Book excerpt: The amount of patients surviving severe brain injury has gradually increased over these last decades thanks to the development of intensive care. These patients either recover quickly from coma or go through prolonged disorders of consciousness such as vegetative state/unresponsive wakefulness syndrome (VS/UWS) or minimally conscious state (MCS). While patients in a minimally conscious state are to some extent aware of themselves and the environment, and show fluctuating but reproducible signs of consciousness, patients in a vegetative state/unresponsive wakefulness syndrome are awake but only show reflexive behaviors. These patients are unable to communicate and present vigilance fluctuation, sensory deficits as well as severe motor and language impairments. Even though behavioral assessment currently remains the gold standard for diagnosis, a number of studies highlights the difficulty in making the distinction between conscious and unconscious patients based on clinical examinations and show a misdiagnosis rate as high as 40%. Misdiagnosis can nevertheless have serious consequences on patient’s management, medically but also ethically (i.e., regarding end-of-life decision). The emergence of functional neuroimaging techniques (such as positron emission tomography – PET and functional magnetic resonance imaging – fMRI) opened new opportunities to study brain activity in patients with disorders of consciousness (DOC). Recent neuroimaging tools have brought new understandings on how the brain functions after coma, and how these tools can complement bedside assessment by improving diagnosis and prognosis of DOC patients. Indeed, we know now that most patients in VS/UWS present partial activation of sensory networks and impaired functional connectivity contrary to patients in MCS. The reemergence of thalamo-cortical connections has also been associated with recovery of consciousness; whereas, thalamic atrophy has been associated to chronic DOC. Recent findings have also stressed the interest of neuroimaging in the management and the treatment of these patients. fMRI has been used as brain computer interfaces to detect consciousness in unresponsive patients and to allow basic communication in minimally conscious patients. Neuroimaging techniques are also currently being used to examine the effects of potential therapy such as pharmacological medications (e.g., Amantadine or Zolpidem) and brain stimulations (e.g., transcranial direct current stimulation or repetitive transcranial magnetic stimulation).

Disorders of consciousness (DOC) is an umbrella term that encompasses patients who present with disorders across a continuum of consciousness including people who are in a coma, in vegetative state (VS)/have unresponsive wakefulness syndrome (UWS), and in minimally conscious state (MCS). Technological developments in recent years, resulting in improvements in medical care and technologies, have increased DOC population numbers, the means for investigating DOC, and the range of clinical and therapeutic interventions under validation. In neuroimaging and behavioural studies, the auditory modality has been shown to be the most sensitive in diagnosing awareness in this complex population. As misdiagnosis remains a major problem in DOC, exploring auditory responsiveness and processing in DOC is, therefore, of central importance to improve therapeutic interventions and medical technologies in DOC. In recent years, there has been a growing interest in the role of music as a potential treatment and medium for diagnosis with patients with DOC, from the perspectives of research, clinical practice and theory. As there are almost no treatment options, such a non-invasive method could constitute a promising strategy to stimulate brain plasticity and to improve consciousness recovery. It is therefore an ideal time to draw together specialists from diverse disciplines and interests to share the latest methods, opinions, and research on this topic in order to identify research priorities and progress inquiry in a coordinated way. This Research Topic aimed to bring together specialists from diverse disciplines involved in using and researching music with DOC populations or who have an interest in theoretical development on this topic. While providing detailed background for neurological and neurosurgical specialists, the practical nature of the material in this book has found its greatest use among Internists, Emergency Medicine, and Intensive Care specialists, who deal with comatose patients frequently, but who may not have had extensive neurological training.

Book excerpt: The aim of this Research Topic is to discuss the state of the art on the use of Information-based methods in the analysis of neuroimaging data. Information-based methods, typically built as extensions of the Shannon Entropy, are at the basis of model-free approaches which, being based on probability distributions rather than on specific expectations, can account for all possible non-linearities present in the data in a model-independent fashion. Mutual Information-like methods can also be applied on interacting dynamical variables described by time-series, thus addressing the uncertainty reduction (or information) in one variable by conditioning on another set of variables. In the last years, different Information-based methods have been shown to be flexible and powerful tools to analyze neuroimaging data, with a wide range of different methodologies, including formulations-based on bivariate vs multivariate representations, frequency vs time domains, etc. Apart from methodological issues, the information bit as a common unit represents a convenient way to open the road for comparison and integration between different measurements of neuroimaging data in three complementary contexts: Structural Connectivity, Dynamical (Functional and Effective) Connectivity, and Modelling of brain activity. Applications are ubiquitous, starting from resting state in healthy subjects to modulations of consciousness and other aspects of pathophysiology. Mutual Information-based methods have provided new insights about common-principles in brain organization, showing the existence of an active default network when the brain is at rest. It is not clear, however, how this default network is generated, the different modules are intra-interacting, or disappearing in the presence of stimulation. Some of these open-questions at the functional level might find their mechanisms on their structural correlates. A key question is the link between structure and function and the use of structural priors for the understanding of the functional connectivity measures. As effective connectivity is concerned, recently a common framework has been proposed for Transfer Entropy and Granger Causality, a well-established methodology originally based on autoregressive models. This framework can open the way to new theories and applications. This Research Topic brings together contributions from researchers from different backgrounds which are either developing new approaches, or applying existing methodologies to new data, and we hope it will set the basis for discussing the development and validation of new Information-based methodologies for the understanding of brain structure, function, and dynamics.
