Book excerpt: Despite an abundance of data concerning colorectal cancer (CRC) screening among various ethnic groups, research is lacking about colorectal cancer screening and early detection beliefs and knowledge among U.S. Slavic immigrants. The aim of this study is to identify knowledge, beliefs and attitudes related to CRC screening practices among the Slavic community in Sacramento. Using a cross-sectional, qualitative design, a purposive sample of seven participants aged 50 and older were recruited. Most participants viewed CRC as someone's fate, difficult to prevent and leading to a terminal diagnosis. Barriers to CRC screening were identified such as lack of full understanding about CRC tests, associated risks with procedure and fear of diagnosis. Cultural beliefs, previous experiences, practices and language barriers must be considered in developing guidelines and recommendations for primary healthcare providers to improve participation of Slavic adults in CRC screening.

CRC in African Americans has increased incidence and mortality to whites; the site of CRC in African Americans is a more proximal site. In almost all cases, however, this disease is entirely treatable if caught early by colonoscopy (MedlinePlus, 2004). The study will investigate the effects of colonoscopy screening and patient education concerning colorectal cancer on the occurrence and mortality of colorectal cancer in African Americans. The study will be a survey design; it will determine if the attitudes of African American patients who receive education about colorectal cancer (CRC) with an additional question and answer session and recommended colonoscopy screening have a change in beliefs and knowledge of CRC. The patients will complete a questionnaire in the beginning of the study and at the end of the study; the results of both questionnaires will be compared and the reviewer will look for specific changes in attitude toward CRC.

For many years, screening was practised without debate, and without evidence, but in the 1960s serious challenges were raised about many of the screening procedures then being practised. Benefits and harms of screening must be measured in high quality trials, and the benefits of screening must be weighed alongside the negative side-effects.
