Book excerpt: In breast cancer as in other cancers on the front line of modern interdisciplinary research we have crossed the threshhold of new understanding. Fueled by an awareness that breast cancer is a leading cause of morbidity and mortality in women and honed by rapid technological advances in molecular mechanisms, major inroads into the darkness of this scourge have been accomplished during the past several years. Basic laboratory and clinical research findings must influence and in turn be influenced by efforts to detect, to diagnose and to cure the human disease. It is indeed gratifying to note that the efforts of scientists and clinicians from throughout the world reported in this volume are achieving this objective. The first section of the book focuses on the molecular and genetic basis of breast cancer. The role of specific oncogenes in mammary tumorigenesis using transgenic mice and mammary glands correlating molecular events with specific stages in neoplastic development are described and discussed. Such topics as the nature of specific oncogenes, levels of oncogene expression, the alteration of expression of other growth regulatory genes and the state of the cell in which the oncogene is expressed are specifically addressed. This section of the book is rounded out with discussions on the potential of genetic alterations as indicators of prognosis, the characterization of full-length cDNA codes for breast cancer markers and the function of antigens in tuomorigenesis.

It integrates an extensive amount of material from diverse sources. The analysis finds that interactions among many features woven into the fabric of our individual, family, and community lives create conditions that make breast cancer more or less likely. Thinking about this as a design problem helps us identify multi-level interventions that will reduce risk and improve outcomes after diagnosis.Preventing breast cancer and reducing recurrences requires measures that confront the systemic roots of the disease. Generally-accepted individual risk factors are important, but they simply do not explain why many people develop the disease. The food system, many aspects of the built and occupational environments, and pervasive hazardous environmental chemicals also contribute, and they cannot be addressed by individuals alone. The book briefly discusses known risk factors for breast cancer, including family and personal history, genetic susceptibility, early puberty, late menopause, late age of first child or having no children, dense breast tissue, chest irradiation, current use of oral contraceptives, combination hormone therapy, cigarette smoking, and alcohol consumption. It goes on to explore what we know about other variables, their interactions, and the importance of taking a life course approach, since breast biology and later risk of breast cancer can be influenced by conditions experienced during fetal development, infancy, childhood, and adolescence as well as adulthood. The first section of the book proposes that a systems perspective or an ecological framework is best suited for studying the origins of breast cancer and designing interventions intended to prevent it and improve outcomes following treatment. A second section reviews numerous studies addressing the roles of nutrition, physical activity, environmental chemicals, vitamin D, electromagnetic fields, shift work, and stress. Examples of interactions among these variables show the value of a systems-based approach to research and interventions. Section three synthesizes this information and identifies practical opportunities for individuals, health care professionals, public health officials, community planners, businesses, schools, governments, and farmers to help reduce the burden of this disease. Biomedical scientists and clinicians have made enormous advances in treating breast cancer in recent decades. Improved outcomes are likely due to combinations of earlier diagnosis, better understanding of cellular pathology, and refined, targeted therapeutic interventions. For many people with the disease, adding weight control, dietary interventions, exercise, stress reduction, and social support to their initial therapy has not only improved the quality of their lives but also reduced the risk of recurrence. A public health perspective widens the lens to look at breast cancer patterns in populations and offers additional insights into prevention and treatment. An ecological framework accommodates both individual and public health points-of-view and adds new science. It expands the ways we can study and address this disease. Finally, applying the ecological framework more widely to other complex disorders, including other kinds of cancer, diabetes, asthma, cardiovascular disease, learning disabilities, cognitive decline, and dementia, is likely to improve our understanding of their origins and point to better strategies for prevention and treatment. Changes in diet and the food system, the built environment, social environment, and reductions in hazardous chemical exposures designed to address breast cancer will help reduce the burden of many of these as well.

At the request of SSA, Diagnosing and Treating Adult Cancers and Associated Impairments provides background information on breast cancer, lung cancer, and selected other cancers to assist SSA in its review of the listing of impairments for disability assessments. This report addresses several specific topics, including determining the latest standards of care as well as new technologies for understanding disease processes, treatment modalities, and the effect of cancer on a person's health and functioning, in order to inform SSA's evaluation of disability claims for adults with cancer.

It has been proven to detect breast cancer at an earlier stage and to reduce the number of women dying from the disease. However, it has a number of limitations. These current limitations in early breast cancer detection technology are driving a surge of new technological developments, from modifications of x-ray mammography such as computer programs that can indicate suspicious areas, to newer methods of detection such as magnetic resonance imaging (MRI) or biochemical tests on breast fluids. To explore the merits and drawbacks of these new breast cancer detection techniques, the Institute of Medicine of the National Academy of Sciences convened a committee of experts. During its year of operation, the committee examined the peer-reviewed literature, consulted with other experts in the field, and held two public workshops. In addition to identifying promising new technologies for early detection, the committee explored potential barriers that might prevent the development of new detection methods and their common usage. Such barriers could include lack of funding from agencies that support research and lack of investment in the commercial sector; complicated, inconsistent, or unpredictable federal regulations; inadequate insurance reimbursement; and limited access to or unacceptability of breast cancer detection technology for women and their doctors. Based on the findings of their study, the committee prepared a report entitled Mammography and Beyond: Developing Technology for Early Detection of Breast Cancer, which was published in the spring of 2001. This is a non-technical summary of that report.

There are growing indications, however, that the current health care system and the clinical research that guides medical decisions in the United States falls far short of this vision. The process of generating medical evidence through clinical trials in the United States is expensive and lengthy, includes a number of regulatory hurdles, and is based on a limited infrastructure. The link between clinical research and medical progress is also frequently misunderstood or unsupported by both patients and providers. The focus of clinical research changes as diseases emerge and new treatments create cures for old conditions. As diseases evolve, the ultimate goal remains to speed new and improved medical treatments to patients throughout the world. To keep pace with rapidly changing health care demands, clinical research resources need to be organized and on hand to address the numerous health care questions that continually emerge. Improving the overall capacity of the clinical research enterprise will depend on ensuring that there is an adequate infrastructure in place to support the investigators who conduct research, the patients with real diseases who volunteer to participate in experimental research, and the institutions that organize and carry out the trials. To address these issues and better understand the current state of clinical research in the United States, the Institute of Medicine's (IOM) Forum on Drug Discovery, Development, and Translation held a 2-day workshop entitled Transforming Clinical Research in the United States. The workshop, summarized in this volume, laid the foundation for a broader initiative of the Forum addressing different aspects of clinical research. Future Forum plans include further examining regulatory, administrative, and structural barriers to the effective conduct of clinical research; developing a vision for a stable, continuously funded clinical research infrastructure in the United States; and considering strategies and collaborative activities to facilitate more robust public engagement in the clinical research enterprise.
