GAO found that federal agencies are participating in research and preparedness activities, from improving the detection of biological agents to developing a national stockpile of pharmaceuticals to treat victims of disasters. Federal agencies also have several efforts underway to coordinate these activities on a formal and informal basis, such as interagency work groups. Despite these efforts however, coordination between agencies remains fragmented. GAO also found emerging concerns about the preparedness of state and local jurisdictions, including insufficient state and local planning for response to terrorist events, inadequate public health infrastructure, a lack of hospital participation in training on terrorism and emergency response planning, insufficient capabilities for treating mass casualties, and the timely availability of medical teams and resources in an emergency. Book excerpt: A number of incidents involving biological agents, including at least one completed bioterrorist act and numerous threats and hoaxes, have occurred domestically. In 1984, a group intentionally contaminated salad bars in local restaurants in Oregon with salmonella bacteria to prevent people from voting in a local election. Although no one died, 751 people were diagnosed with foodborne illness. As was the case with this incident, determining whether an outbreak occurred naturally or was the product of an intentional release may be difficult unless someone announces the release beforehand or claims responsibility for it. Given the terrorist attacks of September 11, 2001, in addition to the terrorist bombings in New York City in 1993 and in Oklahoma City in 1995, some experts predict that domestic bioterrorist attacks are likely to occur. Others consider the likelihood of bioterrorist attacks to be low because of various difficulties, including those involved in processing the biological agents into lethal forms and successfully delivering them to achieve large-scale casualties. However, the ease with which these agents can be concealed and their potential to affect large segments of the population beyond those initially exposed may increase their appeal to terrorists.

Unlike an explosion or chemical attack, which results in immediate and visible casualties, the public health impact of a biological attack can unfold gradually over time. Until a sufficient number of people arrive at the emergency rooms and doctors' offices complaining of similar illnesses, there may be no sign that an attack has taken place. The speed and accuracy with which the doctors and laboratories reach the correct diagnoses and report their findings to public health authorities has a direct impact on the number of people who become ill, and the number that die. The nation's ability to respond to a bioterrorist attack, therefore, depends crucially on the state of preparedness of its medical care systems and public health infrastructure. Public health experts have for years complained about the deterioration of the public health system through neglect and lack of funding. They warn that the nation is ill equipped and insufficiently prepared to respond to a bioterrorist attack. For example, they point out that there are too few medical personnel trained to spot biological attacks, a shortage of sophisticated laboratories to identify the agents, and inadequate supplies of drugs and vaccines to counteract the threat. They also contend that inadequate plans exist for setting up quarantines and emergency facilities to handle the sick and infectious victims. Improving public health preparedness and response capacity offers protection not only from bioterrorist attacks, but also from naturally occurring public health emergencies. Public health officials are increasingly concerned about our exposure and susceptibility to infectious disease and food-borne illness because of global travel, ubiquitous food imports, and the evolution of antibiotic-resistant pathogens. This book reviews critical issues in the US public system and its vulnerability or lack thereof to bioterrorism.

Preparing to respond to the public health and medical consequences of a bioterrorist attack poses some challenges that are different from those in other types of terrorist attacks, such as bombings. On September 28, 2001, we released a report that describes (1) the research and preparedness activities being undertaken by federal departments and agencies to manage the consequences of a bioterrorist attack, 3 (2) the coordination of these activities, and (3) the findings of reports on the preparedness of state and local jurisdictions to respond to a bioterrorist attack. My testimony will summarize the detailed findings included in our report, highlighting weaknesses in the public health infrastructure that we have identified in our ongoing work and which we believe warrant special attention.

In a three-day workshop, convened by the Institute of Medicine's Forum on Emerging Infections, experts from each of these communities came together to identify, clarify, and prioritize the next steps that need to be taken in order to prepare and strengthen bioterrorism response capabilities. From the discussions, it became clear that of utmost urgency is the need to cast the issue of a response in an appropriate framework in order to attract the attention of Congress and the public in order to garner sufficient and sustainable support for such initiatives. No matter how the issue is cast, numerous workshop participants agreed that there are many gaps in the public health infrastructure and countermeasure capabilities that must be prioritized and addressed in order to assure a rapid and effective response to another bioterrorist attack.
