Book excerpt: The Defense Department (DOD) has taken several steps to better manage mental health costs under the Civilian Health and Medical Program of the Uniformed Services (CHAMPUS) and improve the quality of care for beneficiaries. DOD has undertaken initiatives and demonstration projects that use management techniques similar to those used by private-sector companies and health plans that intensively manage mental health care. DOD has also instituted controls over payments to psychiatric facilities and improved standards for residential treatment centers. As a result, CHAMPUS mental health costs leveled off in fiscal years 1990 and 1991. Several problems persist, however, including poor medical record documentation, potentially inappropriate admissions, excessive hospital stays, and poor-quality care. Inspections of residential treatment facilities continue to reveal significant safety and health problems, and corrective actions often took many months. Moreover, DOD has proposed further changes to its methods of reimbursing psychiatric facilities. DOD should adopt these additional changes because it pays considerably higher rates for comparable services than do other public programs.

Book excerpt: For a number of reasons, military beneficiaries--active-duty service members, military retirees, and their dependents--are heavier users of medical care than are comparable civilian populations. These services are currently provided by military treatment facilities (MTFs) or the civilian medical facilities, the latter through the Civilian Health and Medical Program of the Uniformed Services (CHAMPUS). However, over the past several years, the system has faced the twin challenges of downsizing in consonance with the rest of the Department of Defense and of controlling escalating health care costs. While care provided in the civilian sector can be more expensive than that provided in the MTFs, the free care available in MTFs sparks greater demand. Moreover, the MTF system was designed to meet wartime, rather than peacetime needs. In weighing the pros and cons of various alternatives, this report suggests that beneficiaries might prefer civilian health plans, as long as there is no erosion of benefits in making such a shift.

Book excerpt: Pursuant to a congressional request, GAO reviewed the Department of Defense's (DOD) mental health demonstration project for Civilian Health and Medical Program of the Uniformed Services (CHAMPUS) beneficiaries, focusing on: (1) the extent of program cost-savings; and (2) whether DOD oversight was adequate to ensure CHAMPUS beneficiaries' access to quality care. GAO found that: (1) the project's total costs were less than projected totals and averaged about $32 million annually; (2) DOD realized savings of $148 million in comparison with nationwide CHAMPUS mental health care costs; (3) reasons for the project's savings included shifts in utilization patterns from inpatient services to less expensive partial hospitalization and outpatient services, and reductions in provider reimbursement rates; (4) DOD oversight and contractor controls were insufficient to ensure CHAMPUS beneficiaries access to quality treatment; (5) DOD has not provided for effective independent project evaluation or quality-of-care monitoring; and (6) the mental health care contractor's system for assessing and authorizing care may restrict access to care and limit identification of quality treatment problems.
