Chenodeoxycholic acid and ursodeoxycholic acid have proved to be effective agents for the dissolution of cholesterol gallstones, and ursodeoxycholic acid has been shown to have beneficial effects in patients with chronic cholestatic liver disease such as primary biliary cirrhosis or primary sclerosing cholangitis. The question of the long-term benefit for the patient remains open and the mechanism of action is still unclear. As a consequence, much basic and clinical research is being conducted on bile acid therapy. The XIIth International Bile Acid Meeting brought together researchers and clinicians from around the world, and was dedicated to basic and clinical aspects of the use of bile acids in the therapy of hepatobiliary disease.

Book excerpt: The liver is a vital organ involved in numerous metabolic processes such as cholesterol and bile acid metabolism, biliary lipid secretion, and bile formation. Cholesterol balance across the liver has a crucial effect on influencing plasma total and LDL cholesterol levels and biliary cholesterol concentrations. Cholesterol and bile acid biosyntheses are primarily modulated by negative feedback regulatory mechanisms through the sterol regulatory element-binding protein isoform 2 (SREBP-2) and the farnesoid X receptor (FXR) pathways, respectively. The conversion of cholesterol to bile acids in the liver can balance the fecal excretion of bile acids, which is an important route for the removal of cholesterol from the body. Bile formation begins in the bile canaliculi, and maintenance of the enterohepatic circulation of bile acids results in a continuous secretion of bile. Hepatic secretion of biliary lipids is determined mainly by a group of ATP-binding cassette (ABC) transporters that are located on the canalicular membrane of hepatocytes, which are regulated by various nuclear receptors. Bile acids promote bile flow by their osmotic effects. Also, they are essential for the intestinal absorption of cholesterol, fatty acids, and fat-soluble vitamins and play an important role in aiding the digestion of dietary fat. Bile acids function as signaling molecules and anti-inflammatory agents to regulate lipid, glucose, and energy metabolism by rapidly activating nuclear receptors and cell signaling pathways. This eBook summarizes the progress in the molecular and cellular mechanisms of cholesterol and bile acid metabolism and the physical-chemistry of biliary lipids, with emphasis on biliary lipid metabolism that is regulated by nuclear receptors in the hepatobiliary system.

Book excerpt: A volume on the biliary system appeared in 1965, based on a symposium of the NATO Advanced Institute held in September, 1963, in Newcastle upon Tyne in England. It soon became an authorita tive text on the problems of the biliary tract and, for instance, the discussion on the biliary secretion of organic anions, organic cations and inorganic ions were classic references. The worn pages of the volume in many libraries, including my own, bespeak its usefulness over ten long years. The initiative and energy of the Director of the first Institute have to be admired, even more so since he was able to assemble a Second Institute in Aalborg, Denmark, in 1975. His wisdom is reflected in the selection of the subjects discussed. The comparison between the two volumes tells something about the advancement in the time interval in knowledge about liver and biliary tract diseases, about the turnover of researchers interested in the liver and its diseases, and also about the present philosophy in biologic science. For this com parison, this volume is particularly suited because, in contrast to many other recent conferences, the more leisurely pace of the NATO Institutes permits more comprehensive but still scholarly reviews of the problems. The foreword of the first volume, written by my colleague, the late Harry Sobotka, expresses his, now dated, astonishment about the progress of knowledge of biology of the bile, so much less accessible to study than urine.

The present volume is devoted to the pathophysiology of bile acids. As the role of bile acids in health and disease is being increasingly recognized, we have chosen for discussion a wide range of topics of current importance. The presence of bile acids in brain tissue and their possible role in demyelinating diseases form the subject of a provocative discussion. As an extension of this theme, the presence and quantification of bile acids in extrahepatic tissues is the subject of one chapter. The pathophysiological implications of bile acids at the macromolecular level is highlighted by a chapter on the influence of bile salts on the activity of various enzymes. The general area of hepatobiliary diseases is discussed in two chapters: one describes changes in bile salt metabolism in liver diseases and the other focuses on cholesterol gallstones and their formation and dissolution. Although originally mainly physiological data on bile acid synthesis and transport were emphasized, and later on also the therapeutic benefit of bile acids in gallstone disease and cholestasis was discovered, we have come now to the molecular biology and genetic era with major discoveries in transport defects and related diseases. This book is the proceedings of Falk Symposium No. 120, held in The Hague, The Netherlands, on October 12-13, 2000 - the 16th International Bile Acid Meeting. One of the main discoveries recently has been the identification of nuclear receptors for bile acids, which gives them a much broader perspective than previously anticipated. It even suggests that bile acids can regulate their biosynthesis and enterohepatic circulation transcriptionally. It will therefore not be surprising that this topic, together with the molecular regulation of cholesterol 7alpha-hydroxylase and cholesterol homeostasis, has a dominant place in these proceedings. Another important topic is the progress in our molecular understanding of hepatic (both at the basolateral and canalicular sites), cholangiocytic and intestinal bile acid transport processes. Further insights into genetic defects causing cholestasis or intestinal malabsorption in animal models and in human diseases are also discussed by a number of well-known authors. This book, the proceedings of a Falk Workshop held in Freiburg, Germany, on June 2, 2002, is certainly unable to give any definite answers to any of the hundreds of still remaining questions in this fascinating field, but hopefully it will help to stimulate and initiate cooperation among immunologists, bile acid researchers, gynaecologists and internists.
