Book excerpt: The ability to measure accurately the hormones regulating calcium homeosta sis is the fundamental first step toward understanding the roles these hormones play in health and disease. Techniques for such measurements have only been available for the past 10 years or so and remain in a state of rapid development. Sensitive parathyroid hormone (PTH) radioimmunoassays appeared in the early 1970s, and with them came a whole new appreciation for the prevalence and implications of hyperparathyroidism, primary or secondary, in the popu lation. The calcitonin (CT) radioimmunoassay came later and achieved rapid success in the. diagnosis of a previously poorly understood cancer, medullary carcinoma of the thyroid, frequently associated with the familial multiple endo crine neoplasia type 2 syndromes (a and b). As the sensitivity of the calcitonin radioimmunoassay has improved, our understanding of the role of calcitonin in normal physiological processes has increased. The knowledge that vitamin D must be metabolized to produce its biologic effects is only 15 years old. This has had profound implications in our understanding of a variety of metabolic bone, kidney, and gastrointestinal diseases. Assays to measure the major cir culating form of vitamin D, 25-hydroxyvitamin D, were described 10 years ago. Assays for the other metabolites, in particular, 1,25-dihydroxyvitamin D, were described even more recently. As of today, we know of many vitamin D metabolites and have developed the techniques to measure most of them; how ever, many questions remain concerning their physiological role.

Methods for quantitative measurement of, e.g. calcium-regulating hormones and vitamin D metabolites as well as of cAMP are absolutely necessary for safe differential diagnosis of diseases of the parathyroid like hypo- and hyperparathyroidism. In this monograph all presently available methods are summarized as to whether they can already be obtained as test kits or are still being tested. Radioimmunoassays for the various peptide fragments such as 53-84, 44-68, 28-48, and 1-34 are described, as are immunochemiluminescence methods and adenylate cyclase bioassays. The same is true for the vitamin D metabolites, where the various assays for calcidiol and calcitriol such as HPLC, protein-binding assay with second antibody separation, and double antibody RIA are deA scribed. Finally, determination of calcitonin and cAMP is discussed in detail. This is a practically and clinically oriented monograph for working in the fields of internal medicine, endocrinology, and laboratory medicine.

A short account of the history of the effort led to the Phosphate Workshops is appro priate and can be of interest to the reader. The idea for Phosphate Workshops was born in the early days of November, 1974. One of us (S. G. M. ) suggested the thought to a group of scientists gathered for a luncheon in one of the attrac tive small restaurants in Weisbaden, Germany. The purpose of the workshop was to bring together interested scientists to discuss the newer developments and the recent advances in the field of phosphate metabolism and the other related minerals. An Organizing Committee made of Shaul G. Massry (USA), Louis V. Avioli (USA), Philippe Bordier (France), Herbert Fleisch (Switzerland), and Eduardo Slatopolsky (USA) was formed. The First Workshop was held in Paris during June 5-6, 1975 and was hosted by Dr. Philippe Bordier. Its proceeding was already published. The Second Workshop took place in Heidelberg during June 28-30, 1976 and was hosted by Dr. Eberhard Ritz. Both of these workshops were extremely successful scientific endeavors, and the need for them was demonstrated by the great interest they generated among the scientific community. The Or ganizing Committee, therefore, decided to continue with the tradi tion to hold additional Workshops annually or every other year.
