This dynamic and bioactive fluid allows mother–infant signalling over lactation, guiding the infant in the developmental and physiological processes. It exerts protection and life-long biological effects, playing a crucial role in promoting healthy growth and optimal cognitive development. The latest scientific advances have provided insight into different components of human milk and their dynamic changes over time. However, the complexity of human milk composition and the synergistic mechanisms responsible for its beneficial health effects have not yet been unravelled. Filling this knowledge gap will shed light on the biology of the developing infant and will contribute to the optimization of infant feeding, particularly that of the most vulnerable infants. As we turned to the original literature several reasons for the dearth of usable material became clear: (1) In the areas of mammary gland biology and physiology, particularly as they relate to the human, reviews simply have not kept up with current research, which has in the last two decades provided tremendous insight into the mechanisms of milk secretion and its control. (2) The lack of interest in human milk as infant food inhibited researchers until very recently from investigating human lactation. (3) Much of the relevant clinical information remains anecdotal with little scientific basis. In this book we have tried to present the fundamentals of mammary gland physiology at the organismic and cellular levels in a form readily understood by physicians, scientists, and other professionals. This basic information is accompanied by authoritative reviews of the nutritional and immunological properties of human milk and by clinically relevant chapters designed to help health care professionals deal with the medical problems of the breast-feeding mother and her infant. We have strived in these chapters for up-to-date, authoritative, but readable accounts. In so far as possible we have avoided areas where much of our understanding rests on speculation. In the clinical domain this was not always possible because of a lack of solid, scientific information about breast-feeding.

Book excerpt: The rate of breastfeeding in the United States has risen and fallen over time as a result of changing lifestyles and the availability of substitutes for human milk. However, since the 1970s the rates of breastfeeding initiation and duration have increased, in part due to the increasing medical evidence of the significant health benefits both to mothers and children. The federal government's Health Objectives for 2010 are for 75% of mothers to initiate breastfeeding at the time of birth and for 50% to continue breastfeeding until their infants reach 6 months of age. Current rates are about 69.5% at the time of hospital discharge and 32.5% at 6 months of age. The health benefits to the infant include the high nutritional quality of human milk, and a decrease in various infectious and other diseases of infancy that are reduced by the anti-microbial, anti-inflammatory and immunological-stimulating agents present. Mothers seem to benefit with a more rapid return to the pre-pregnancy state of their bodies, improved glucose and lipid metabolism, delayed ovulation, and the possible reduction of certain cancers. This new book examines the impact on health, employment and society, laws and government support of this important health action.
