Since the initiation of this infrastructure award, we have continued to expand on our previous success in the UCLA-Community Oncology Research Network (UCLA-CORN) through: 1) the recruitment of additional research sites, increasing access for women from underserved populations, 2) the initiation of translational research protocols relevant to the breast cancer problem and 3) an overall increase in accrual to breast cancer research protocols in the UCLA-CORN.

Book excerpt: An ideal health care system relies on efficiently generating timely, accurate evidence to deliver on its promise of diminishing the divide between clinical practice and research. There are growing indications, however, that the current health care system and the clinical research that guides medical decisions in the United States falls far short of this vision. The process of generating medical evidence through clinical trials in the United States is expensive and lengthy, includes a number of regulatory hurdles, and is based on a limited infrastructure. The link between clinical research and medical progress is also frequently misunderstood or unsupported by both patients and providers. The focus of clinical research changes as diseases emerge and new treatments create cures for old conditions. As diseases evolve, the ultimate goal remains to speed new and improved medical treatments to patients throughout the world. To keep pace with rapidly changing health care demands, clinical research resources need to be organized and on hand to address the numerous health care questions that continually emerge. Improving the overall capacity of the clinical research enterprise will depend on ensuring that there is an adequate infrastructure in place to support the investigators who conduct research, the patients with real diseases who volunteer to participate in experimental research, and the institutions that organize and carry out the trials. To address these issues and better understand the current state of clinical research in the United States, the Institute of Medicine's (IOM) Forum on Drug Discovery, Development, and Translation held a 2-day workshop entitled Transforming Clinical Research in the United States. The workshop, summarized in this volume, laid the foundation for a broader initiative of the Forum addressing different aspects of clinical research. Future Forum plans include further examining regulatory, administrative, and structural barriers to the effective conduct of clinical research; developing a vision for a stable, continuously funded clinical research infrastructure in the United States; and considering strategies and collaborative activities to facilitate more robust public engagement in the clinical research enterprise.

Book excerpt: Despite recent advances in adjuvant therapies of cancer, the regi mens of postoperative adjuvant chemotherapy treatment which are presently available fail to cure the majority of cancer patients. Pre operative (neoadjuvant) chemotherapy represents a new approach in drug scheduling, based on sound theoretical, pharmacokinetic, and experimental principles. The preoperative timing of chemotherapy before definitive sur gery is not a minor change in the therapy of cancer. To be successful, large numbers of practitioners and their patients must participate. Substantial alterations of many aspects of the present management of cancer will have to follow. Therefore, before such therapy can be fully and routinely implemented, results of the novel treatment and its rationale have to be carefully evaluated. In preoperative treatment, other features will likely gain impor tance. For the first time, clinicians have a chance to follow the in vivo response of the tumor exposed to preoperative chemotherapy. The subsequent histological assessment of the tumor sample may likely become an important prognostic guide, permitting more re fined individual approaches to the planning of postoperative adju vant treatment. The value of such a treatment strategy can already be appreciated in the clinical setting, as seen from the therapy of osteosarcoma. Furthermore, preoperative chemotherapy might render previously inoperable tumors operable and hence resectable with a curative intention. The preoperative reduction of tumor bulk may also effectively decrease the need for more radical operations, permitting a more uniform adoption of conservative surgery.

This first volume, Metastasis Research Protocols: Analysis of Cells and Tissues, concentrates on analysis and mapping of molecules produced by cells and tissues and analysis of the molecular biology underlying their expression, whereas the second volume, Metastasis Research Protocols: Cell Behavior In Vitro and In Vivo,focuses sharply on the determination of cell behavior in vitro and in vivo. We have deliberately included chapters describing well-established and familiar te- niques (for example, SDS-PAGE and Western blotting [Chapter 11], and immunocytochemistry [Chapter 2]) in addition to the newer and more speci- ized approaches and specific examples of their application, because—although the methodology is readily available in the published literature and established in many laboratories—we wished these volumes to “stand alone” and to make accessible here the standard techniques that underpin much metastasis research for both the newcomer to the field and the seasoned researcher. Undoubtedly, owing to the complexity of the metastatic cascade and the wealth of research techniques involved in scientific approaches to its unraveling, and despite our best efforts to make these volumes as comprehensive as seems feasible, this is a tall order, and there will inevitably be omissions. The five-year survival rate for breast cancer patients has improved in recent years, but the overall mortality rates have changed little. In 1993 Congress allocated $210 million for breast cancer research as part of the Department of Defense budget. An Institute of Medicine (IOM) committee was convened at that time to advise the U.S. Army Medical Research and Development Command on strategies for managing a breast cancer research program. This book evaluates the program's management and achievements to date. Although it is too early to evaluate the program in terms of breakthrough results and new insights produced by the funded projects or investigators, this book documents the process used to select research proposals for funding and analyzes the portfolio of funded projects in terms of their responsiveness to the recommendations and fundamental questions articulated in the 1993 IOM report.
