When, in the early 1960's, Dr. F. Mason Sones demonstrated that visualization of the coronary anatomy in living humans was not only feasible but sufficiently safe and reliable to be used as a clinical tool in the evaluation of patients with known or suspected ischemic heart disease, the thus far somewhat neglected area of coronary circulation became the focus of interest. Naturally, for a considerable period of time a great deal of emphasis was placed upon coronary anatomy. Simple relations between narrowing lesions, impediment to flow, and prognosis were assumed to exist. Spectacular results of surgical coronary revascularization seemed to confirm this concept. Gradually it has become evident that the pathophysiology of coronary artery disease is considerably more complex. Diagnostic methods were introduced to assess and quantify exercise-induced myocardial ischemia. At first, these tests were used mainly to achieve a more discriminative selection of candidates for coronary arteriography and the coronary arteriogram remained the gold standard. Currently, these techniques have evolved to the point where they provide valuable functional and metabolic information. They have become powerful independent tools in clinical investigations and evaluation of individual patients.

NOBLE The "Starling's Law of the Heart" and "The Frank-Starling Mechanism" have long been the cornerstone of cardiac mechanical physiology. It is often forgotten that Frank and Starling carried out fundamentally different exper iments. Frankl measured the isovolumic pressure developed by frog heart at different volumes. He therefore discovered the pressure-volume-volume rela tionship which depends directly on the force-length relationship of the 2 sarcomeres. Starling ,3 studied cardiac shortening as manifest by cardiac output and its relationship to end-diastolic conditions as manifest by right atrial pressure. Thus he was studying the ability of cardiac muscle to shorten more at a given load from a greater initial length. Starling in the promulga 4 tions of his law implied a common mechanism for these two phenomena and spoke of the "energy liberated" being a function of initial muscle fiber length. However, there has been much confusion about the interrelationship between the two different aspects studied by Frank and Starling. The 1960s saw the era of isolated cardiac muscle mechanics, beginning with 5 the paper of Abbott and Mommaerts. Whole muscle length-tension relations were equated with sarcomere-length-tension relations by fixation of muscle at a particular point on the curve and determination of sarcomere length by electronmicroscopy.

Book excerpt: According to Schopenhauer problems are usually passing through three stages: - in the first stage they are ignored or just smiled at, - in the second stage they are fought, and in the third stage they are considered to be self-evident, just taken for granted. Whereas digital subtraction angiography (DSA) has obviously reached stage three of that scale, i.e. routine use in radiology, digital angiocardiography, in particular imaging the heart and coronary circulation, is still on its way to the final goal: the filmless heart catheterization laboratory for all invasive and interventional procedures. A few pioneers have already completely abandoned the conventional cine coronary and angiocardiographic technique, others - as we do -still combine both digital and conventional methods in clinical routine, but most cardiol ogists up till now stay sceptically aside. We hope that at least some of the articles published in this volume may convince more and more cardiologists that digital imaging procedures are the method of choice, in particular if quantitative assessment of the anatomical or functional status of the cardiovascular system is required (pre- and post operations or pre-and post-interventions). Such a critical control of all therapeutic procedures, be it by surgical, catheter-or medical 'interventions', is indeed an urgent and widely under estimated or neglected requirement.

Bypass surgery as well as transluminal coronary angioplasty would not have been possible without coronary angiography serving as a 'midwife' in their evolu tion. Despite the widespread and long-standing use in clinical practice, however, the interpretation of coronary angiograms has not changed very much since the early days. Most angiogr~s are still reviewed in a visual and semi-quantitative and thus often very subjective way. In the face of an almost exploding field for interventional catheterization including thrombolysis, balloon dilatation, and other rapidly evolving techniques for transluminal revascularization or recanalization, a more detailed and quantitative analysis of coronary arteriograms is urgently required. In addition to the delineation of coronary morphology, we need dynamic and functional information about flow and perfusion to understand the physiological significance of anatomic abnormalities. Coronary arteriography contains and can provide most of this information. With the application of appropriate techniques, it can be made available in the catheterization laboratory even during the patient's investiga tion, thus facilitating and improving clinical decision making. Objective and reproducible analysis will furthermore enhance our understanding about the pathophysiology of coronary disease.

On 5-8 May, 1985, an international symposium on 'Invasive Cardio vascular Therapy' was held in Cologne, which provided a survey on the experi ment~l and routine therapeutic measures presently available and practiced in cardiovascular medicine. This volume is based on the oral presentations given during the symposium. In five chapters the most important traditional and new interventional techniques are discussed by experts in the field. Chapter I contains a description of results from catheter palliation of congeni tal shunt disorders or relief of congenital pulmonary or aortic valve stenoses as well as the recent experience with surgical repair of single ventricle, Fallot's tetralogy and tricuspid atresia. Chapter II presents the surgical results of valve replacement with different valve prostheses in acquired valvular disease, the surgical management of bacte rial endocarditis, as well as the various techniques of partial transient left heart support devices and of cardiac transplantation. In chapter III, the invasive management of chronic coronary heart disease, peripheral vascular disease by balloon catheter or laser techniques, and the surgical approach to coronary heart disease are discussed.

Since cardiovascular nuclear imaging emerged as a new subspecialty in the mid-1970s, the field has gone through an explosive growth. Radionuclide techniques became readily recognized as important new diagnostic aids in the armamentarium of the clinical cardiologist. Initially, cardiovascular nuclear imaging focused on static myocardial imaging using either thallium-201 or technetium-99m-pyrophosphate for diagnosing acute myocardial infarction. Shortly thereafter, multigated equilibrium radionuclide angiocardiography became the most widely used noninvasive method for assessing cardiac function. Furthermore, attention and clinical application shifted towards the use of radionuclide techniques in conjunction with exercise testing, either with thallium-20 1 myocardial perfusion imaging or technetium-99m left ventricular function studies. The future of cardiovascular nuclear imaging appeared exciting and promising. However, around 1980 pessimists predicted the premature demise of cardiovascular nuclear imaging with the introduction of digital subtraction angiography and nuclear magnetic resonance imaging. These doomsayers have been proven wrong: in 1985 cardiovascular nuclear imaging is thriving and, in many centers, even expanding. Although digital substraction angiography and magnetic resonance imaging provided exquisite anatomic detail, for practical evaluation of patients with ischemic heart disease - in the Coronary Care Unit or exercise laboratory - nuclear techniques appeared to be more practical.

A copious amount of representative figures and illustrations is included so that the reader is able to understand the clinical application of each modality in the various cardiovascular pathologies and the echocardiographic diagnosis. In keeping with the concept of an integrated echocardiographic examination, the two-dimensional, M-mode and Doppler echocardiographic findings are included in many of the diseases. Usually, the echocardiographic examination is approached first by two-dimensional imag ing to give an understanding of the anatomical correlations, if necessary adding M-mode to clarify the time course of intracardiac movements, and secondly by each Doppler technique to evaluate the hemodynamic conditions. When familiar with these approaches, one can make not only the diagnosis of many cardiovascular diseases with echocardiography alone, but also the rational and expeditious management of patients. Today, some of the cardiac diseases (Atrial Myxoma, Atrial Septal Defect, Infective endocarditis and so forth) have been oper ated based only on echocardiographic findings.
