Serious cardiovascular diseases, e.g., myocardial infarction, heart failure, cardiac dysrhythmias, and stroke, are resulted from cardiovascular remodeling secondary to the cardiovascular risk factors, including hypertension, atherosclerosis, cigarette smoking, diabetes mellitus, hyperlipidemia, aging, etc. Cardiac remodeling involves a series of molecular, cellular, and interstitial alterations, which manifest clinically as changes in size, mass, geometry, and function of the heart. In addition, vascular remodeling is an active process of structural alteration involving vascular wall thickening and lumen stenosis due to endothelial injury, vascular smooth muscle cell proliferation, and matrix deposition. This results in atherosclerosis and/or stenosis followed by myocardial infarction, stroke, etc. Thus, studies on the mechanism of cardiovascular remodeling and new therapeutic approaches are important scientifically and clinically. Cardiovascular remodeling is a response of specific cardiovascular cells, including cardiomyocytes, endothelial, smooth muscle cells, and interstitial cells, to noxious hemodynamic, metabolic, and inflammatory stimuli. The remodeling may initially be functional, compensatory, and adaptive, but it progresses to structural alterations that become pathogenic when the damaging stimuli are sustained. The mechanism of cardiovascular remodeling is complex, and recent studies focus on improving and/or reversing the remodeling by clarifying molecular mechanisms and identifying novel therapies. Pharmacological anti-remodeling, RNA-based (long non-coding RNA and microRNA) treatment, cell-based (iPS) cardiac repair, and cell-free exosome approaches have been shown to improve cardiovascular remodeling alongside proangiogenic, pro-survival, proliferative, and immunogenic factors, stimulating endogenous proliferative potential of cardiomyocytes. However, though advanced studies have progressed, the prognosis of the affected patients remains poor. Therefore, understanding novel mechanisms, identifying remodeling biomarkers, and developing new therapeutic drugs and approaches can lead to new therapeutic options for treating patients with cardiovascular diseases.

The cardiac remodeling involves loss of cardiac myocytes, reactive hypertrophy of the remaining cells, and increased extracellular matrix (ECM) and fibrosis in the aging heart. In contrast, exercise training not only improves cardiac function, but also reduces the risk of heart disease. However, the ability of exercise training to modulate ECM and remodeling in the aging heart remains unknown. Therefore, the purpose of this study was to determine the effects of exercise training on ECM remodeling in the aging heart. We hypothesized that (1) exercise training would attenuate age-related changes in left ventricle morphology including extramyocyte space and collagen contents, and (2) exercise training would ameliorate age-induced changes in ECM-related factors including MMPs, TIMPs, TNF-[alpha], TGF-[beta]1, and [alpha]-SMA in the heart. Three and 31 month old Fischer 344 x Brown Norway F1 hybrid rats were assigned to four groups: young sedentary (YS), young exercise-trained (YE), old sedentary (OS), and old exercise-trained (OE). Exercise training groups walked briskly on a treadmill for 45 min/day (12° incline) at 20m/min (young) or 10 m/min (old), 5 d/wk for 12 wk. We found that endurance exercise training might ameliorate the ageinduced increase in extramyocyte space and collagen contents of the left ventricle. Exercise training might protect against age-induced fibrosis by increasing MMP-2, MMP-14 in the soluble fraction and MMP-1, MMP-3, MMP-14 in the insoluble fraction of old rat hearts. Conversely, exercise training might reduce the fibrosis by decreasing TIMP-1 in the soluble fraction of old rat hearts. Further, exercise training reduced potential upstream pro-fibrotic mediators including TNF-[alpha] and TGF-[beta]1 in the aging rat hearts. These results are the first to demonstrate that exercise training has a protective effect against age-induced extracellular collagen matrix remodeling in the aging heart, associated with increased MMP-1, -2, -3, -14 and decreased TIMP-1, TNF-[alpha], and TGF- [beta]1.

Advanced aging imposes a cadre of chronic disease including cardiovascular diseases. The aging heart displays unfavorable changes in both geometry and function including increased left ventricular (LV) mass, reduced cardiac reserve capacity, myocardial fibrosis and increased risk of arrhythmia, contributing to poor exercise capacitance (effort intolerance) and frailty in the elderly. In addition, aging also increases the risk of many chronic diseases that jeopardize the cardiovascular function and quality of life in the elderly. With progressive aging, several pathological conditions including obesity, type II diabetes mellitus, Alzheimer’s disease and cancer are on the horizon. For example, obesity has been considered as a trigger for premature aging, leading to pathological cardiac remodeling and compromised ventricular function. Alzheimer’s disease is a neurodegenerative disease featured by accumulation of amyloid plaque and neurofibrillary tangles. Alzheimer’s disease may provoke cardiovascular dysfunction impairment of diastolic function. Over the past decades intense effort has been made towards understanding the mechanistic framework behind aging, obesity and Alzheimer’s disease in the pathogenesis of cardiovascular anomalies. An emerging role of dysregulated autophagy, a conservative homeostatic process for cellular quality control by way of disposal and recycling of cellular components, has surfaced in cardiac anomalies under all these pathological settings. To this end, this dissertation research will focus on the underlying mechanism of cardiac remodeling and contractile dysfunction during the normal aging process, as well as aging-associated conditions such as obesity and Alzheimer’s disease. The major findings from my work revealed that (1): Ablation of TLR4 restores autophagy level through the regulation of NCoR1/HDAC and improves heart function in aging; (2): Knocking out of Akt and AMPK facilitates premature cardiac aging in a joint manner; (3): Mitochondrial aldehyde dehydrogenase (ALDH2) overexpression protects obese heart through SUV39H1-Sirt1 signaling; and (4): Melatonin protects Alzheimer’s disease-associated heart dysfunction through cGAS-STING regulated autophagy in the presence of ALDH2.

Other prevalent diseases that have been associated with age-related epigenetic changes include cancer and diabetes. Paternal age and epigenetic changes appear to have an effect on schizophrenia and epigenetic silencing has been associated with several of the progeroid syndromes of premature aging. Moreover, the impact of dietary or drug intervention into epigenetic processes as they affect normal aging or age-related diseases is becoming increasingly feasible.
