It was a vaccine that enabled global eradication of the dreaded disease smallpox. Mass immunization of children forms the anchor of the strategy of the World Health Organization (WHO) to attain "health for all" status by the year 2000. Vaccinology is undergoing a dimensional change with the advances that have taken place in immunology and genetic engineering. Vaccines that confer short or inadequate immunity or that have side effects are being replaced by better vaccines. New vaccines are being developed for a variety of maladies. Monoclonal antibodies and T cell clones have been employed to delineate the immunodeterminants on microbes, an approach elegantly complemented by computer graphics and molecular imaging techniques. Possibilities have opened for obtaining hitherto scarce antigens of parasites by the DNA recombinant route. Better appreciation of the idiotypic network has aroused research on anti idiotypic vaccines. Solid-phase synthesis of peptides is leading to an array of synthetic vaccines, an approach that is expected to attain its full potential once the sequences activating suppressor cells are discovered and the rules for presentation of antigens to T and B cells are better worked out. A new breed of vaccines is on the horizon that seeks to control fertility. Originally conceived to intercept a step in the reproductive process, they are conceptual models for developing approaches to regulate the body's internal processes.

In some regions like the oesophagus transit it rapid and measured in seconds while in others like the colon transit is measured in hours and even days, commensurate with the relative slow fermentation that takes place in the large bowel. An hierarchy of controls, neural and endocrine, serve to regulate the various cellular targets that exist in the gut wall. These include muscle cells for contraction and epithelial cells for secretion and absorption. However, there are complex interactions between these digestive mechanisms and other mechanisms that regulate blood flow, immune function, endocrine secretion and food intake. These ensure a fine balance between the ostensibly conflicting tasks of digestion and absorption and protection from potentially harmful ingested materials. They match assimilation of nutrients with hunger and satiety and they ensure that regions of the GI tract that are meters apart work together in a coordinated fashion to match these diverse functions to the digestive needs of the individual. Anyone traveling in developing countries knows that the control of this invasive disease of the intestine is a priority task for physicians and public health authorities. Victims are essentially young children, and complications such as the hemolytic uremic syndrome make shigellosis a systemic disease rather than simply an infection of the colonic mucosa. However, "Westerners" should not consider shigeJlosis as an unlikely threat of the tropics. The disease arises in industrialized countries as soon as breaches in sanitation appear. A few months ago, at least 500 people developed shigellosis in northern France in an outbreak of Shigella sonnei infection due to accidental contamination of an urban water delivery system. The pathogenesis of shigellosis is an extraordinary topic of research because study of the invasion of the colonic mucosa addresses fundamental questions on. the molecular and cellular mechanisms by which a bacterial pathogen can pene trate non phagocytic cells, survive, multiply, spread in the intra cellular compartment, and eventually kill host cells. Further development of the infection within subepithelial tissues as well as the mechanisms that contribute to the eradication of this process have barely been studied.
