Book excerpt: Cytological screening for the identification of intraepithelial neoplasia of the cervix as a precursor lesion for cervical cancer has been well established as an effective means for decreasing the incidence of invasive carcinoma. Despite these screening efforts, carcinoma of the cervix remains one of the more common malignancies in women and it is the leading cause of cancer death in many countries in the western hemisphere. It is estimated that in 1986 there will still be 14,000 new cases of invasive cancer, with 6,800 deaths in the United States alone. Unfortunately, many of these patients present with advanced disease, posing difficult management problems for the clinician responsible for their care. The treatment of early stage invasive carcinoma of the cervix (lesions confined to the cervix and vagina) remains either radical surgery, radical radiation therapy or a combination thereof This approach is extraordinarily effective in the vast majority of patients. However, there remains a subset of patients with early stage disease that are at high risk for recurrence. Dr Kjorstad (Chapter 2) has identified adenocarcinomas and adenosqua mous carcinomas as having a particularly poor prognosis. In addition, patients with more than three positive lymph nodes or with involvement of lymph nodes outside of the pelvis have a very poor prognosis. He has iden tified the CEA as a potentially predictive marker for these patients with poor prognosis, especially in patients with adenocarcinomas.

Data on how complications affect survivors' quality of life could be useful when making treatment decisions. MANUSCRIPT I - Quality of Life in Cervical Cancer Survivors: Patient and Provider Perspectives on Common Complications of Cervical Cancer. BACKGROUND: Perceived impacts on survivors' quality of life (QOL) influence cervical cancer treatment decisions. OBJECTIVES: This study's objective was to quantify the value of common complications of early cervical cancer treatment, to be used as utilities in a decision model. Utilities assigned by survivors were compared to those assigned by providers. METHODS: 30 survivors of stage I/II cervical cancer identified from our Tumor Registry and 10 gynecologic oncology providers were interviewed. Participants evaluated complications (health states) using the standard gamble (SG) and visual analogue scale (VAS). Mixed-effects linear models were used to generate confidence intervals for utility means, and evaluate the effect of group (survivors versus providers). Higher utilities indicate the health state is closer to perfect health. RESULTS: Survivors' and providers' mean ages were 44 and 40 respectively. Most survivors had no evidence of disease and 56% self-identified as having had complications. Using SG, providers consistently assigned utilities 7% higher than survivors (p=0.035) for all health states except "ileostomy", which survivors rated higher than providers. Providers rated ileostomy lowest. Survivors assigned the lowest utilities to small bowel obstruction (fixable without an ostomy) and ureteral obstruction. Personal history of complications or higher stage did not have a consistent effect on utilities. There was no consistent difference between survivors and providers on the VAS (p=0.45). DISCUSSION: Providers assign higher utilities than survivors to health states. Providers and survivors diverge on which complications impact QOL the most. Data on patient preferences should be considered when weighing treatment options with similar survival but different associated complications. MANUSCRIPT II - Stage IB2 Cervical Cancer: A Decision Analysis Comparing Quality Adjusted Survival Associated with Chemoradiation Versus Radical Surgery With Tailored Adjuvant Chemoradiation. BACKGROUND: Optimal treatment of Stage IB2 cervical cancer is unclear. Treatment side effects pose serious survivorship issues. We hypothesized that increased long-term side effects associated with surgery plus tailored chemoradiation (RS-tCR) would make the quality adjusted survival (QAS) associated with primary chemoradiation (CR) greater than that of RS-tCR. METHODS: A Markov transition model was created comparing CR to RS-tCR in 45 year old women with IB2 cervical cancer. Surgical patients who met high (positive lymph nodes and/or margins) or intermediate (deep stromal and/or lymph vascular space invasion) risk criteria received tCR. Outcomes were overall survival (OS) and quality-adjusted life years (QALY). Five year survival rates and probabilities were obtained by searching Pubmed, Cochrane Review, and Web of Science. Quality adjustments were made using utilities obtained by interviewing cervical cancer patients with the standard gamble. One-way sensitivity analyses were performed. RESULTS: OS was slightly improved with RS-tCR versus CR (29.79 vs 29.01 years). QAS was almost identical (28.23 vs 28.11 QALY). QAS was sensitive to small changes in survival rates.
