Book excerpt: Sir Hedley Atkins In an article in the London Times of November 26th 1973 their medical correspondent, Dr Tony Smith, discussed the many methods nowadays available for the treatment of common diseases. In this article he wrote: "Perhaps the most difficult of the common conditions is breast cancer, where the number of alternative treat ments is larger than most people suppose. After the surgeon has assessed the size and nature of the growth, how far it has spread, and the general health of the woman, he has to select the optimum treatment from a range that includes simple removal of lump, removal of the breast and removal of the breast together with the underlying muscle and the lymph glands in the shoulder region. Any of these operations may be combined with various forms of X-ray treatment; or the best chance may be given by treating the tumour with high-voltage X-rays without any operation at all. Each of these treatments may give a complete cure, and, in part, the choice depends on the surgeon's skill in assessing the type of cancer present and how far it is advanced. "The other element in the surgeon's choice, however, is his preference for one operation rather than another based on his experience and his interpretation of the results obtained by other surgical teams.

Book excerpt: The synthesis during the present decade of different arts and sciences to form oncology as a mUltidisciplinary subject is of profound importance for coordinating the clinical and research efforts to control a number of diseases that cause high mortality and morbidity in the human race and to elucidate their causation. These diseases traditionally have been grouped together under the general term of cancer, without any scientific reason and irrespective of many differences existing between them. The word cancer, because it is synonymous with diseases that cause suffering and death, naturally generates fear in people throughout the world. Cancer fortunately has a changing face caused by the realization that these diseases are different diseases with variable etiology and prognosis; they need different kinds of treatment, and even prevention is a practical proposition. The time has therefore come to delete the term cancer from our terminol ogy as unscientific and unhelpful and to substitute oncological diseases governed by the system of knowledge designated oncology. This conforms also with the designations used for other groups of diseases. In the divisions of oncology an important place is held by surgical oncology because many oncological diseases require surgical treatment alone, or in combination with radiotherapy and chemotherapy. Combina tion therapy is being used effectively for an increasing number of these diseases, and this trend will become more pronounced as chemotherapy develops. The results achieved with available chemicals and present dosage schedules are impressive.

Book excerpt: The ultimate "consumer" of the data presented at conferences on the primary treatment of operable breast cancer is the patient, and when, as in this disease, the benefits of therapy are relatively mod est, the availability and interpretation of the data from trials be comes an issue of primary importance. The effects of present treat ment are in fact such that more patients relapse despite therapy than are estimated to benefit from it. It is, therefore, extremely dif ficult for the physician to recommend unequivocally one particular adjuvant treatment modality for the vast population of women with breast cancer. The interpretation of results from clinical research-oriented pro grams is constantly applied, however, in the treatment of breast cancer patients outside of clinical trials. From presented or publish ed data, many physicians extrapolate indications for the use of a given treatment regimen for their patients, perceiving it as the "best available therapy. " It is essential that the "best available therapy" be selected individually for each patient. However, considering the modest effect of treatment upon outcome, it is imperative that those who provide the data - those who are involved in both pa tient care and clinical research - make it known that the best cur rent treatment for the population of breast cancer patients is avail able within the framework of clinical trials. In this way not only present-day patients but also future ones will derive the greatest benefit.

The program was prepared by Drs. MELVIN GRIEM, ELWOOD JENSEN, HAROLD SUTTON, JOHN ULTMANN, and ROBERT WISSLER. The purpose of the symposium was to present the current status of the challenging cancer problem, breast carcinoma, to the staff and students of this medical center and to students and in terested physicians from other institutions in the Chicago area. In a fashion similar to the other teaching symposia held in 1964, 1966, 1968,. and 1970, this symposium attracted over 450 physicians and scientists. In the course of one and one half days the audience had the opportunity to listen to 18 invited speakers and to the lively discussions. The formal presentations are recorded in these pages. This teaching symposium could not have been undertaken without the faithful assistance of the program committee, the cancer training grant education committee, the staff who recorded and transcribed the proceedings, and the editorial assistants.
