Angiogenesis can cause constriction and squeezing of the urethra, resulting in an array of urinary symptoms. Hypothesis: Although the precise etiology of benign prostatic hyperplasia (BPH) remains elusive, hormonal fluctuations, specifically the impact of testosterone and its derivative dihydrotestosterone, are thought to play a role. BPH is significantly influenced by aging and is significantly more prevalent in older males. BPH is a prevalent condition among older men, and its incidence rises as age increases. Although benign and not associated with an increased risk of prostate cancer, benign prostatic hyperplasia (BPH) can substantially impair an individual's quality of life due to its distressing symptoms. Medication, lifestyle modifications, and, in more severe instances, surgical procedures to remove or reduce the size of the prostate may be utilized to treat BPH. BPH symptoms necessitate that affected individuals seek the advice of a medical professional in order to obtain an accurate diagnosis and develop an appropriate treatment plan.

A number of factors have led to this discussion. In an increasing proportion of aging men, for example, BPH causes so-called obstructive symptoms that must be relieved by medical or operative means. This entails an immense social and economic impact in terms of health costs. In addition, recent data indicate the most frequently performed operation for BPH - transurethral resection of the prostate - is associated with a higher risk of death due to cardiac disease than open prostatectomy. Furthermore, studies using the recently developed technique of uro dynamics to assess bladder outflow obstruction reveal that about 20% -30% of patients treated with transurethral resection or open prostatectomy are actually not obstructed. This means that these patients do not receive the most effective therapy. Book excerpt: In 1963 Professor Huggins! wrote "more than one half of the male population over the age of 50 suffer from benign tumors known as prostatic hypertrophy. Often an enlarged prostate is the only difficulty to cloud an otherwise tranquil old age". This statement emphasizes two important features of benign prostatic hypertrophy (BPH); the frequency with which it occurs and its association with increasing age. The true incidence is difficult to determine as most data relate to selected groups of patients; moreover, the diagnostic criteria of prostatic hypertrophy are not clearly defined. Normality merges imperceptibly into abnormality, for even in men who are clinically normal, rates of urinary flow decrease with 2 increasing age . Despite these inaccuracies, incidence figures show the magnitude of the problem; Flocks (1963) 3 reported that 65% of American men over 60 years suffer from it and in a selected group of Danish men 43% had symptoms of the 4 disease ; similar figures are reported for the U ni ted Kingdom 5. At present most patients with BPH, who need treatment, undergo surgical prostatectomy which may be considered the usual treatment. The frequency of the disease alone, creates certain problems, for annually more than 30000 men are admitted to hospital in the United States with this as a primary diagnosis and with an ageing population the figures can be expected to increase.
