Book excerpt: This report is a systematic review and critical appraisal of the literature on the use of formal outpatient diabetes education as a therapeutic tool for self-management in the adult patients with type 2 diabetes. Formal patient diabetes education to promote self-management (PDE) at different levels of complexity and comprehensiveness has emerged from instructions by physicians, nurses, and dieticians in physicianþs offices and hospitals. Since 1990 there has been a shift from traditional approaches of PDE to focus on patient-centered perspectives, self-efficacy, self-management and empowerment issuses. Quantitatives research on the use of formal PDE as a therapeutic tool promote self-management in adults with type 2 diabetes has produced mixed results in terms of improved metabolic control and reduced risks for cardiovascular disease. The long-term diabetes control outcomes of formal PDE when used to promote self-management in adults with type 2 diabetes have yet to be established. Further investigations are needed to determine which types of programs or what components are most effective in improving the ability of adults with type 2 diabetes to self-manage their disease; or which category of patients might benefit most. Findnings from qualitative research studies helped to better understand the context in which formal PDE should be applied to be successful in promoting self-managemenet behaviors in adults with type 2 diabetes.

This paper presents an evidenced-base practice (EBP) change project addressing uncontrolled T2DM in adults at a private practice in Northern Texas involving the implementation of an educational intervention for patients with uncontrolled T2DM at the clinical practice setting. Outcomes were measured by using the Revised Diabetes Knowledge Test as the measurement tool and tracking changes in glycemic index of the participants pre and post intervention. Data analysis revealed an increase in knowledge acquisition and a reduction in glycemic index.. As a result of the findings of this EBP project, policy changes were made in the clinical practice setting, to include diabetes education as part of the treatment regimen. Elle comprenait l'exploration de la perspective du patient, une demarche explicative, des conseils hygieno-dietetiques adaptes, un objectif de comprehension commune avec le medecin et une evaluation a 3 mois des resultats. En confrontant les donnees du discours avec les modeles transtheorique et transactionnel par procedure de triangulation theorique, cette demarche a corrobore une dynamique d'adaptation coping, des processus experientiels et comportementaux favorisant ou pas des changements. L'etude a illustre l'interaction symbolique entre medecins et patients a travers les representations personnages que chacun se fait de l'autre, bloquant ou facilitant la comprehension commune. L'ACP est apparu comme un processus therapeutique systemique adapte a l'education therapeutique."

In her important and passionate new book, anthropologist Alcida Ramos explains this irony, exploring Indian and non-Indian attitudes about interethnic relations. Ramos contends that imagery about indigenous people reflects an ambivalence Brazil has about itself as a nation, for Indians reveal Brazilians' contradiction between their pride in ethnic pluralism and desire for national homogeneity.
