Book excerpt: The autonomic nervous system (ANS) plays a central role in the pathogenesis of several cardiovascular diseases such as hypertension, pulmonary hypertension, myocardial infarction, atrial fibrillation, ventricular tachyarrhythmias/fibrillation (VT/VF) and heart failure. Cardiovascular diseases (CVDs) are the leading cause of death -globally, taking an estimated 17.9 million lives each year ( 31 % of all deaths worldwide ). Mechanisms implicated in the pathophysiology of CVD include autonomic nervous system (ANS) dysfunction, endothelial dysfunction , vasoconstriction, and systemic inflammation. Today, several therapeutic options are available to treat heart and cardiovascular patients, mainly medical therapies targeting specific pathways in vascular biology and physiology, centered around vasoconstriction, cell proliferation, sympathetic overactivity, defective renin-angiotensin-aldosterone system and ANS imbalance. Those drugs have improved the quality of life and survival of patients. On the other hand, they do not cure the disease and are associated with side effects like dry cough, flushing, peripheral edema, hyperkalemia, hypotension, and reduction of cardiac output. Despite significant advances, CVDs remain a serious, life-threatening condition and new therapeutic modalities must be pursued. Neuromodulation therapies, which is one of the upsurging fields of modern medicine, involving a wide range of specialties and a variety of disorders. It is the process of stimulation, inhibition, regulation, or therapeutic alterations of the electrical or/and chemical activities, in the central, peripheral, or autonomic nervous systems that can be directed toward reverse/prevent progression of cardiovascular disease, by using implantable or non-implantable medical devices such as pacemakers, neural stimulators or micro-infusion pumps.

To determine why some patients experience sudden death while others sustain life in the presence of severely compromised cardiac function, the neuronal control of cardiac electrical and mechanical events must be considered. Starting at the level of individual neurons and building upwards, this book describes the synergistic interactions that occur among intrathoracic and CNS feedback loops to permit precise control of regional cardiac behavior. On this basic science foundation, subsequent clinical chapters explore the remodeling that occurs in this system with aging, with the evolution of specific cardiac pathologies, and with the psychological concomitants of heart disease. It describes structural and functional properties of the cerebral circulation that are unique to the brain, an organ with high metabolic demands and the need for tight water and ion homeostasis. Autoregulation is pronounced in the brain, with myogenic, metabolic and neurogenic mechanisms contributing to maintain relatively constant blood flow during both increases and decreases in pressure. In addition, unlike peripheral organs where the majority of vascular resistance resides in small arteries and arterioles, large extracranial and intracranial arteries contribute significantly to vascular resistance in the brain. The prominent role of large arteries in cerebrovascular resistance helps maintain blood flow and protect downstream vessels during changes in perfusion pressure. The cerebral endothelium is also unique in that its barrier properties are in some way more like epithelium than endothelium in the periphery. The cerebral endothelium, known as the blood-brain barrier, has specialized tight junctions that do not allow ions to pass freely and has very low hydraulic conductivity and transcellular transport. This special configuration modifies Starling's forces in the brain microcirculation such that ions retained in the vascular lumen oppose water movement due to hydrostatic pressure. Tight water regulation is necessary in the brain because it has limited capacity for expansion within the skull. Increased intracranial pressure due to vasogenic edema can cause severe neurologic complications and death.

In comparison with psychotropic medications, we now know that ECT can act more effectively and more rapidly, with substantial clinical improvement that is often seen after only a few treatments. This is especially true for severely ill patients -- those with severe major depression with psychotic features, acute mania with psychotic features, or catatonia. For patients who are physically debilitated, elderly, or pregnant, ECT is also safer than psychotropic medications. The findings of the American Psychiatric Association (APA) Task Force on ECT were published by the APA in 1990 as the first edition of The Practice of Electroconvulsive Therapy, inaugurating the development of ECT guidelines by groups both within the United States and internationally. The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.
