Book excerpt: Biopsy Pathology of the Bronchi is intended to provide the practicing pathologist with a convenient source of diagnostic information, combined with pertinent clinical patterns and a background of histogenesis. It is hoped that the book will facilitate the accuracy of diagnosis, minimize pitfalls of interpretation and assist in the rapid evaluation of acute and chronic bronchial lesions, including neoplasms. Biopsy is taken in the widest sense and the book includes discussions on cytology, microbiopsies (fine needle aspiration biopsies, curette samples, and biopsies obtained by microforceps) and macrobiopsies, obtained at the time of surgery. Part I of the book deals with the procurement and preparation of bronchial specimens and includes a chapter on fetal development and structure of the normal adult epithelium. In Part II, the histopathology of numerous bronchial diseases are described in detail. Emphasis is placed on the diagnostic features of human lesions' which are illustrated by numerous high quality light and electron micrographs. Understanding normal bron chial structure, cell kinetics, and modulations of phenotypic expression provide insight into many pathological processes. Therefore, although emphasis is placed on the diagnostic features of human disease, the histopathology of corresponding lesions in experimental animals is dis cussed where this provides information helpful in the interpretation of the human lesions. Book excerpt: A review of the publications, dealing with the morphology of the cartilages of the tracheo-bronchial tree, shows how scarce they are and how fragmentary the contributions based on research. Isolated parts only of the bronchial tree have been investigated, mostly in single specimens or small series. Anatomical textbooks merely state that the trachea and main bronchi are supported by rings and the more distal branches by irregular, circumferentially placed plates which become smaller towards the periphery, until they disappear in the bronchioli. It is at once obvious that this old-time view is not only superficial, but it leaves one completely ignorant about the site where rings cease and plates begin. No information is available about the arrangement of cartilages around the bifurcations of the main bronchi and contradictory descriptions are given about the cartilaginous architecture of the lobar bronchi. A more searching study reveals that cartilages are hardly mentioned in the huge amount of literature on bronchial anatomy which has accumulated since 1880. It is therefore not exagerated to say that the study of this subject has been grossly neglected.

These statistical increments may only reflect an increasing awareness of the disease, or its earlier and more sophisticated diagnosis. More important, however, asthma mortality appears to be increasing. This increase has occurred despite the continuing expansion of a diag nostic and management information base, and the developmentofnovel andevermoreeffective therapeutic modalities. Severalexplanations have been offered for this increase in mortality, including that it may result from a statistical artifact [based on a change in the coding criteria for asthma from the International ClassificationofDiseases Version 8(ICD 8) to ICD-9], worsened pollution, delays in seeking medical help, behav ioral changes, deficits in the asthma education of both patients and primary careproviders, toxicity ofbeta-agonists, and noncompliance with instructions for the proper use of medications. It should also be empha sized that the increases in both incidence and mortality may be a reflec tion of accumulating body burdens of environmental toxicants and of increased oxidativedamage. Book excerpt: Since the respiratory airways branch to all parts of the lungs and ready access is provided through the nose or mouth, exploration of these passages for direct visualization or tissue sampling has long been a clinical challenge. This would be particularly helpful in diagnosing those pulmonary diseases that involve the bronchial tree or affect the surrounding lung parenchyma which prove difficult to diagnose or define by indirect methods. The pioneering efforts of Chevalier Jackson in 1918, using bismuth insufflation for radiologic visualization of the bronchial tree, and of Sicard and Forestier, who introduced poppy seed oil (lipiodol) in 1922, rapidly established bronchography as a practical radiologic diagnostic procedure. The initial enthusiasm was soon tempered by recognition of practical problems, and over the years the popularity of bronchography has waxed and waned as techniques were refined and new equipment, instrumentation and contrast agents evaluated. At the same time, alternative methods of diagnosis were being developed, notably sputum cytology, percutaneous needle aspira tion and biopsy, and bronchial brushing. In recent years, a number of medical and technologic developments have revived interest in transbronchial techniques and have made such a diagnostic approach more attractive. Improvements in topical airway anesthesia effectiveness have simplified passage of a variety of catheters, brushes, biopsy devices, fiberoptic or other bronchoscopic instruments along the bronchial passageways. Methods of guiding the catheter or other trans bronchial instrument toward the target site in the lung have also been refined.
