Book excerpt: Voluntary environmental agreements (VEAs) – generally agreements between government and business – have been regarded by many as a key new instrument for meeting environmental objectives in a flexible manner. Their performance to date has, however, also led to considerable criticism, with several parties arguing that they are methods for avoiding real action that goes beyond "business-as-usual". Is either of these positions justified? The aim of this book is to highlight and learn the lessons from existing experience, looking not just at results but also at specific elements of agreements and also at the process of the agreement itself. Lessons are drawn from experience from across the world, covering the full range of environmental challenges, and from the perspective of key stakeholder groups. Importantly, the book also presents tools for assessing and improving existing agreements and includes recommendations and guidelines for future agreements in key areas such as climate change. It also deals at length with the problem of how such agreements might be used in developing and transitional economies. The overall view of the book is that there is a real potential for the future use of VEAs as part of the policy mix and as a tool for sharing the responsibility for meeting environmental objectives. For the agreements to play this role, however, significant steps are needed to ensure that they are effective, efficient, equitable and appropriately linked to a portfolio of other instruments. The book is divided into four sections. First, existing agreements, their development and efficacy are considered; second, the prospects for voluntary agreements in developing and transitional economies are discussed; third, a range of authors examine the role of VEAs as part of the policy mix to combat climate change; and, finally, the book concludes with an examination of how new tools for evaluating and improving VEAs could be utilized in the future. Voluntary Environmental Agreements will be of interest not only to academics, governments and businesses wishing to understand this specific instrument, but also to those already implementing or considering applying VEAs to meet their environmental objectives.

Book excerpt: As the United States devotes extensive resources to health care, evaluating how successfully the U.S. system delivers high-quality, high-value care in an equitable manner is essential. At the request of Congress, the Agency for Healthcare Research and Quality (AHRQ) annually produces the National Healthcare Quality Report (NHQR) and the National Healthcare Disparities Report (NHDR). The reports have revealed areas in which health care performance has improved over time, but they also have identified major shortcomings. After five years of producing the NHQR and NHDR, AHRQ asked the IOM for guidance on how to improve the next generation of reports. The IOM concludes that the NHQR and NHDR can be improved in ways that would make them more influential in promoting change in the health care system. In addition to being sources of data on past trends, the national healthcare reports can provide more detailed insights into current performance, establish the value of closing gaps in quality and equity, and project the time required to bridge those gaps at the current pace of improvement.

Book excerpt: Critical congenital heart defects (CCHDs) are potentially life-threatening malformations that remain a significant cause of neonatal mortality and morbidity. Failure to diagnose these conditions shortly after birth may result in acute cardiovascular collapse and death. The identification of CCHDs by routine newborn clinical examination is routine in many countries, but consistently misses over a third of cases, and, although antenatal ultrasound screening can be very effective in early diagnosis, the provision and accuracy of ultrasound screening is highly variable. As most CCHDs present with mild cyanosis (hypoxaemia), which is frequently clinically undetectable, pulse oximetry is a rapid, simple, painless method of accurately identifying hypoxaemia, which has gained popularity as a screen for CCHD. This Special Issue of the International Journal of Neonatal Screening, devoted to "Neonatal Screening for Critical Congenital Heart Defects (CCHDs)", will consider the evidence for CCHD screening with pulse oximetry, the acceptability and cost-effectiveness of this intervention, the additional non-cardiac conditions which it may also identify, and international experiences of introducing CCHD screening across the globe.

Book excerpt: Frequent moves are a feature of Service life, and mobility can have a detrimental impact on children's emotional well-being and their educational attainment. The Committee's report examines issues relating to the education of the children of armed forces service personnel, including Service schools overseas and the performance of the Ministry of Defence's agency, Service Children's Education (SCE), which is responsible for providing these schools, as well as issues affecting Service children in UK state-maintained and independent sector day and boarding schools. The Committee concludes that Service personnel deserve assurance that their children's education will not suffer because of their parent's employment, and expresses its doubts that the DfES and the MoD currently take the interests of Service children sufficiently into account. Findings include: the need for better co-ordination between the MoD, the DfES and the devolved administrations to ensure continuity of education for children moving between different parts of the UK; the DfES needs to undertake work to determine how many Service children are in UK schools and the system for transferring student records between schools needs to be improved, particularly for Service children with special needs; and the MoD should increase the provision of telephones and internet access for Service personnel on operations so that they can communicate with their children.

For almost two decades, the citizens of Western Mexico have called for a cleanup of the Santiago River, a water source so polluted it emanates an overwhelming acidic stench. Toxic clouds of foam lift off the river in a strong wind. In Sewer of Progress, Cindy McCulligh examines why industrial dumping continues in the Santiago despite the corporate embrace of social responsibility and regulatory frameworks intended to mitigate environmental damage. The fault, she finds, lies in a disingenuous discourse of progress and development that privileges capitalist growth over the health and well-being of ecosystems. Rooted in research on institutional behavior and corporate business practices, Sewer of Progress exposes a type of regulatory greenwashing that allows authorities to deflect accusations of environmental dumping while “regulated” dumping continues in an environment of legal certainty. For transnational corporations, this type of simulation allows companies to take advantage of double standards in environmental regulations, while presenting themselves as socially responsible and green global actors. Through this inversion, the Santiago and other rivers in Mexico have become sewers for urban and industrial waste. Institutionalized corruption, a concept McCulligh introduces in the book, is the main culprit, a system that permits and normalizes environmental degradation, specifically in the creation and enforcement of a regulatory framework for wastewater discharge that prioritizes private interests over the common good. Through a research paradigm based in institutional ethnography and political ecology, Sewer of Progress provides a critical, in-depth look at the power relations subverting the role of the state in environmental regulation and the maintenance of public health.
