Book excerpt: On December 26, 2009, a 37-year-old male career fire fighter/paramedic (the victim) was ejected from the front passenger seat of a fire department ambulance during a rollover incident. The ambulance was responding with lights and siren to a car fire with injuries. A motorist in a privately owned vehicle (POV) turned into the path of the ambulance as it was passing the POV. The ambulance struck the POV, lost control, and left the right side of the roadway. The ambulance overturned three times, and the victim was ejected. The driver sustained minor injuries. The victim's use of a seat belt could not be determined with certainty. The victim was airlifted by helicopter to a local trauma center where blunt force chest injuries, and underlying medical conditions, contributed to his death on December 28, 2009.

Book excerpt: On March 7, 2013, , a 49-year-old male career driver-engineer ("the Engineer") reported for duty at 0730 hours for his 24-hour shift. In the afternoon, the Engineer completed a 25-minute training session for an upcoming physical ability test. Approximately 25 minutes later, his engine was dispatched to a motor vehicle crash. At the scene, the Engineer was assisting the ambulance paramedics with a stretcher when he suddenly collapsed. Cardiopulmonary resuscitation (CPR) and advanced life support (ALS), including cardiac monitoring, multiple defibrillation attempts, intubation, and intravenous line placement, began immediately. Despite CPR and ALS for almost an hour on the scene, in transport, and at the hospital, the Engineer died. The death certificate and the autopsy listed "sequelae of atherosclerotic cardiovascular disease" as the cause of death. Given the Engineer's underlying cardiovascular disease, NIOSH investigators concluded that the physical exertion of the training session and the stress of responding to the motor vehicle crash most likely triggered an arrhythmia that resulted in his sudden cardiac death. NIOSH investigators offer these recommendations to strengthen the fire department's (FD's) comprehensive safety and health program and to prevent future events. Modify the frequency of exercise stress tests required by the FD for annual medical evaluations. Modify the formula used by the FD to estimate a fire fighter's aerobic capacity. Phase in a mandatory comprehensive wellness and fitness program for fire fighters. Ensure that fire fighters are cleared for duty by a physician knowledgeable about the physical demands of fire fighting, the personal protective equipment used by fire fighters, and the various components of NFPA 1582.

He responded to several calls including a vehicle fire, medical call, and activated alarm. The following day, he worked with the Fire Prevention bureau for over 6 hours. In the evening, the FF/P shoveled snow at his home and then went to his bedroom. His family found him unresponsive in his bed 10–15 minutes later. They started cardiopulmonary resuscitation (CPR). Emergency medical services (EMS) found the FF/P unresponsive, pulseless, and not breathing. He was in asystole (no heart rhythm). On scene and enroute to the emergency department (ED), EMS provided CPR and advanced cardiac life support measures. ED staff treated the FF/D without success for 35 minutes and pronounced him dead at 2147 hours.

The fire fighters were on the front porch attempting to gain entry into the single family dwelling when they were shot by the resident. At approximately 1930 hours, the county 911 center received a call from a civilian who reported that he was outside of his brother’s house and his brother was not answering his phone calls or knocks on the front door. The caller reported that he had spoken with his brother earlier in the day and that his brother’s vehicle was parked in the driveway in front of his house. He further stated that his brother had known medical issues. He requested assistance in gaining entry into his brother’s house. Rescue Engine 827 with six volunteer fire fighters and Paramedic Ambulance 823 with two career fire fighter/paramedics were dispatched at 19:35 hours. After arriving on scene, the fire fighters met the homeowner’s brother in the driveway and observed that all visible windows were covered. The fire fighters knocked on the front door, announced their presence several times and checked for an open door, however they did not perform a 360-degree walk around. After again announcing their presence, the fire fighters began to force open the front doors. Forcing both doors took 5-8 minutes with multiple strikes from three fire fighters using a halogen tool, axe and a sledge. The fire fighters forced the metal outer door but had trouble forcing the inner wooden door and ended up knocking a lower panel out of the wooden door and reaching through the hole to open the door from the inside. Four fire fighters, two medics and the homeowner’s brother were standing on the small front porch and the steps in front of the door. As the door was opened the homeowner’s brother entered. The homeowner fired a pistol multiple times through the open doorway striking his brother, the fire fighter/paramedic and a volunteer fire fighter.

Book excerpt: On September 16, 2010, a 56-year-old male career Fire Fighter/Paramedic (FF/P) participated in rescue training that included classroom lectures, stretching exercises, and lifting/moving heavy concrete blocks. During the stretching exercises, the FF/P experienced chest discomfort. He and his paramedic partner administered and interpreted an electrocardiogram (EKG). The EKG revealed a slow heart rate but no changes suggestive of cardiac ischemia. The FF/P resumed the training, which, at that time, involved lifting and moving concrete blocks. This evolution lasted approximately 25 minutes after which crews were debriefed and dismissed for lunch. After walking to his vehicle, the FF/P collapsed. Despite cardiopulmonary resuscitation (CPR) and advanced life support (ALS) at the scene, in the ambulance, and in the hospital's emergency department (ED), the FF/P died. The death certificate and the autopsy listed "severe coronary atherosclerosis" as the cause of death with "cardiomegaly" as a significant other condition. Given the FF/P's severe underlying coronary artery disease (CAD), NIOSH investigators concluded that the physical exertion involved in performing the rescue training probably triggered his sudden cardiac death. NIOSH investigators offer the following recommendations to address general safety and health issues. It is unclear if these recommended programs would have prevented the FF/P's death. 1) Report signs or symptoms consistent with a heart attack to authorities for prompt medical evaluation. 2) Provide mandatory annual medical evaluations to all fire fighters consistent with the current edition of National Fire Protection Association (NFPA) 1582, Standard on Comprehensive Occupational Medical Program for Fire Departments. 3) Consider reviewing the fire department's policy for conducting member exercise stress tests. 4) Phase in a mandatory comprehensive wellness and fitness program for fire fighters. 5) Perform an annual physical performance (physical ability) evaluation for all members.
