Book excerpt: The evidence surrounding the prevalence of cervical cancer (CC) shows that about 80-90% of the disease becoming pronounced when they are at an advanced stage. Based on the literature, CC is a preventable disease. Studies have shown that the reported 528,000 annual cases and more than 266,000 fatal cases in developing countries could have been significantly reduced if the symptoms were known or if patients had effective preventive programs. In recent times, sub-Saharan Africa (SSA) has been deeply hit by this disease. According to some studies done in Cameroon, the prevalence of CC is about 13.4%. The incriminating agent is the human papillomavirus (HPV) which is found in 99.7% of cases of cervical cancer The HPV agent is sexually transmissible and there exist about 100 species which are not all pathogen.

Gardasil quickly became implicated in two pre-existing debates—about adolescent sexuality and pediatric vaccinations more generally. Prior to its market debut, Gardasil seemed to offer female empowerment, touting protection against HPV and its potential for cervical cancer. Gottlieb questions the marketing pitch’s vaunted promise and asks why vaccine marketing unnecessarily gendered the vaccine’s utility, undermining Gardasil’s benefit for men and women alike. Nelly Oudshoorn explains why it is that, although the technical feasibility of male contraceptives was demonstrated as early as the 1970s, there is, to date, no male pill. Ever since the idea of hormonal contraceptives for men was introduced, scientists, feminists, journalists, and pharmaceutical entrepreneurs have questioned whether men and women would accept a new male contraceptive if one were available. Providing a richly detailed examination of the cultural, scientific, and policy work around the male pill from the 1960s through the 1990s, Oudshoorn advances work at the intersection of gender studies and the sociology of technology. Oudshoorn emphasizes that the introduction of contraceptives for men depends to a great extent on changing ideas about reproductive responsibility. Initial interest in the male pill, she shows, came from outside the scientific community: from the governments of China and India, which were interested in population control, and from Western feminists, who wanted the responsibilities and health risks associated with contraception shared more equally between the sexes. She documents how in the 1970s, the World Health Organization took the lead in investigating male contraceptives by coordinating an unprecedented, worldwide research network. She chronicles how the search for a male pill required significant reorganization of drug-testing standards and protocols and of the family-planning infrastructure—including founding special clinics for men, creating separate spaces for men within existing clinics, enrolling new professionals, and defining new categories of patients. The Male Pill is ultimately a story as much about the design of masculinities in the last decades of the twentieth century as it is about the development of safe and effective technologies.

Cervical cancer and infection with human immunodeficiency virus are both major public health concerns in SA. This study describes knowledge of female adolescents and young adults (AYA) about HPV, cervical cancer and explores predictors of HPV vaccine and HPV self-sample testing acceptability. In this cross-sectional study, questionnaires were administered to 122 female AYA who involved in a longitudinal study (AYAZAZI) which examined risk factors involved with HIV acquisition. Results indicated that although awareness and knowledge about these topics was very low among participants, as were perceptions of risk of acquiring HPV and developing cervical cancer, acceptability was very high towards HPV vaccines for self (97%) and (future) children (95%), as well for self-sample testing (85%). No significant variables were found to be associated with risk perception or self-sample acceptance. A significant difference was found between participants' perceived risk of acquiring HPV, HIV, and developing cervical cancer compared to the risk they felt other female AYA in their communities were at. The most influential sexual and reproductive health (SRH) information source and significant influences on HPV vaccine recommendations for participants were health care providers. Findings from this study are important in designing effective cervical cancer control programs that can attract more AYA for HPV vaccines and screening. As the HPV vaccine has only recently been introduced at a national level in SA, this study about awareness and vaccine acceptability is timely.

Various strains of the human papillomavirus have been linked to the incidence of this disease. The gradual development of cervical cancer makes it one of the most preventable female cancers, as malignant cell changes can take up to two decades to occur. Regular Pap smears lead to early detection of persistent HPV infection. Together with HPV vaccination, Pap smears are effective in controlling cervical cancer incidence. Although these preventative programs are readily available in the U.S., screening, and vaccination rates have been consistently suboptimal among immigrants. Previous studies have examined the enablers as well as the challenges experienced by immigrants in accessing cervical cancer prevention services. The purpose of this qualitative research was to explore the attitudes, barriers, and sociocultural factors that facilitate the uptake of cervical cancer preventative services among Ghanaians in Ghana, and Ghanaian immigrants living in Georgia. Methods: Semi-structured interviews were conducted in two parts; (a) a preliminary study in Ghana with 35 females and 15 males, and (b) the main study in Georgia, U.S.A. among Ghanaian immigrants, with 17 females and 10 males. Nvivo 12 was used to code the interview transcriptions, from which emerging themes and sub-themes were identified. The Social Ecological Model and the Theory of Reasoned Action were applied to examine the impact of personal and contextual influences on the participants’ decision to access prevention programs. Results: Barriers to screening and vaccination included the lack of knowledge, fear of cervical cancer, fear of the side effects of the HPV vaccine, embarrassment with a physical examination, and cost. Facilitators to screening and vaccination included increased knowledge of and access to cervical cancer prevention programs, health insurance, and encouragement from healthcare providers to utilize these services. Immigration, improved knowledge, and access to Pap smear and HPV vaccines were the strongest influences of change. Conclusion: The study results show that health education and social support could significantly improve the willingness of Ghanaian immigrants to access Pap smear and HPV vaccination. These findings could serve as an outline for the implementation of related programs in Georgia, and other locations with similar high-risk populations.

Book excerpt: Examine a variety of determinants that contribute to HPV vaccination decisions, with a particular focus on the influence of the mass media. A hierarchical linear regression showed that three main factors accounted for 42.7% of the variance in individual intention to be vaccinated against HPV. They were: 1) belief that the vaccine is safe and that information about the vaccine provided by advertisements and the news media is trustworthy; 2) perceived family support regarding vaccination; and 3) use of interpersonal sources of information about the vaccine. Stronger beliefs in vaccine safety and information trustworthiness were also correlated with a positive overall impression of the HPV vaccine. The results suggest that college women may be significantly influenced by HPV information provided by advertisements, the news media, and interpersonal sources--despite the fact that respondents reported relatively low use of these sources. The survey also revealed that the women were highly informed about the HPV vaccine. Their knowledge, combined with the information they gained from the news media and interpersonal sources, may have helped develop and/or reinforce their beliefs regarding HPV vaccine safety, which contributed directly to the decision to be vaccinated. This study filled a gap in knowledge about how college women have responded to the availability of an HPV vaccine and what factors most impact their vaccination decisions. These findings can help guide health educators and public health professionals as they work to promote HPV vaccination and eradicate cervical cancer.
