It will also discuss their therapeutic mechanisms that make them useful in dentistry and dental treatments. Over the past few decades, stem cell research has gained extensive scientific inquiry. This aspect is attributable to the significance of stem cells in tissue engineering. It is apparent that tissue regeneration has emerged as a reliable medical approach for the treatment of tissue disorders and injuries. Initially, embryonic stem cells were preferred as candidates for regenerative medicine because these cells can be induced to replicate in a pluripotent state. However, stem cell research involving embryonic stem cells has attracted immense controversy. It is also associated with legal and ethical issues, thus limiting the use of embryonic stem cells in regenerative medicine. Fortunately, the discovery of mesenchymal stem cells (MSCs), also referred to as adult stem cells, has restored promise for the development of stem cell therapies. Unlike embryonic stem cells, MSCs are free from legal and ethical concerns. MSCs are usually pluripotent progenitor cells that are generated in an array of tissues in both foetal and adult life. It is reported that these progenitor cells differentiate into cell types of the tissues that generate them, although studies indicate that they can differentiate cell types of other tissues. Currently, MSCs are used for regenerative therapies for a number of tissue disorders and injuries including bone regeneration. For instance, MSCs generated by the dental pulps and the oral cavity tissues have been found to possess the potential for dental tissue regeneration. These cells have also been found to useful in non-dental tissue repair.

Book excerpt: Sequential and reciprocal interactions between oral epithelial and cranial neural crest-derived mesenchymal cells give rise to the teeth and periodontium. Teeth are vital organs containing a rich number of blood vessels and nerve fibers within the dental pulp and periodontium. Teeth are composed by unique and specific collagenous (dentin, fibrillar cementum) and non-collagenous (enamel) highly mineralized extracellular matrices. Alveolar bone is another collagenous hard tissue that supports tooth stability and function through its close interaction with the periodontal ligament. Dental hard tissues are often damaged after infection or traumatic injuries that lead to the partial or complete destruction of the functional dental and supportive tissues. Well-established protocols are routinely used in dental clinics for the restoration or replacement of the damaged tooth and alveolar bone areas. Recent progress in the fields of cell biology, tissue engineering, and nanotechnology offers promising opportunities to repair damaged or missing dental tissues. Indeed, pulp and periodontal tissue regeneration is progressing rapidly with the application of stem cells, biodegradable scaffolds, and growth factors. Furthermore, methods that enable partial dental hard tissue repair and regeneration are being evaluated with variable degrees of success. However, these cell-based therapies are still incipient and many issues need to be addressed before any clinical application. The understanding of tooth and periodontal tissues formation would be beneficial for improving regenerative attempts in dental clinics. In the present e-book we have covered the various aspects dealing with dental and periodontal tissues physiology and regeneration in 6 chapters: 1. From the prospective of tissue engineering, it is of utmost importance to understand and emulate the complex cell interactions that make up a tissue or organ. Unlike other tissues in the body, dental tissues are unique in their development, function, and even in their maintenance throughout life. The harmonized stimulations of biology and mechanical regulators to promote cellular activities have matured our understanding of the value of regenerative therapy of dental tissue versus the reparative treatment. In this book, we review the current knowledge available to regenerate alveolar bone, periodontal structure, and pulp/dentin complex. The book provides researchers with detailed information about development and functional characteristics of the dental unit with detailed protocols covering a comprehensive range of various approaches to engineer dental tissues: to use isolated cells or cell substitutes as cellular replacement, to use acellular biomaterials capable of inducing tissue regeneration, and/or to use a combination of cells, biomaterial and growth factors. We are well aware, with the concept changes in the field toward in-vitro biomimetics of in-vivo tissue development. This interest stems from the capacity of these cells to differentiate into multiple lineages and from their secretion of growth factors that can activate progenitor cells, which make MSC potentially applicable for tissue regenerative purposes. When exposed to inflammatory conditions, MSC furthermore exhibit immunosuppressive properties. The reduction of chronic or acute inflammatory responses by MSC may halt the development of injury and allow regenerative processes to take place. This prospective has initiated attempts to use MSC as an immunosuppressive and regenerative agent in transplantation and regeneration of tissues like kidney, liver, heart, bone and multiple others. Nevertheless, the conditions under which MSC therapy is effective and via what mechanisms MSC operate are, in particular in in vivo settings, not clear. The route of administration and the reigning immunological conditions are likely to be key for the efficacy of MSC. Studies have demonstrated for instance that MSC may be immunostimulatory under immunological quiescent conditions. Whereas some reports indicate that homing of MSC to inflamed tissues and their secretion of growth and anti-inflammatory factors are responsible for their effect, others show that MSC rapidly disappear after administration and may not have time to actively contribute to immunosuppression and tissue repair. Many aspects of MSC have to be clarified in order to develop efficient therapies. In this special topic, different views on the mechanisms of immunomodulation and regeneration by MSC and on the possible applications of MSC in transplantation and tissue regeneration will be put together.
