A common complaint has been that our system is unfair, a concern that comes from providers as well as consumers and from both public and private sectors. This bookaims to develop a framework for measuring various health reform proposals and current trends in relation to underlying social values in the U.S. In so doing, it seeks to expose social values that are at stake in current and future changes. At the heart of this book is the question: If the current situation is perceived to be unfair, will the future improve or worsen our situation? Targeted primarily for policy makers in government and throughout the health sector, this book will also stimulate graduate students in the health and social sciences. A wide audience will find the book of interest in providing a different perspective as to how current trends and specific legislative and policy proposals stack up against the authors' ten benchmarks of fairness. The book makes very limited use of illustrations, although tables provide understandable summaries of the concepts and their application in scoring proposals and trends. References are ample and pertinent. This is a stimulating and provocative work that shifts our focus to the collective social values at stake in an evolving health system. The book argues that our current system is unfair both in comparison to our values and the approaches taken throughout the rest of the industrialized world. Its sobering message is that the gap between what we value and what we have will likely increase until we recognize what is at stake.

Book excerpt: This paper challenges the fitness of Angela Ballantyne's proposed theory of exploitation by situating her 'fair risk account' in an ongoing dialogue about the adequacy conditions for benchmarks of fairness. It identifies four adequacy conditions: (1) the ability to focus on level rather than type of benefit; (2) the ability to focus on micro-level rather than macro-level fairness; (3) the ability to prevent discrimination based on need; and (4) the ability to prescribe a certain distribution as superior to all others. While the fair risk account satisfies the first condition, this paper argues that it has difficulty satisfying the last three conditions. Ballantyne's proposal includes several new and promising features, but in order for the fair risk account to be useful in identifying and preventing exploitation, Ballantyne must either clarify and augment her theory or challenge the relevance of the adequacy conditions it fails to meet.

Participants in two studies, one consisting of 208 participants and the other of 393 participants, evaluated the fairness of common relative and absolute rating formats. The participants in the second study also evaluated the fairness of two rating formats, one absolute and one relative, presented in organizational contexts of varying procedural and distributive justice. Results indicate that not only are absolute formats perceived as more fair than relative formats, but differences in fairness perceptions also occur among relative and absolute formats. Furthermore, it appears that rating format influences procedural justice, especially when outcomes are perceived as fair. Implications for organizations' appraisal practices are discussed.
