Book excerpt: The permanent effects of traumatic brain injury (TBI) are not limited to the person who suffers the injury. People who care for the individual, particularly family members, suffer in various ways. Family members are often confused as to the behavioral and neuropsychological changes that they see in a brain-injured rela tive. They can become frustrated and angry when the individual does not return to premorbid levels of functioning. They can become tired and worn down from repeated problems in trying to manage the individual's difficulties while having only fragmented information regarding them. Drs. Smith and Godfrey have provided a useful service for family members by summarizing important neuropsychological changes associated with TBI and providing practical guidelines for coping with these problems. While the neuropsychological problems they describe are not completely understood, the authors provide a useful description of many of the neuro behavioral problems seen following TBI in young adults. They attempt to provide guidelines for family members that have practical utility in understanding and managing these patients. Theirs is a cognitive-behavioral approach that can have utility for this group of individuals. I applaud their efforts to provide something systematic and practical for family members.

Then comes a section on specific groups of people (children, people with dementia and people in reduced states of awareness. Although the main focus of the book is on adults with non-progressive brain injury, these other groups are included as NR is being increasingly employed with them. The book concludes with a chapter on systems of service delivery and another on the future of NR. Thus this book covers a number of aspects of NR and is broader in outlook than most existing books in this area. It presents current practice techniques in cognitive rehabilitation from a conceptual and theoretical perspective. It offers both clinicians and researchers a sense of the research and theory underlying current clinical applications. The main audience will be clinical neuropsychologists especially those working in rehabilitation. Other audiences include clinical psychologists working with people who have mental health problems, schizophrenia or are elderly; occupational therapists; speech and language therapists and rehabilitation doctors. A search for relevant experimental evidence to support the clinical use of behavioral principles for retrain ing, which began in 1977, turned up little empirical support. Moreover, the literature on retraining was dispersed among a variety of journals published in various countries. Nowhere was there a compendium of literature that addressed issues of assessment and retraining. There was no place to turn if one wanted to move from a standard neuropsy chological evaluation to the retraining of skill deficits revealed in the evaluation. We have attempted to edit a book that represents what we had hoped to find in the literature and could be used by professionals in clinical psychology, clinical neuropsychology, rehabilitation medicine, physical therapy, speech therapy, and other disciplines that address rehabilitation of brain-damaged adults-a book that addresses assess ment and rehabilitation issues and is sufficiently detailed to offer the reader a starting point in developing behavioral assessment and re habilitation programs. The book contains conceptual foundations, re views of research, descriptions of successful rehabilitation programs, and relatively detailed approaches to the retraining of specific skills. A shift from an assessment-based practice to one encompassing both prescriptive assessment and treatment has become a recognized transition in the neuropsychological literature and was best articulated in an article by Gerald Goldstein in March of 1979.

In any society patients who have suffered a stroke or a traumatic brain lesion constitute a large group requiring special therapy; even nowadays only a small group of these patients obtain adequate rehabilitational support. Brain injuries may lead to loss or impair ment of functions like language, sight, memory, attention, emo tional control, or movement, and such impairments are usually ac companied by handicaps in the patient's daily life. Every attempt should be made to improve functional competence and the pa tient's capacity to cope with their disability and handicap. In recent years, the aim of much research in the basic sciences has been to gain insight into the mechanisms of restitution of function, partly by trying to understand the pathophysiological mechanisms that are initiated by a traumatic event. However, in spite of this broad research initiative into recovery of function and the possibilities of cognitive remediation, our knowledge is still rather limited, with respect to both the neurobiological mecha nisms that may underlie functional plasticity and the factors that may account for neuropsychological rehabilitation. In spite of these shortcomings, we would like to stress that progress can only be expected if an intense research effort is made to unite the con cepts and results from the basic sciences with the practical demands of neuropsychological rehabilitation.
