Book excerpt: Excerpt from The British and Foreign Medico-Chirurgical Review, or Quarterly Journal of Practical Medicine and Surgery, Vol. 37: January-April, 1866 Within the last few years the entirely lay character and manage ment of British hospitals has, in some degree, been broken down by the introduction into several of their number of ladies and others trained as nurses, and at the same time members of private associations, banded together by religious ties, amenable to particular rules, and governed by a responsible head. In Dublin, the establishment of the Mater Misericordim hos pital by the Sisters of Charity, is an instance of a revival of the ancient connexion between the convent and the house for the sick poor. The prevailing tendency of the present time in England seems to be to re-invoke the religious element in the nursing of the sick, both in public and in private the conviction obtaining, and being supported also by experience (now that pre judices are becoming weaker), that nursing is more efficient and humane when carried out under religious sanction by individuals devoted to it as a work of love and mercy. In the above remarks credit has been taken for the influence of that primary principle of Christianity - charity, in the insti tution of receptacles for the nurture and tending of the sick poor; and, so far as that principle has operated, Christianity may be fairly deemed instrumental in the establishment of hos pitals. Yet there is no question that, prior to the dawn of Christianity, institutions for the benefit of the sick were to be found in every country that had attained the higher degrees of civilization then existing. Wherever the religion of Greece and Rome extended temples arose dedicated to Esculapius, and health-giving fountains were discovered and consecrated to Hygeia, to which sick folk might resort, and where they might receive aid and advice from the attendant priests and ministers. Certain Observances and ceremonies were to be gone through, which, though identified with religion, were at the same time calculated to benefit the health, being derived undoubtedly in a great measure from'the teachings of experience. Indeed, much knowledge of disease must have been acquired by the temple attendants, both from their own observation of the applicants for relief and from the records of former cases, which were, as we know, preserved; and, besides this, their interests were more or less identified with the reputation and success of the sacred place they served. It is thoughtfully compiled, splendidly printed and choicely bound. It contains four hundred and eighty-four pages and three times that number of prescriptions. It is divided in such manner that all the diseases of the community are classified, while the prescriptions for the cure of each class of disease are placed under their proper heads. The prescriptions are from the pens of none but the elite of physic; if the authors of them were dead or had retired from practice when the book was written, their names arc added; if they were living, their names are omitted, "to avoid giving offence." The favorite recipes of princely Richard Mead, Sir Janies Millard, Huxham, Sir Clifton Winter-iugham, Sir E. Halse, Sir E. Barry, and of many others, are here in correct form. We think at once, as we look at this book, how easy it must have been to prescribe in 1777. We have a disease to treat; we know the name of the disease, and before us there is the remedy for the disease fresh from the most learned of hands. When we turn to our book and ask what the medical elite of the metropolis prescribed for the benefit of mankind, and for the imitation of their less fortunate brethren of the provinces, we note singularly and specially one great fact, that their formulas arc all based on the most absolute of empirical principles. We notice further that the medicines prescribed arc in nine cases out of ten simples of simples. There are, it is true, a few active medicines, to wit, opium, antimony, ammonia, elateriurn, mercury, lead, sulphuric acid, jalap, rhubarb, lemon juice, and one or two other agents more or less active, but none of those really useful medicines, as we now consider them, take prominent place; prominence is rather given to carminatives and comparatively inert medicines - cardiac confection, red coral, chalk julep, stomachic tincture, musk, dragon's blood, Castile soap, oil of juniper, wormwood, James's powder, saffron, balsam of Tolu, syrup of marsh-mallow, cascarilla, ginger, gum ammoniaeum, mace, camphor, and aqua pura; these clearly were the stock-in-trade, par excellence, of the apothecary to the Princess Amelia, and friend of the leading physicians of London, one hundred years ago. For the different classes of diseases the prescriptions are strangely varied, no principle of action being anywhere indicated. For consumptions, spermaceti, made up with yelk of egg, with nitre, volatile aromatic spirit, and cinnamon water, forms one mixture intended for cough; viper broth, to be taken about midday, in four ounce doses, is another remedy; antimonial wine, with orange, cardamoms and white wine, is a third prescription, from the authority of Dr. James; and decoction of Peruvian bark with balsam of Tolu and vitriolic elixir forms a fourth combination. We go entirely with a modern authority, who has demonstrated by ingenious methods how absolute and essential is the intrusion of caloric into physiological processes; and as far as great principles are concerned, it is best for all of us to keep fully in view the transformations whereby chemical force, in connexion with production of heat, comes to be finally expended in those objective and subjective manifestations which make up the sum of man's activity. This argument serves to enforce the physiological truth, that it is impossible to get more out of man in the way of work than you allow him in the form of food. Caloric is nu doubtedly a prime agent in all the recondite phenomena within the frame. As regards the use of hot drinks otherwise than exceptionally, unless it be in the morning, when the function of the stomach is naturally feeble, we incline, however, to think that popular experience is in favour of those slower processes of conversion of force, whereby alcoholic and other food furnish fuel to the frame. In rickets and scrofula, produced by our ever-changing clime, wine keeps its place well in the esteem of physicians, and even gains upon the (past. Cold beer is preferred to hot by most, it will be found, of those that use it during exposure to weather. But in all these questions we refer to the test of experience; and it is fair to state that Dr. Parkes argues this one on no theoretical grounds. The refusal of the Duke of Wel lingtou to allow of any teetotal combination in the army showed, we think, a caution and a wisdom beyond our time. This volume, from its intrinsic importance as well as the distinguished position and reputation of its author, demands special examination and candid criticism. Our first impression is one of disappointment. We had hoped for a book which, while it faithfully reproduced the accepted results of previous workers, should yet bear the stamp of originality; which should be written in a style of clear composition unburdened by needless technicalities, and which, lastly, should be of use, not merely to members of Professor Rolleston's class, but to all persons who can understand the English language. Now what do we find? Professor Rolleston's book can scarcely, even by the most partial Oxford friend, be termed original: its best points are borrowed from Gegenbaur's 'Grundziige der Vegleich ende Anatomiethe style bristles with the most recondite terms of Germanic origin; while each sentence, probably from a laudable desire to save space, has been compressed into so narrow a compass that sense is too often sacrificed to brevity. Finally, as the illustrative specimens exist in the Oxford Museum only, we fear that few but Oxford men can derive much benefit from a perusal of the book. We now proceed to consider it in detail. That the invention of the sphygmo graph has already led to a much more exact knowledge of the circulation, none will dispute; and, indeed, there can belittle doubt that it is also destined to render great services in the hands of the physician. The few industrious investigators who have applied it to purposes of clinical research in this country have shown that it is capable of extended usefulness and in the following pages we propose to consider the results which their publications placed at the head of this article contain. In so doing we shall first present our readers with a brief exposition of the recent advances of our knowledge of the healthy pulse curve, and then, after alluding to the suggested modifications in the form and application of the instrument, we shall proceed to discuss its importance as an aid in the study of disease. To this post of honour it has won its way, much more by its own in. Trinaie merits, than by the exertions of the Associations, some of whose literary productions will be found at the head of this article. In making this remark, we desire rather to do honour to the cause itself, than to detract from the merits of the earnest and philanthropic individuals who have given themselves to this labour of love; and the fact that the majority of the working members of the associations belong to our own pro tsasion, makes us only, the more willin to do justice to their patriotic efforts. The cause which. They have on in hand forms one of the few exceptions to that wholesome practice which the members of the profession have, for the most part, very strictly observed, of abstaining from all poli tical agitation. Indeed it may admit of doubt whether the movement in favour of sanitary reform can be justly placed in the category of political agitations. Such an association would do rade it from the dignity which it justly claims of a world-wide question, gown to the low leve of a party conflict of which England isthe narrow theatre. This is not the light in which its friends regard it; and the interest which it has excited on the Continent proves that they are not peculiar in the view they take of it.
