In the last years the Spanish Society of Cardiology has shown a great interest in the continuous medical education in cardiology. Consequently, the Society has decided to publish this book, an update of the therapeutics in cardiology, of which a Spanish version is also available. World renowned experts and outstanding Spanish cardiologists were invited to review all aspects of therapy in cardiology. We would like to thank the Spanish Society of Cardiology and the Catalan Cardiac Society for the generous assistance they have given us during all stages of book preparation. It should be possible for the data to be easily entered into the database-structure and afterwards also easily read, corrected and processed. The later analysis of data from such a database is greatly enhanced by the availability of special query languages and statistical analysis programs, not only for serial items but also for large combinations of data. Query languages, such as SQL (Structured Query Language) developed especially for these purposes, make databases easily accessible, also to researchers who may not be very well versed in computer programming. The cardiological/medical clinician and researcher of today is of necessity confronted more and more with computer-based data storage. Interest is of course focused primarily on the clinical use of such databases more than on the technical design itself, except for some very specific, personalized applications. For the latter approach, there are at present many software packages commercially available, especially designed for use in the personal computer environment. This book is comprised out of a number of contributions by various authors with differing backgrounds and from many different countries. The editors, being a cardiologist and an information scientist, have strived to achieve an equilibrium between these two fields. The chapters in this book form a cross-section of the many approaches to database design and implementation in the area of cardiology.

Book excerpt: The first invasive evaluation of cardiac arrhythmias in humans was performed in 1967 in Paris (Prof. P. Coumel) and Amsterdam (Prof. D. Durrer). This was the start of a rapid increase in our knowledge of the diagnosis, mechanism and treatment of cardiac arrhythmias. In that same year Prof. Hein J.J. Wellens became cardiologist in the Wilhelmina Gasthuis in Amsterdam. Initially in Amsterdam (1967-1977) and later on in Maastricht (from 1977), he was the driving force for many breakthroughs in clinical cardiac electrophysiology. With an active interplay between the knowledge derived from the 12-lead electrocardiogram and the recordings made with invasive electrophysiology, he composed new ideas leading to major contributions in clinical cardiac electrophysiology and, more generally, in arrhythmology. He published over 650 scientific papers and 14 books, and had numerous functions within scientific boards of prestigious journals. In addition he trained more than 120 cardiologists in clinical cardiac electrophysiology. On the occasion of the congress `2000, Future of Arrhythmology: Lessons From the Past, Promises For Tomorrow', we highlight the scientific work of Prof. Hein J.J. Wellens. A selection of more than 60 articles over the whole time span has been selected. These articles are accompanied by comments from an expert, co-worker and/or former fellow in order to place the paper in a scientific time frame, including the relationship of the author with Prof. Hein J.J. Wellens.

The title, although quite appropriate, does not adequately indicate the range of topics considered or the rational interrelationships among them. Indeed, perhaps the most important point to be learned from the book is that a serious consideration of the response of the heart to mechanical overload, ischemia, or excessive humoral stimuli must include evaluation of each of the topics in the table of contents. The heart responds to stress through alterations in both structure and function. How these changes are brought about is the subject of the initial chapters. These consider first the normal regulation of gene expression in the heart, the rapid response to mechanical overload that leads to both quantitative and qualitative changes in the contractile proteins, and our current understand ing of the signals that might be elicited by stress and alter gene expression. One chapter emphasizes the fact that, regardless of the nature of the stress, the common denominator is a discrepancy between energy requirements and expenditure. The central role of cellular acidosis in initiating the sequence of responses to stress and the possible roles of peptide regulators of transcription and protein regulators of translation are considered in detail.

Book excerpt: Many noninvasive examination methods of the heart have not held out against the invasive methods, which modern cardiac therapy, surgically or with catheterization, requires. They have disappeared completely or are only used by isolated groups of researchers. However, there is an obvious tendency to apply the invasive procedures as the last diagnostic possibility. In the attempt to select clinically relevant methods, the expert authors of this book demonstrate that echocardiography, expanded with contrast and Doppler, has been developed into one of the most important noninvasive methods. The results with tissue characterization show that the possibilities of this method have not yet been fully explored. Nuclear procedures are widely used, although they should only be applied in direct connection with clinical cardiology. The new lead methods of the ecg, such as ecg-mapping, show that standard electrocardiography of electrical functions is not yet fully exploited. The rapidly developing method of computer tomography is also being applied to cardiology. Since nuclear magnetic resonance requires extensive equipment con struction, its future is as yet unsure. Of course, a book like this does not intend to treat the subject of noninvasive cardiology in extensive detail. Established methods like standard electrocardio graphy, phonocardiography and sphygmography are not discussed. The aim of this book is rather to demonstrate the trend of present developments in the field.
