectional area (MCA) was always found within the RP airway. The experimental group (n = 30) required maxillary expansion to correct the deficiency in the palatal, and the control group (n = 30) had fixed orthodontic treatment only. Subjects age, gender, predominant mode of breathing, length of treatment and initial and final measurements of upper and lower pharyngeal airway were recorded. Results using Student t-test showed that age and gender were no statistically significant difference between the two groups (p = 0.2810). Mouth breathers distribution showed no statistically significant difference between the two groups (p = 0.7866). Upper pharyngeal airway increased an additional 0.77 mm in the expansion group which was statistically significant (Student's t-test, t = 2.492), while the lower pharyngeal airway showed no statistically significant difference (Student's t-test t = 1.041). In conclusion rapid maxillary expansion during orthodontic treatment had a positive affect on the upper pharyngeal airway, with no significant change on the lower airway.

Book excerpt: Vibration forces (VF) have been shown to alter the formative and resorptive activities of bone. Studies have investigated the use of VF in applications such as the treatment of osteoporosis, bone fracture healing and implant osteointegration with favorable results. In dentistry, orthodontic tooth movement and maxillary suture expansion are common procedures typically requiring prolonged treatment durations with high relapse rates. We hypothesized that local, intermittent VF applications can enhance bone formation during rapid maxillary expansion and accelerate orthodontic tooth movement. Moreover, we also investigated expression of periostin/OSF-2, an adhesion molecule implicated in the formation of bone during maxillary suture expansion. Our results showed that intermittent VF significantly increased bone volume density of the expended palatal bone but limited the amount of palatal expansion and mineral apposition rate at the suture margins. Also, intermittent VF forces did not show statistically significant acceleration of orthodontic tooth movement but significantly enhanced bone volume density of the interradicular bone after tooth movement. Maxillary expansion was also shown to induce the expression of periostin which was proportional to the magnitude of the expansion force with increased bone mineral deposition.
