Book excerpt: Chronic obstructive pulmonary disease (COPD) remains a highly devastating lung malady and one of the leading causes of death worldwide. As numerous epidemiological and clinical studies have demonstrated, tobacco smoke significantly contributes to the genesis of COPD, encompassing airway, parenchyma, and vascular remodeling to varying degrees of severity. With sufficient advancement of this disease process, destruction of delicate alveolar tissue structures arises, leading to a condition clinically and anatomically known as emphysema. Loss of alveolar structures is accompanied by a reduction in alveolar surface area and extensive capillary loss, inhibiting gas exchange. However, a pressing question about the full extent of vascular remodeling due to the inhalation of tobacco smoke remains. In this study, the spontaneously hypertensive rat (SHR) was used to determine the nature and extent of pulmonary vascular changes that occur from smoking which subsequently may lead to COPD. A primary objective of this study was to determine whether vascular changes involve the pulmonary arterial system, and, if so, to what degree, extent, and severity did vascular changes occur using unbiased, systematic morphometric techniques. A second objective was to determine if such changes persist throughout the pulmonary arterial vascular tree following smoking cessation. Finally, we wished to address whether vascular-based protein expression was changed due to exposure to tobacco smoke and/or following smoking cessation for vascular cell adhesion molecule-1 (VCAM-1), vascular endothelial growth factor (VEGF), and endothelin-1 (ET-1), all important markers of vascular injury and function.
