Book excerpt: 'Breast is best' is today’s prevailing mantra. However, women – particularly first-time mothers – frequently feel unsupported when they come to feed their baby. This new experience often takes place in the impersonal and medicalized surroundings of a hospital maternity ward where women are 'seen to' by overworked midwives. Using a UK-based ethnographic study and interview material, this book provides a new, radical and critical perspective on the ways in which women experience breastfeeding in hospitals. It highlights that, in spite of heavy promotion of breastfeeding, there is often a lack of support for women who begin to breastfeed in hospitals, thus challenging the current system of postnatal care within a culture in which neither service-user nor provider feel satisfied. Incorporating recommendations for policy and practice on infant feeding, Breastfeeding in Hospital is highly relevant to health professionals and breastfeeding supporters as well as to students in health and social care, medical anthropology and medical sociology, as it explores practice issues while contextualising them within a broad social, political and economic context.

Babies who are breastfed are less likely to become overweight and obese. Many mothers in the United States want to breastfeed, and most try. And yet within only three months after giving birth, more than two-thirds of breastfeeding mothers have already begun using formula. By six months postpartum, more than half of mothers have given up on breastfeeding, and mothers who breastfeed one-yearolds or toddlers are a rarity in our society. October 2010 marked the 10th anniversary of the release of the HHS Blueprint for Action on Breastfeeding, in which former Surgeon General David Satcher, M.D., Ph. D., reiterated the commitment of previous Surgeons General to support breastfeeding as a public health goal. This was the first comprehensive framework for national action on breastfeeding. It was created through collaboration among representatives from medical, business, women's health, and advocacy groups as well as academic communities. The Blueprint provided specific action steps for the health care system, researchers, employers, and communities to better protect, promote, and support breastfeeding. I have issued this Call to Action because the time has come to set forth the important roles and responsibilities of clinicians, employers, communities, researchers, and government leaders and to urge us all to take on a commitment to enable mothers to meet their personal goals for breastfeeding. Mothers are acutely aware of and devoted to their responsibilities when it comes to feeding their children, but the responsibilities of others must be identified so that all mothers can obtain the information, help, and support they deserve when they breastfeed their infants. Identifying the support systems that are needed to help mothers meet their personal breastfeeding goals will allow them to stop feeling guilty and alone when problems with breastfeeding arise. All too often, mothers who wish to breastfeed encounter daunting challenges in moving through the health care system. Furthermore, there is often an incompatibility between employment and breastfeeding, but with help this is not impossible to overcome. Even so, because the barriers can seem insurmountable at times, many mothers stop breastfeeding. In addition, families are often unable to find the support they need in their communities to make breastfeeding work for them. From a societal perspective, many research questions related to breastfeeding remain unanswered, and for too long, breastfeeding has received insufficient national attention as a public health issue. This Call to Action describes in detail how different people and organizations can contribute to the health of mothers and their children. Rarely are we given the chance to make such a profound and lasting difference in the lives of so many. I am confident that this Call to Action will spark countless imaginative, effective, and mutually supportive endeavors that improve support for breastfeeding mothers and children in our nation."--Page v.

Book excerpt: Exclusive breastfeeding (EBF) for the baby's first six months after birth, and continue breastfeed (BF) up to two years and beyond, has been an ultimate goal of World Health Organization (WHO), Department of Health and Human Services (DHHS), American Academy of Pediatrics (AAP), and American College of Obstetricians and Gynecologists (ACOG) (Willumsen, 2013). BF has been recognized as having numerous health benefits for mothers and babies as well as cost-saving to families, and environmental friendly to the world. The aim of this study is to examine the effects of provision of BF education and EBF policies to health care professionals and mothers; intensive BF support to mothers, and the increase of staffing ratios on the improvement of EBF rate in the in-patient hospital setting. The significant drop of EBF rate (from 80-90% down to 60-70%) in the past few months at the author's baby-friendly hospital (BFH) drew the departmental managers' intention to initiate a pilot study to improve and maintain the EBF rate to required level (75%). Based on the research studies, low EBF rates could be increased by using intensified BF education and support from lactation consultants and nursing staff; strategies such as appropriate staffing ratios, collaboration of family members and support groups. Mothers who have health problems and unexpected birth experiences should receive extra BF support from health professionals and family members. To obtain the informal approval from key stakeholders is critical for the development of the implementation plan. Meetings are needed to address the issues; the evidence to support the proposed solutions and the urgency to change. After approval, a BF committee for the project will be formed and meeting will be carried out at a regular basis. The study takes a four-week period emphasizing on Breastfeeding Bundle (BFB) (Include: skin-to-skin care after delivery, initiate BF within the first hour, rooming-in, and no pacifier or supplement other than breast milk except medically indicated). Data collection for the evaluation process is chart review. The standard data measuring table will be used to assess the rate. BF committee members will review charts of discharged mothers and babies. Percentage of practicing skin-to-skin care, initiation of BF, BF education, and satisfaction will be calculated. Using the data collected at discharge from postpartum unit to identify the perinatal BF rates and the type of feeding. Previous studies concluded low EBF rates could be increased by intensified BF education and support; appropriate staffing ratios, and collaboration of family members and support groups. To disseminate the results, key committee members need to schedule meetings with the stakeholders to report the project outcome, to receive their input, and to ensure this EBP can be continued to sustain in the long run after implementation. Strategies for effective dissemination of results are presenting by PowerPoint, sending e-mails, posters, or flyers, et cetera.

Since 1992, the Chinese government has led the world in reorganizing more than 7,000 hospitals into “Baby- Friendly” hospitals. The initiative’s goal, overseen by UNICEF and the World Health Organization, is to promote the practice of breastfeeding by reorganizing hospital routines, spaces, and knowledge in maternity wards and obstetrics clinics. At the same time, China’s hospitals in the mid- 1990s operated as sites where the effects of economic reform and capitalism increasingly blurred the boundaries between state imperatives to produce healthy future citizens and the flexibility accorded individuals through their participation in an emerging consumer culture. Formulas for Motherhood follows a group of women over eighteen months as they visited a Beijing Baby- Friendly Hospital over the course of their pregnancies and throughout their postpartum recoveries. The book shows how the space of the hospital operates as a microcosm of the larger social, political, and economic forces that urban Chinese women navigate in the process of becoming a mother. Relations between biomedical practices, heightened expectations of femininity and sexuality demanded by a consumer culture, alongside international and national agendas to promote maternal and child health, reveal new agents of maternal governance emerging at the very moment China’s economy heats up. This ethnography provides insight into how women’s creative pragmatism in a rapidly changing society leads to their views and decisions about motherhood.

Our conclusion is Baby Friendly Hospital initiative has a beneficial effect on feeding practices of mothers and duration of exclusive breastfeeding. But some areas of optimal breastfeeding practices are not taken care well especially initiation of breastfeeding within half to one hour of birth needs attention.
