Book excerpt: On April 30, 2004, a 58-year-old male career fire fighter (the victim) sustained a fatal head injury when he fell from a moving, cab-forward engine. The engine was responding to a reported gas odor with a fire fighter/driver and an officer in the cab, and two firefighters, including the victim, seated in the open jump seats. Upon departure from the station, the engine made a right turn from the apron onto the street. During this turn, the victim fell out of the drivers side jump seat door landing on the street and striking his head. He was treated at the scene for head trauma and transported to a local hospital. He died from his injuries three days after the incident.

After he positioned the mini-pumper at a hydrant, he began to unroll the four-inch supply hose. Soon after unrolling the hose he collapsed. A crew member, just arriving at this location, initially thought the victim was checking something under the mini-pumper. The crew member asked the victim what he was doing, but quickly noted he was unresponsive with shallow respirations. After notifying dispatch of the situation, the crew member began cardiopulmonary resuscitation (CPR). Approximately 63 minutes later, despite CPR and advanced life support (ALS) administered on the scene and at the hospital, the victim died.

Four other fire fighters were injured, one critically, while fighting the fire or performing search and rescue for the victim. The fire started near a dumpster on the exterior of the structure and extended through openings in the loading dock area into the storage area, and then into the main shopping area of the supermarket. The fire progressed to five alarms and involved more than 100 personnel. Fire fighters removed the victim from the structure and transported him to a local hospital, where he was pronounced dead.

The victim was alone while driving an engine with lights and sirens operating to a dispatched medical emergency on a highway. He was following a rescue truck with an officer and a fire fighter who were dispatched from the same station. The victim was approaching an intersection as a tractor trailer pulled out in front of him. The victim struck the tractor trailer and the force of the impact pinned him in the drivers seat. Rescue crews extricated the victim and flew him to a local trauma center where he died as a result of his injuries.

While enroute, as the engine was rounding a bend and accelerating up a slight grade to enter a highway, the victim lost her balance and fell from the apparatus onto the road. The victim was treated at the scene for multiple traumatic injuries and transported to a local hospital. She died from her injuries five days after the incident.

Book excerpt: On June 16, 2001, a 22-year-old male career fire fighter sustained injuries while fighting a residential house fire that subsequently resulted in his death. Central Dispatch received notification of a smoke detector alarm at a single-family residence. After several apparatus had arrived on the scene, the victim arrived in Tanker 34, donned his gear, and approached the house. He was met by another firefighter who told him the fire conditions. The victim proceeded to the garage area, and shortly thereafter, a partial roof and garage door collapse occurred, trapping him. A civilian bystander notified the Incident Commander (IC) that a firefighter was down. The IC ran to the garage area and helped pull the victim out from the debris. Emergency medical personnel moved the victim to the street and began administering first aid. The victim was flown to a regional hospital where he remained until his death on July 12, 2001.

Book excerpt: Drug overdose, driven largely by overdose related to the use of opioids, is now the leading cause of unintentional injury death in the United States. The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.
