Une part importante des patients de l'Unité de Post-Urgence Gériatrique (UPUG) du Centre Hospitalier de Pau sont admis pour chute. Ce service leur propose une prise en charge globale, sur une durée d'hospitalisation courte, avec un objectif de retour à domicile. LES OBJECTIFS de cette étude sont : déterminer le taux de récidive précoce de chute (un mois après la sortie de l'UPUG), et en rechercher les facteurs de risque. Nous avons ainsi mesuré la prévalence de la chute comme motif d'hospitalisation à l'UPUG en 2009, et décrit les caractéristiques de cette population chuteuse, METHODES : Etude prospective et descriptive sur l'année 2009, concernant les patients hospitalisés pour chute, avec sortie à domicile, soit 134 patients. Les données de l'Evaluation Gérontologique Standardisée ainsi que la plupart des facteurs de risque de chute connus ont été recueillis. Un rappel téléphonique des patients un mois près leur sortie a permis d'identifier les patients ayant récifivé leur chute. RESULTATS : 24,3 % des patients de l'UPUG ont été admis pour chute. Parmi les patients rentrés à domicile, 8,96 % ont chuté au moins une fois dans le mois suivant leur sortie. Ces patients rechuteurs ont été plus hospitalisés que les autres. L'entourage du patient est l'élément le plus déterminant dans la protection de la récidive de chute. La présence d'une polymédication et la désadaptation à l'effort, apparaissent ici comme facteurs protecteurs. CONCLUSION : La prise en charge des chutes à l'UPUG du CH Pau y est l'activité de soin la plus importante. Cette étude, qui présente de nombreuses limites, a permis de mettre en évidence la gravité de la récidive de chute, et le fait que les patients rechuteurs n'étaient pas les plus polypathologiques, les plus dépendants ou les plus âgés. L'interprétation de certains résultats doit cependant être prudente, car en contradiction avec les données de la littérature scientifique.

Generalized anxiety disorder (GAD) -- For a number of years GAD was thought of as a residual disorder occasionally seen in combination with more primary conditions. Nowadays, we know that GAD is an independent condition that is prevalent, persistent, and disabling.
